El reconocimiento temprano de un cuadro séptico debe ser uno de los principales objetivos en el momento de la atención de un paciente pediátrico, la instauración del tratamiento temprano representa la oportunidad para obtener una mayor posibilidad de supervivencia. Aunque aún existen pocos estudios en el manejo específico en la población infantil, la detección e intervención debe adaptarse a la edad, la gravedad y en el presunto origen infeccioso siendo individualizado el tratamiento. La siguiente revisión permite profundizar en los conocimientos básicos sobre la sepsis y el shock séptico en pediatría con la finalidad de promover una detección temprana y así destacar las medidas efectivas para un adecuado manejo.
La Fiebre es uno de los motivos de consulta más frecuentes en Pediatría, la mayoría de veces se trata de procesos infecciosos banales. Al conocer la epidemiología según los grupos etarios, las vacunas recibidas, acompañado de una anamnesis y examen físico detallado podremos identificar pacientes que estén en riesgo de infecciones bacterianas graves (IBG) la misma que puede presentarse de un 7-11% en niños con fiebre, e instaurar el tratamiento adecuado y temprano. Un niño podría presentar en promedio de 4 a 6 episodios de fiebre al año. Aproximadamente en el 20% no se encontrará un foco después de la anamnesis y exploración física minuciosa.
Las enfermedades diarreicas son consideradas como la segunda causa de muerte en población infantil, especialmente menor a cinco años. La Organización Mundial de la Salud (OMS) define a la diarrea como la deposición, tres o más veces al día (o con una frecuencia mayor que la normal para la persona) de heces sueltas o líquidas. La deposición frecuente de heces formes (de consistencia sólida) no es diarrea, ni tampoco la deposición de heces de consistencia suelta y “pastosa” por bebés amamantados.
