La leishmaniasis es una enfermedad cutánea causado por un parásito del género leishmania. En la mayoría de los casos se trata de una zoonosis transmitida por la picadura de un vector. Los reservorios son animales peri domésticos o salvajes. Constituye un problema de salud pública en Costa Rica y es considerada una enfermedad del estrato social bajo. Presenta 3 formas clínicas: la visceral, mucocutánea y cutánea; esta última siendo la más común y en la que se basa este artículo. El diagnóstico requiere la sospecha clínica y la identificación del parasito, la microscopía de tejidos con el fin de visualizar el parasito es la técnica diagnostica más usada en el mundo y en Costa Rica. El tratamiento de primera línea son los antimonios pentavalentes, sin embargo, existen otras alternativas tanto como tratamientos locales y sistémicos.
La displasia fibrosa es una patología ósea benigna causada por una anomalía genética del gen GNSA1. Se caracteriza por el reemplazo de tejido óseo normal por tejido fibroso. Se presenta en dos modalidades; la monostótica que afecta un solo hueso y poliostótica que afecta a múltiples huesos con predilección por los huesos largos y craneofaciales. La presentación y manifestación clínica es variable y depende de la ubicación y la extensión de las lesiones esqueléticas; va desde lesiones solitarias incidentales sin relevancia clínica, hasta enfermedad grave asociada a deformidad, fracturas, dolor y deterioro funcional. La patología ósea podría ocurrir aislada o asociada a manifestaciones endocrinas. El síndrome de McCune-Albright se caracteriza por la tríada de displasia fibrosa poliostótica, pubertad precoz y manchas café con leche. El diagnostico se basa en las características clínicas, radiológicas e histopatológicas y en la mayoría de los casos, el diagnóstico puede ser clínico mediante una adecuada evaluación de las características esqueléticas, endocrinas y dermatológicas. El tratamiento puede ser médico o quirúrgico. Y según la severidad de la presentación clínica y en casos severos se requiere de manejo multidisciplinario. El manejo quirúrgico incluye múltiples modalidades y tiene como objetivo prevenir, manejar las fracturas y minimizar las deformidades.
El cáncer de cérvix es un importante problema de salud pública en los países en vías de desarrollo, lo que representa un desafío actual por su alta mortalidad, con un 90 por ciento % de muertes por esta causa, la cual ocurre en países de ingresos bajos a medianos. El cáncer de cuello uterino comprende el 8 por ciento de todos los cánceres entre las mujeres y es el cuarto cáncer más común en las mujeres después de los cánceres de mama, colon y pulmón. El carcinoma de células escamosas junto con el adenocarcinoma constituye más del 98% de todos los tipos de cáncer de cérvix. Aproximadamente un tercio de los tumores cancerosos están relacionados con la inflamación inducida por un microorganismo, en este caso el cáncer de cérvix guarda una estrecha relación con el virus del papiloma humano, en especial con los tipos 16 y 18 involucrados en el 99% de este tipo de cáncer. Es importante destacar que la sintomatología del cáncer de cérvix generalmente se presenta en estadios tardíos de la enfermedad, por lo que la prevención con los métodos de tamizaje es esencial para diagnosticar tanto lesiones premalignas como las lesiones cancerosas.
La psoriasis es un trastorno crónico inmunitario que se desarrolla posterior a la exposición de una injuria en la piel; ya sea traumática, medicamentosa o infecciosa, en personas genéticamente predispuestas. Se presenta de forma clásica como placas confluentes eritematosas, escamas micáceas con bordes bien delimitados, sin embargo, existen otras formas clínicas cutáneas. Más allá de la afectación de la piel la psoriasis también se asocia con una serie de manifestaciones sistémicas e importantes comorbilidades médicas y psiquiátricas, que incluyen artritis psoriásica, enfermedades cardiovasculares, diabetes, depresión y ansiedad. Su tratamiento depende de la severidad del cuadro clínico que va desde preparaciones tópicas hasta tratamientos sistémicos, sin olvidar el manejo multidisciplinario de sus comorbilidades.
