El uso de los plaguicidas ha producido grandes beneficios agrícolas y, a la vez, graves problemas de salud pública que requieren solución. Para destacar la importancia económica de la industria de los plaguicidas en el mundo, basta señalar que las ventas mundiales de las 20 principales compañías productoras sobrepasaron los tres millones de toneladas y que su facturación ascendió a US$ 21 000 millones a inicios de la presente década. Numerosos trabajos han demostrado la producción de intoxicaciones agudas por plaguicidas en seres humanos en diversos lugares del mundo. Calculan algunos estudios que el número anual de intoxicaciones agudas por plaguicidas oscila entre 500 000 y 1 528 000 y que cada año se producen de 3 000 a 28 000 defunciones por esa causa. En las estadísticas se indica que las intoxicaciones constituyen el 2.5 al 3 % de los casos atendidos en emergencia. Y del total de pacientes intoxicados el 28 % corresponde a organofosforados y de estos el 15% fallecen por no tener acceso inmediato a un área de salud y recibir atención oportuna. La metodología de la investigación, es una revisión bibliográfica, la cual se ha apoyado en medios electrónicos como Google Académico, PubMed, entre otros, en donde se ha encontrado información de diferentes trabajos de investigación, que aborda la temática aquí planteada. Ente algunas de las conclusiones destacan que una intoxicación por plaguicida puede ser mortal, independientemente de por cuál vía la persona haya tenido contacto con el mismo. Entre algunos de los síntomas que se pueden presentar, son dificultad respiratoria, disnea, irritación en ojos y piel, bradicardia, entre otros. Es por ello que la valoración inicial que se haga en el centro de salud por el personal médico más la información que pueda proporcionar los familiares, compañeros de trabajo, del intoxicado acerca del tipo de plaguicida puede ser determinante para el abordaje rápido en cuanto a la administración del tratamiento más adecuado. El tratamiento va a depender del grado de la intoxicación, sin embargo, entre los tipos de tratamiento está el uso de Atropina y Oximinas, colocación de sondas nasogástricas y lavado gástricos, administración de carbón activado, entre otros.
El mieloma múltiple es una enfermedad maligna caracterizada por la proliferación anormal de células plasmáticas, el pronóstico de los pacientes con esta enfermedad es muy variable, con una mediana de sobrevida de 2,5 a 3 años, la media de edad de su padecimiento se sitúa alrededor de los 65 años. Únicamente el 12 por ciento y el 3 por ciento de los pacientes tienen menos de 50 y 40 años, respectivamente. La metodología utilizada para el presente trabajo de investigación, se enmarcada dentro de una revisión bibliográfica de tipo documental, ya que nos vamos a ocupar de temas planteados a nivel teórico como es Diagnóstico de mieloma múltiple. La técnica para la recolección de datos está constituida por materiales electrónicos, estos últimos como Google Académico, PubMed, entre otros, apoyándose para ello en el uso de descriptores en ciencias de la salud o terminología MESH. La información aquí obtenida será revisada para su posterior análisis. Podemos concluir dentro de lo analizado en el desarrollo de esta investigación, que el mieloma múltiple es una afección crónica, y que a pesar de que existen diferentes tipos de tratamientos, la misma es incurable, por lo menos hasta el momento, a pesar de la combinación de diferentes fármacos y los trasplantes existentes, estos tratamientos lo que hacen es prolongar la vida de los pacientes, por un lapso hasta ahora no mayor de 5 años en la mayoría de los casos y uno de los factores predominantes en el desenlace de esta enfermedad es la edad, ya que afecta a personas mayores de 60 años asociados a comorbilidades que se puedan presentar que empeoran el pronóstico y la sobrevida y factores de riesgo asociados hasta ahora a exposiciones a radiaciones y a sustancias químicas, sustentadas por estudios a nivel internacional. En cuanto al diagnóstico como el resto de enfermedades, tiene una valoración física inicial, seguida de exámenes de laboratorio y por imágenes que la confirman.
El síndrome de Lutembacher se refiere a la rara combinación de comunicación interauricular congénita (CIA) y estenosis mitral adquirida. Las manifestaciones clínicas dependen del tamaño de la CIA, la gravedad de la estenosis mitral y de la distensibilidad del ventrículo derecho. Los síntomas clásicos de la estenosis mitral son atenuados debido a que la CIA actúa disminuyendo la presión en la aurícula izquierda, venas pulmonares y capilares pulmonares. Debido a la variabilidad de las manifestaciones cardiacas la presente investigación se desarrolló a los fines de ubicar material digitalizado necesario para estudiar, evaluar, sintetizar y exponer todo lo relacionado con el Síndrome de Lutembacher, en razón de lo cual, se enmarca en un diseño de revisión bibliográfica. Se limitó a la búsqueda y revisión de material académico accesible mediante el uso de diversas bases de datos, entre las que se usaron se encuentran: Organización Mundial de la Salud, MedlinePlus, Mayo Clinic, PubMed, Biblioteca Virtual de la Salud (BVS), SciELO, Dialnet, entre otras. Los resultados de registros fueron filtrados bajo los siguientes criterios: idioma español, correlación temática, relevancia. El Síndrome de Lutembacher, como se ha podido observar a lo largo de la investigación no tiene muchas publicaciones en el habla hispana, ni de reciente data pues su definición no se ha modificado. Se entiende que su diagnóstico y tratamiento depende del diagnóstico de la Comunicación Interauricular y la estenosis mitral y el nivel de complejidad que pueda mostrar el caso. Tal como se ha encontrado en la literatura la corrección de síndrome se resuelve quirúrgicamente cuyas implicaciones a largo plazo pueden ser materia de otra profundización en el tema.
