Objetivo: Comparar los resultados funcionales en pacientes operados de resección transuretral de próstata (RTUP), fotovaporización prostática con láser GreenLight ® (FTV) y enucleación prostática con láser Holmio (HOLEP), en el tratamiento de la obstrucción prostática benigna en el Hospital Central Militar. Material y métodos: Se aplicó cuestionario de índice internacional de sintomatología prostática (IPSS), Uroflujometría (Qmax), ultrasonografía vesical pre y postmiccional (PVR) a los pacientes llevados a RTUP, FTV y HOLEP de manera pre y post operatoria de enero del 2018 a marzo del 2019. Se analizaron las pruebas descriptivas y de normalidad de Shapiro-Wilks para las variables cuantitativas; los datos de comportamiento paramétrico se analizaron con pruebas de ANOVA, y los no paramétricos se analizaron con pruebas de U de Mann-Whitney y Kruskal-Wallis. Se utilizó paquete estadístico GraphPad PRISM v7.0. Resultados: Se evaluaron 46 pacientes de RTUP, 62 de HOLEP y 17 de FTV, eliminando aquellos que no lograron seguimiento postoperatorio completo; describiendo las características demográficas, las comorbilidades más frecuentes y los parámetros evaluados de manera preoperatoria. En el seguimiento, HOLEP fue ligeramente mejor que FTV o RTUP; sin encontrar significancia estadística entre los grupos, observando mejoría marcada de los valores de referencia de IPSS, Qmax y PVR pre y post operatorios en todas las poblaciones. Conclusiones: No se comprobó superioridad de alguna de las técnicas en cuanto a resultados funcionales postoperatorios a corto plazo, por lo que se propone realizar estudios a largo plazo, considerando agregar parámetros trans y postoperatorios a fin de demostrar superioridad de estas técnicas.
Antecedentes: La Neuromodulación Sacra es una técnica mínimamente invasiva para modular los reflejos espinales que tienen influencia sobre la vejiga, el intestino, el esfínter uretral externo y el piso pélvico, para mejorar y restaurar las funciones de micción y evacuación. Sus principales indicaciones terapéuticas son incontinencia urinaria de urgencia, síndrome urgencia-frecuencia, alteraciones en el vaciado no obstructivas, retención urinaria crónica, dolor pélvico crónico, incontinencia fecal y estreñimiento. En nuestro país se han realizado pocos estudios que evalúen la eficacia y seguridad de esta terapia. Objetivo del estudio: Evaluar la eficacia terapéutica, seguridad y calidad de vida de los pacientes con disfunción miccional crónica tratados con Neuromodulación sacra en el Hospital Central Militar. Material y métodos: Desde enero de 2010 hasta el 30 de junio del 2018, se realizó una revisión de los expedientes de pacientes tratados con terapia de Neuromodulación sacra por disfunción miccional crónica y que fueron refractarios a tratamiento médico, para hacer una evaluación de la calidad de vida mediante el empleo del instrumento SF-36 v2 (versión mexicana) e ICIQSF, eficacia con el empleo de los resultados de los diarios miccionales y parámetros urodinámicos, y se clasificaron las complicaciones del procedimiento según la escala de Clavien Dindo. Se utilizó el sistema estadístico IBM SPSS 22.0. Resultados: Se incluyeron 41 pacientes en el estudio con los siguientes diagnósticos: disinergia detrusor-esfínter, vejiga hiperactiva y esfínter urinario hipertónico, ubicados por género (16 masculinos y 25 femeninos). El 100% de los pacientes presentaron mejoría de los síntomas con la fase de prueba de la terapia y se les colocó neuroestimulador definitivo con los siguientes resultados: se obtuvo más del 65% de mejoría en la calidad de vida con parámetros cuantitativos y cualitativos, así como en las variables de capacidad vesical y volumen post miccional. Discusión y Conclusiones: Como se ha reportado en la literatura mundial, los resultados fueron similares en cuanto a la eficacia, mejoría en la calidad de vida y seguridad de la terapia de Neuromodulación sacra. Se han reportado porcentajes de éxito mayores al 50% en el cese total de los episodios de incontinencia. La Neuromodulación sacra es una terapia tanto mínimamente invasiva como duradera para el tratamiento de la disfunción miccional de etiología no obstructiva. En los casos en los que las pruebas de Neuromodulación no son efectivas, este tratamiento tiene la ventaja de ser reversible. Es necesario continuar con el desarrollo de estudios que puedan comprobar a mediano y largo plazo, mediante parámetros urodinámicos, la eficacia de este tratamiento.

Introducción: Los coronavirus son patógenos que afectan a animales y humanos y ocasionan enfermedades neurológicas, respiratorias, intestinales y hepáticas; se conocen 6 coronavirus que causan enfermedades en los humanos. La pandemia por la que ahora atravesamos se originó en la provincia de Hubei en Wuhan, China, reportando el primer caso de neumonía el 12 de diciembre de 2019. En febrero de 2020, la Organización Mundial de la Salud, le denominó COVID-19 y el Comité Internacional de Taxonomía de Virus (ICTV) como SARS-CoV-2. Hasta el 30 de abril de 2020, se han confirmado más de 3 000 000 casos en el mundo, afecta a 185 países, con más de 200 000 defunciones y aproximadamente 420 000 pacientes recuperados. Relevancia: Se realizó una revisión de la literatura con la finalidad de brindar a los urólogos el estado actual de la enfermedad en México, la fisiopatología de la enfermedad ocasionada por el SARS-CoV-2, así como revisión de las últimas actualizaciones acerca de los diferentes tratamientos utilizados para el COVID-19. Conclusiones: Es importante que como urólogos nos encontremos actualizados e informados acerca de un tema interés y relevancia a nivel mundial por ser parte del sistema de salud, ya que en algunos centros nos vemos involucrados con el manejo de pacientes con diagnóstico de COVID-19.
