La población indígena tiene condiciones de vida inferiores al resto, reflejadas en mayor morbilidad y mortalidad a pesar de la cobertura del Sistema de Salud. Por ello, es importante conocer las causas de estas diferencias. Para esto, se hace uso de la interculturalidad como puente entre la cultura occidental y la cultura indígena. En este encuentro de saberes se identifica el modelo de salud indígena como respuesta cultural a la necesidad de mantener la salud y tratar la enfermedad, un modelo organizado jerárquicamente en el que la salud del individuo depende además de sus hábitos, de la armonía con la naturaleza, el espíritu, los dioses y su comunidad. Este modelo había sido menospreciado hasta hace poco tiempo por la comunidad científica; pero, gracias a los estudios en interculturalidad, se sabe que la salud también debe ser intercultural y que las políticas públicas deben incluirla para poder obtener los resultados esperados en la comunidad objetivo. Para hacer realidad estas políticas públicas debe haber voluntad y agenda política, una adecuada estructura en los servicios de salud y formación de los profesionales de la salud en interculturalidad desde sus estudios técnicos, tecnológicos, profesionales y de posgrado. Esas políticas públicas deben contener: capacitación, empleo de la lengua indígena local, alimentación y equipamiento con elementos tradicionales, diálogo respetuoso con los médicos tradicionales, atención humanizada, entre otros. Así se brinda una atención en salud de calidad que respeta las diferencias culturales de toda la población.
Objetivo Conocer las barreras y facilitadores para la continuidad en la implementación de la estrategia de Atención Primaria en Salud en Palmira.Métodos Se tomó la experiencia de Atención Primaria en Salud (APS) del ente territorial de salud del Municipio de Palmira, Colombia, un territorio de aproximadamente 283 431 habitantes. Se usó el enfoque metodológico cualitativo mediante el análisis de contenido temático. La población objeto fueron los informantes clave, individuos con un papel potencialmente significativo en la formulación y desarrollo de la estrategia de APS, elegidos con un enfoque no probabilístico por conveniencia. La información se obtuvo de fuentes primarias y secundarias y se utilizó el software informático de análisis cualitativo Atlas Ti V7, como herramienta de apoyo para el manejo de datos.Resultados El análisis de barreras y facilitadores para la continuidad de la estrategia de APS identificó las principales características de la estrategia y una serie de temas recurrentes al momento de analizar las posibles barreras y facilitadores en los componentes de proceso, actores, contenido y contexto. Cada uno de estos temas presentó comportamientos diversos según la percepción de los participantes. Algunos temas fueron percibidos como facilitadores, barreras o como ambos.Conclusión Al comparar las barreras y facilitadores, se encontró que, a nivel global, son mayores las barreras que los facilitadores, comportamiento reflejado en la teoría, que indica que la población con algún tipo de vulnerabilidad presenta mayor cantidad de barreras frente a los servicios de salud y que la estrategia de APS hace especial énfasis en este tipo de población.
In the last issue of Colombia Médica, the Cali Population Registry of Cancer (RPCC, for its initials in Spanish) documented that important advances in cancer control have been achieved (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6018818/). Mortality due to this group of diseases has decreased significantly during the last 25 years, thanks to the partial control of cancers related to infectious agents (stomach and cervix) and cancers associated with tobacco consumption (lung, bladder, oral cavity). However, there is less control of cancers related to screening activities (breast, prostate and colorectal), and hematolymphoid tumors that have successful treatment options with chemotherapy. As a result, cancer survival in children and adults is 20 percentage points below that observed in Europe and the United States 1. One strategy to improve the indicators is the creation of a cancer observatory in the city. Health observatories are conceived as smart, proactive, dynamic, synchronic and participatory systems that generate knowledge for the formulation of policies and strategies that allow to impact, in a positive way, the social determinants of health (2). With this perspective and in congruence with the technical line of health observatories given by the Ministry of Health and Social Protection, it is necessary to generate strategies; to have articulation spaces; and to integrate sources of information, which in addition to being standardized and synchronized, facilitate the monitoring and identification of trends in the behavior of events, and the understanding and explanation of the health situation of the population based on complex analyses of inequalities and health vulnerabilities 2.
