Purpose Investigate the association between levodopa therapy and vocal characteristics in Parkinson’s disease patients. Search strategy Studies published at MEDLINE, LILACS, and SciELO, from 1960 to December 2016. A systematic review and meta-analysis was performed using the following keywords: Parkinson’s disease; levodopa; L-dopa; voice; speech disorders; dysphonia; dysarthria. After analyzing titles and abstracts, two independent reviewers selected all clinical trials that met the eligibility criteria and selected the articles and the data recorded in a previously standardized table. Selection criteria Trials published in English between 1960 and December 2016 individuals with clinical diagnosis of Parkinson’s disease; use of levodopa therapy in stable doses; acoustic analysis combined or not with auditory-perceptual analysis to evaluate the vocal parameters under investigation. Data analysis The following vocal parameters were analyzed: fundamental frequency (F 0), jitter, and vocal intensity. Standardized mean differences (SMD) were calculated using the Comprehensive Meta-analysis V2 software. Results Nine articles met the eligibility criteria and were selected, with a total of 119 individuals. From these, six articles with 83 individuals were included in the meta-analysis. During the levodopa therapy “on” state, modifications in F 0 (SMD=0.39; 95% CI - 0.21-0.57) and jitter (SMD=0.23; 95% CI - 0.02-0.45) were observed. Vocal intensity was not affected (SMD=0.09; 95% CI - 0.22-0.39) by levodopa ingestion. Data of the included studies were controversial in the auditory-perceptual analysis of voice. Conclusion Levodopa therapy modifies F0 and jitter. No changes in vocal intensity were observed in either the “on” or “off” states of levodopa therapy.
Purpose
Smoothed cepstral peak prominence (CPPS) and harmonics-to-noise ratio (HNR) are acoustic measures related to the periodicity, harmonicity, and noise components of an acoustic signal. To date, there is little evidence about the advantages of CPPS over HNR in voice diagnostics. Recent studies indicate that voice fundamental frequency (F0) and intensity (sound pressure level [SPL]), sample duration (DUR), vowel context (speech vs. sustained phonation), and syllable stress (SS) may influence CPPS and HNR results. The scope of this work was to investigate the effects of voice F0 and SPL, DUR, SS, and token on CPPS and HNR in dysphonic voices.
Method
In this retrospective study, 27 Brazilian Portuguese speakers with voice disorders were investigated. Recordings of sustained vowels (SVs) /a:/ and manually extracted vowels (EVs) /a/ from Consensus Auditory-Perceptual Evaluation of Voice sentences were acoustically analyzed with the Praat program.
Results
There was a highly significant effect of F0, SPL, and DUR on both CPPS and HNR (
p
< .001), whereas SS and vowel context significantly affected CPPS only (
p
< .05). Higher SPL, F0, and lower DUR were related to higher CPPS and HNR. SVs moderately-to-highly correlated with EVs for CPPS, whereas HNR had few and moderate correlations. In addition, CPPS and HNR highly correlated in SVs and seven EVs (
p
< .05).
Conclusion
Speaking prosodic variations of F0, SPL, and DUR influenced both CPPS and HNR measures and led to acoustic differences between sustained and excised vowels, especially in CPPS. Vowel context, prosodic factors, and token type should be controlled for in clinical acoustic voice assessment.
Resumo Este trabalho discute os efeitos da covid-19 na saúde de idosos, considerados principal grupo de risco nesta pandemia. Para tanto, partiremos de uma breve exposição demográfica do envelhecimento no Brasil para, então, discutir sobre como este período tem produzido e reforçado discursos que revelam estereótipos sobre envelhecimento. Esses discursos se relacionam com as dificuldades no enfrentamento dos efeitos deste período de distanciamento social e de suas possiblidades, tanto no contexto do cuidado residencial quanto nas instituições de longa permanência na Bahia, onde centralizamos esta discussão. Para finalizar, ressaltamos a urgência de ações organizadas e coordenadas que compreendam a complexidade do processo de envelhecimento para o enfrentamento, tanto dos discursos preconceituosos sobre os idosos quanto para os efeitos do isolamento. Também apontamos para a necessidade de nos reconhecermos e nos implicarmos nas demais gerações de que fazemos parte, seja em memória ou projeção.
