La incompetencia cervical (IC) se caracteriza por la dilatación progresiva del cuello uterino en ausencia de contracciones uterinas. Esta patología es uno de los principales motivos de pérdidas gestacionales precoces y de los nacimientos ocurridos pretérmino. Se trata de la dilatación del cuello uterino, que es la porción del útero que se encarga de mantener el embarazo hasta el término, con una medición promedio normal de 35 a 40 milímetros, aunque esta dilatación no se asocia a contracciones dolorosas y percibidas por la gestante. Clínicamente se puede manifestar por diferentes signos como la dilatación del cérvix, el acortamiento cervical y el inicio pretérmino del trabajo de parto. Además, se encuentra relacionado con la ruptura prematura de membranas y la corioamnionitis. Para tratar esta patología, evitar pérdidas en el segundo trimestre y desencadenamiento del trabajo de parto de forma prematura, se pueden utilizar diferentes técnicas, tales como la colocación de un pesario, la administración de progesterona y la realización de un cerclaje cervical. Ésta última es la técnica de elección para el tratamiento de la incompetencia del cérvix, siendo indicado por malos antecedentes obstétricos, como método terapéutico en caso de objetivarse por ecografía modificaciones cervicales sugestivas de inicio del trabajo de parto, o bien de urgencia cuando la gestante muestra dilatación cervical y/o exposición de las membranas. Palabras clave: Cerclaje, incompetencia cervical, cervicometría. ABSTRACT Cervical incompetence (CI) is characterized by progressive dilation of the cervix in the absence of uterine contractions. This pathology is one of the main reasons for early gestational losses and preterm births. It is the dilation of the cervix, which is the portion of the uterus that is responsible for maintaining the pregnancy until term, with a normal average measurement of 35 to 40 millimeters, although this dilation is not associated with painful contractions and perceived by the pregnant. Clinically it can be manifested by different signs such as dilation of the cervix, cervical shortening and preterm onset of labor. In addition, it is related to premature rupture of membranes and chorioamnionitis. To treat this pathology, avoid second-trimester losses and trigger premature labor, different techniques can be used, such as the placement of a pessary, the administration of progesterone and the performance of a cervical cerclage. The latter is the technique of choice for the treatment of incompetent cervix, being indicated due to poor obstetric history, as a therapeutic method in case cervical changes suggestive of the onset of labor are detected by ultrasound, or emergency when the pregnant woman shows cervical dilatation and/or exposure of the membranes.
