In 1996, 20 of 21 patients with mucosal leishmaniasis, treated in 1994 with aminosidine sulfate, 16mg/kg/day salt, by intramuscular injection for 20 days, were clinically evaluated. One patient died due to disease not related to mucosal leishmaniasis. Seven of 14 patients (66.7% N = 21) who achieved complete remission three months after treatment remained clinically cured 24 months later and seven relapsed in the same period (50% N = 14). Sorological follow-up showed poor correlation with the results of clinical examination.
Os esquemas utilizados são variáveis mas atualmente se recomenda a dose de 20mg/kg/dia de Sbv por 28 dias915. Este esquema terapêutico tem produzido resultados d ife re n te s em v á ria s re g iõ e s g e o g rá fic a s apresentando falta de resposta em considerável número de pacientes6721 22, fenômeno que pode ser atribuído em parte ao aparecimento de resistência ao medicamento8. O tratamento com antimoniais acompanhase de toxicidade importante que aparece mais frequentemente quando se utiliza por tempo prolon gad o213. R e co n h ece-se tam bém que estas drogas (Glucantime® e Pentostam®) são compostos cuja estrutura química não tem sido completamente caracterizada e a padronização do conteúdo de Sbv em cada lote e ampola poderia ser diferente dependendo de variáveis ainda não determinadas5. As alternativas nos casos de falta de resposta aos antimoniais pentavalentes são anfotericina B e pentamidina, ambas com níveis de toxicidade elevados que fazem impossível a aplicação nas condições das áreas rurais onde se concentra a maioria dos pacientes com LM2 u.Na região sudeste da Bahia existe uma área de transmissão quase exclusiva de L(V)b e na localidade de Corte de Pedra, no município de T ancredo N eves, há um cen tro de saúde que atende principalmente pacientes com le is h m a n io s e o n d e p e s q u is a d o r e s da Universidade de Brasília têm realizado estudos sobre esta doença4 10 20. A incidência de LM registrada no Centro de Saúde de Corte de Pedra é em média de 22 casos/ano.
