RESUMOOBJETIVO: correlacionar a hipertrigliceridemia pós-prandial com conhecidos fatores de risco para aterosclerose. MATERIAL E MÉTODOS: Foram estudados 47 pacientes não diabéticos (30 mulheres e 17 homens, idade: 40,5 ± 14,9 anos, IMC: 26,1 ± 5,4kg/m 2 ) com trigliceridemia de jejum normal (<200mg/dl). Triglicerídeos, HDL e colesterol total foram medidos nos tempos 0, 3 e 5 horas após ingestão de aproximadamente 70g de gordura (200g de creme de leite a 25% e 2 gemas), sendo então avaliados vários parâmetros. RESULTADOS: Pacientes com pico de triglicerídeos de 3 horas maior que o 2º quartil (164,8mg/dl), apesar de triglicerídeos de jejum normais, apresentaram maior IMC (28,1 ± 5,6 vs. 24,2 ± 4,5kg/m
RESUMOObjetivos: Avaliar os fatores associados com persistência ou recorrência do hipertireoidismo após tratamento com 131 I. Metodologia: Análise retrospectiva de prontuários de todos os pacientes com hipertireoidismo tratados com dose fixa de 400MBq de 131 I em um centro especializado (1992 a 1997 From the 104 patients treated during that period, six had a multinodular goitre and one, a toxic adenoma; 96 were taking thionamide preparations, which were discontinued 7 days prior to radioiodine administration. Results: Early progression to hypothyroidism was observed at a median of 88 days (22-214) in 60 patients (57.7%). Persistence or early recurrence of hyperthyroidism was observed in 13 (12.5%) and was positively correlated with age (p<0.007), although unrelated to underlying diagnosis. There was a higher prevalence of persistent disease in those treated with propylthiouracil rather than carbimazol prior to radioiodine (35% vs. 8%; p<0.006). Conclusions: This study suggests that the blocking effect of thionamides on radioiodine uptake may be more prolonged in older people. It also suggests that the effect of propylthiouracil may last longer than that of carbimazol. O uso de corticosteróides é um importante fator no desenvolvimento de insuficiência adrenal secundária, não sendo a via nasal considerada por muitos autores como capaz de causar esta doença. A rinite alérgica é uma condição muito comum na qual é utilizado corticosteróide nasal sem o associar aos possíveis efeitos sistêmicos. Os autores descrevem dois casos de insuficiência adrenal secundária devido ao uso de dexametasona nasal. Alguns aspectos desta condição são discutidos.
RESUMOObjetivo: Determinar a variação da CPK sérica em função do TSH e T4 livre (FT4) e avaliar a CPK sérica nos estados patológicos da tireoide (hipertireoidismo e hipotireoidismo), em relação ao eutireoidismo. Materiais e métodos: Analisaram-se, retrospectivamente, 6.230 exames laboratoriais de 2007 a 2011, com as dosagens de TSH e CPK. Destas, 3.369 com dosagens de FT4. Avaliou-se a correlação entre CPK com TSH e com os estados patológicos da tireoide. Resultados: A correlação entre TSH e CPK foi positiva (r = 0,065), enquanto entre CPK e FT4 foi inversamente proporcional (r = -0,091, p < 0,05). Do total, 586 (9,4%) medidas foram de hipertireoidismo com mediana (intervalo) do CPK de 98 U/L (27 a 1.113) e 556 (8,9%), hipotireói-deos com CPK de 114 U/L (25-4.182). Conclusão: Houve correlação positiva entre CPK sérica e TSH e negativa com FT4, sendo a CPK menor no grupo com hipertireoidismo e maior no grupo de hipotireoidismo.
