Prevalence of NASH among patients with gallstones is lower than estimated previously, but NASH is frequent particularly in those patients with concurrent metabolic syndrome. The combination of an increased HOMA score with fatty liver on ultrasound has a good accuracy for predicting NASH in patients with gallstones.
Background Sodium-glucose cotransporter 2 inhibitors (SGLT2i) lower cardiovascular events in type 2 diabetes mellitus (T2DM) patients, although the mechanisms underlying these benefits are not clearly understood. Our aim was to study the effects of SGLT2i on left ventricular remodelling and longitudinal strain. Methods Between November 2019 and April 2020, we included 52 patients with T2DM ≥ 18 years old, with HbA1c between 6.5 and 10.0%, and estimated glomerular filtration ≥ 45 ml/min/1.73 m2. Patients were classified into SGLT2i group and control group, according to prescribed treatment by their referring physician. Conventional and speckle tracking echocardiography were performed by blinded sonographers, at baseline and after 6 months of treatment. Results Among the 52 included patients (44% females, mean age 66.8 ± 8.6 years, mean HbA1c was 7.40 ± 0.7%), 30 patients were prescribed SGLT2i and 22 patients were classified as control group. Mean change in indexed left ventricular mass (LVM) was − 0.85 ± 3.31 g/m2 (p = 0.003) in the SGLT2i group, and + 2.34 ± 4.13 g/m2 (p = 0.58) in the control group. Absolute value of Global Longitudinal Strain (GLS) increased by a mean of 1.29 ± 0.47 (p = 0.011) in the SGLT2i group, and 0.40 ± 0.62 (p = 0.34) in the control group. We did not find correlations between changes in LVM and GLS, and other variables like change in HbA1c. Conclusions Among patients with T2DM, SGLT2i were associated with a significant reduction in indexed LVM and a significant increment in longitudinal strain measured by speckle tracking echocardiography, which may explain in part the clinical benefits found in clinical trials.
Resumen El libro de texto es un recurso ampliamente usado en la práctica de la enseñanza y, en consecuencia, es objeto de investigación educativa, aplicando diversas perspectivas teóricas. En este trabajo se ejemplifica el uso de las herramientas teóricas del Enfoque Ontosemiótico del conocimiento y la instrucción matemáticos para realizar el análisis didáctico de una lección sobre proporcionalidad de sexto curso de primaria (11-12 años de edad). Se considera que una lección refleja el proceso de instrucción planificado por el autor como medio para lograr el aprendizaje del contenido por parte de los estudiantes potenciales del mismo. En consecuencia, se plantean las siguientes cuestiones que guían el análisis: 1) ¿Cuál es el grado de idoneidad didáctica del proceso de instrucción planificado? 2) ¿Cómo gestionar el uso del texto para incrementar la idoneidad del proceso? El método de análisis implementado permite obtener conocimientos didáctico-matemáticos que pueden ayudar al profesor a tomar decisiones sobre las posibilidades y limitaciones de los libros de texto cuando se asumen los postulados socioculturales sobre el aprendizaje matemático.

Actualmente, los costos de una vivienda para una familia pobre superan el precio máximo que establece el programa de vivienda social. Estos costos, más altos que nunca antes en la historia del financiamiento habitacional, superan el valor de la vivienda para sectores medios bajos. Además, en los últimos años el monto del subsidio estatal aumenta a más del doble y, aunque contempla un financiamiento complementario para localización, no alcanza a cubrir los costos de los proyectos de vivienda mejor ubicados en la ciudad que requieren recursos adicionales.En la práctica, el aumento del subsidio ha cubierto el incremento histórico del valor del suelo y de algunos materiales de construcción y no se traduce ni en mejoras de la calidad de la edificación o del diseño, ni en una mayor superficie o en una mejor localización. En realidad, los mayores fondos se transfieren directamente a la industria inmobiliaria y de la construcción.Así, la política chilena de financiamiento que fomenta la propiedad privada de la vivienda y que se basa en subsidios, no es viable a largo plazo. Primero porque en las ciudades ya no hay suelo asequible por los montos que el Estado está dispuesto a aportar. De hecho, se ha comprobado que en Santiago el precio de los terrenos aumenta proporcionalmente mucho más en las comunas donde se construyen viviendas sociales (Brain y Sabatini, 2006). Segundo, porque actualmente los valores a los que llega la vivienda social alcanzan los precios de la vivienda para sectores medios aunque nadie lo denuncie abiertamente, de modo que el producto vivienda social tradicionalmente aceptado como una vivienda de menor estándar por su bajo costo, ya no es válido. Tercero, porque como el precio de la vivienda ha aumentado y el aporte de las familias ha disminuido, además de emplearse para cubrir el alza del precio del suelo y de la construcción, el subsidio se ha destinado a financiar el monto que antes cubría el crédito hipotecario otorgado a las familias.Entonces, en las condiciones actuales, la propiedad de la vivienda como única alternativa es insostenible. Por estas razones no es aceptable seguir expulsando a los pobres fuera de la ciudad. No se justifica financiar prototipos de vivienda de menor estándar que cuestan lo mismo que viviendas destinadas a otros sectores socioeconómicos y, por último, el Estado ya no está en condiciones de seguir aumentando el subsidio con fondos que van en directo beneficio de los propietarios del suelo y de las empresas inmobiliarias, fomentando la especulación. Es imprescindible buscar otras soluciones que le permitan a la gente vivir en barrios socialmente integrados, bien servidos y seguros: arrendamiento con compra a largo plazo, arrendamiento mediante subsidios, cooperativas que aseguren el acceso a la vivienda a las familias que no tienen otras opciones. Achieving the competence to reflect on the degree of suitability of textbooks is not easy, so it is necessary to train prospective professionals in the knowledge and use of analytical tools to guide them in critically analyzing these curricular materials. In response to this research problem, this paper describes and analyses the implementation of a training intervention that aims to promote reflective competence in prospective Costa Rican teachers through the analysis of textbook lessons. The content of proportionality is chosen because of its central in the curriculum. The training activity was motivating for the participants and helped them to progress in the competence of lesson analysis. However, it is noted that the design needs to be improved in terms of time dedication, as well as attention to the necessary didactical-mathematical background knowledge on proportionality to achieve better results.
