ObjectiveTo know the association of birth order with the risk of morbidity and mortality in very low-birthweight (VLBW) twin infants less than 32 weeks’ gestational age (GA).DesignRetrospective cohort study.SettingInfants admitted to the collaborating centres of the Spanish SEN1500 neonatal network.PatientsLiveborn VLBW twin infants, with GA from 23+0weeks to 31+6weeks, without congenital anomalies, admitted from 2011 to 2020. Outborn patients were excluded.Main outcome measuresRespiratory distress syndrome (RDS), patent ductus arteriosus, bronchopulmonary dysplasia (BPD), necrotising enterocolitis, major brain damage (MBD), late-onset neonatal sepsis, severe retinopathy of prematurity, survival and survival without morbidity. Crude and adjusted incidence rate ratios were calculated.ResultsAmong 2111 twin pairs included, the second twin had higher risk (adjusted risk ratio (aRR) of RDS (aRR 1.08, 95% CI 1.03 to 1.12) and need for surfactant (aRR1.10, 95% CI 1.05 to 1.16). No other significant differences were found, neither in survival (aRR 1.01, 95% CI 0.99 to 1.03) nor in survival without BPD (aRR 1.02, 95% CI 0.99 to 1.05), survival without MBD (aRR 1.02, 95% CI 0.99 to 1.06) nor in survival without major morbidity (aRR 0.97, 95% CI 0.92 to 1.03). However, second twins born by caesarean section (C-section) after a vaginally delivered first twin had less overall survival and survival without MBD.ConclusionIn modern perinatology, second twins are still more unstable immediately after birth and require more resuscitation. After admission to the neonatal intensive care unit, they are at increased risk of RDS, but not other conditions, except for second twins delivered by C-section after a first twin delivered vaginally, who have decreased overall survival and survival without major brain injury.

The conceptual metaphor, developed within the framework of cognitive linguistics, constitutes a tool for both supporting and refuting trains of argument; it therefore enables a collection of polyphonic voices to be identified, reflecting opposing ideologies and representing their particular vision of the events under debate. From a corpus of texts belonging to the "hypergenre" of the social debate concerning immigration, we analyse the conceptual and linguistic metaphors that categorize the host country and the image of immigrants. Through a study of this imagery, we demonstrate that the choice of source domains and of conceptual structures generates a virtual dialogue between opposing ideologiessupporters and detractors of the phenomenon of immigration-and show that, in effect, metaphor plays an important role in the process of argument.KEYWORDS: argument; metaphor; immigration; ideology; debate.RESUMEN: La metáfora conceptual, desarrollada en el marco de la lingüística cognitiva, constituye una herramienta más al servicio de las operaciones de apoyo y refutación propias de la secuencia argumentativa y, por ello, permite reconocer un juego de voces polifónicas a través del cual se reflejan ideologías opuestas que representan su particular visión sobre los acontecimientos debatidos. A partir de un corpus de textos pertenecientes al hipergénero del debate social en torno a la inmigración, se analizan las metáforas conceptuales y lingüísticas que categorizan el país receptor de inmigrantes y la imagen que de estos se tiene. Mediante el estudio de esta imaginería se comprueba cómo la selección de los dominios fuente y de las estructuras conceptuales que generan propician el diálogo virtual entre ideologías opuestas, defensora y detractora del fenómeno de la inmigración, y se pone de manifiesto que, efectivamente, la metáfora desempeña una función relevante en la argumentación.

La elaboración de entornos accesibles desde el punto de vista cognitivo es un eje legislativo e institucional básico desde comienzos del siglo XXI. La metodología de Lectura Fácil permite elaborar textos accesibles para todas aquellas personas que presenten algún tipo de problema de comprensión lectora, en especial para personas con discapacidad intelectual o del desarrollo. La divulgación de esta metodología y el desarrollo de textos normativos sobre el procedimiento de trasvase de un texto origen a otro texto accesible no han traído consigo, sin embargo, una formación reglada que permita abordar la tarea de adaptación textual con las mayores garantías posibles en España. Por ello, en este artículo presentamos una propuesta de asignatura inserta en un título de posgrado perteneciente a los estudios de Traducción e Interpretación, en los que se abordan competencias traductológicas que son necesarias también para la adaptación de textos con el método de Lectura Fácil. Esto se debe, como explicaremos en las líneas que siguen, a que consideramos que las competencias asociadas a este título son, asimismo, competencias necesarias para la formación de profesionales que aborden el proceso de adaptación de textos con el método de LF.
