Objetivo: analizar y mejorar las prescripciones de antibióticos por parte de odontólogos, a través de una intervención educativa. Método: se realizó un estudio analizando las recetas de prescripción de medicamentos antes y después de la intervención educativa, en el Instituto de Servicios Sociales de la Universidad Nacional del Nordeste. El estudio comprendió tres etapas; en la etapa A se identificaron problemas de prescripción, que fueron socializados con los profesionales durante la intervención educativa como parte de la etapa B, cuya modalidad consistió en dos entrevistas presenciales, con talleres de discusión y debate. En esta se trabajó, además, con el informe de los resultados obtenidos en el análisis de situación de la etapa A. En la etapa C se analizó el impacto de la intervención educativa. Resultados: en la etapa A se observaron 417 recetas con 202 prescripciones de antibióticos, los más indicados fueron azitromicina (31%), amoxicilina con clavulánico (10%) y amoxicilina (8%). La adecuación de prescripción en relación a la indicación fue del 26%. En la etapa C, de un total de 191 recetas, se analizaron 107 recetas con prescripción de antibióticos. La amoxicilina fue el medicamento más prescripto (37%); la prescripción de azitromicina disminuyó a 14% y su adecuación fue del 46%. Ello evidenció una mejoría del 20%, la cual fue validada a través del test chi2 (p=0,00). Conclusión: por medio de la intervención educativa presencial es posible mejorar las prescripciones de antibióticos, no obstante se desconoce si el cambio de conducta prescriptiva se mantiene en el tiempo.El estudio comprendió tres etapas, la primera para identificar problemas de prescripción a fin de ser socializado con los profesionales durante la intervención educativa, que incluía procedimientos educativos presenciales. En conclusión se observó que a través de una intervención educativa con los profesionales prescriptores, se pudo mejorar las prescripciones de antibióticos, no obstante se desconoce si el cambio de conducta prescriptiva se mantiene en el tiempo.
La mucormicosis es una infección fúngica rara, con alta morbilidad y mortalidad. Se presenta principalmente en pacientes con diabetes mellitus no controlada, inmunocomprometidos, con tratamiento crónicos con esteroides, entre otros. Actualmente, se cree que la pandemia de COVID-19 y los tratamientos con corticosteroides podrían estar implicados en el aumento de casos de esta micosis. Este hongo invade el sistema vascular, ocluyendo el flujo sanguíneo arterial y generando una rápida trombosis e isquemia, lo que provoca la necrosis de los tejidos duros y blandos, con invasión rápida a los tejidos circundantes. Hay varias formas clínicas. En la cavidad bucal se presenta la variante rino-orbito-cerebral, que afecta el paladar en forma de lesión eritematosa o grisácea que puede progresar hacia la formación de una masa necrótica o ulceración con muy escaso sangrado de mucosa. Se manifiesta con síntomas típicos de una rinosinusitis con fiebre y dolor en las piezas dentarias superiores. El tratamiento consta de tres pilares fundamentales: el diagnóstico, un manejo adecuado de las comorbilidades y la combinación de las terapias antifúngica y quirúrgica. Desde el año 2020, la mucormicosis asociada a COVID-19 pasó a ser un evento de notificación obligatoria inmediata al Sistema Nacional de Vigilancia de la Salud (SNVS2.0) mediante el Sistema Integrado de Información Sanitaria Argentina (SISA). Es importante destacar que se han reportado casos de mucormicosis luego de extracciones dentales; lo que impulsa a afianzar los conocimientos sobre esta enfermedad, extremar las medidas preventivas e incentivar el diagnóstico precoz en la atención odontológica, debido a la rapidez en la evolución de la patología. La seguridad del paciente es una dimensión de calidad en atención de salud, porque hay cierto grado de peligrosidad en cada paso del proceso; centrándonos en el uso de medicamentos los problemas más perjudiciales están relacionados a errores en la selección de los medicamentos y su correlación con el diagnóstico y en la utilización injustificada de los mismos. El objetivo de este trabajo fue analizar situaciones de prescripción de medicamentos en relación a la seguridad del paciente. Se trata de un estudio prospectivo, observacional y descriptivo; de una muestra constituida por todas las recetas recibidas durante 12 meses continuos, prescriptas por odontólogos para patologías bucodentales, de un instituto de servicio social, de la ciudad de Corrientes, Argentina. Las situaciones de prescripción se analizaron y clasificaron en adecuadas e inadecuadas, y estas últimas fueron subdivididas en: a) prescripción excesiva o submedicación y b) prescripción inapropiada en relación entre medicamento o dosis indicadas, con el diagnóstico. Se realizó el análisis cuantitativo de los medicamentos prescritos con estadística descriptiva y un análisis cualitativo a través de parámetros de calidad como número de principios activos que contiene el medicamento y el “valor intrínseco terapéutico potencial” (VITP). Resultados: de 518 recetas prescriptas por odontólogos para patologías bucodentales, el 20% presentaban situaciones de uso inseguro, 17 recetas con prescripciones excesivas o submedicación y 86 recetas con inadecuada relación entre el medicamento y diagnóstico. En conclusión hemos detectado prescripciones con alto potencial de producir efectos adversos, como así también situaciones de falta de conocimiento sobre indicaciones terapéuticas, con uso innecesario de antibióticos.
El objetivo de la investigación fue determinar la prevalencia de complicaciones postoperatorias de la exodoncia y su tratamiento en pacientes atendidos en la clínica de Patología y Diagnóstico de la Facultad de Odontología de Corrientes, Argentina. Se realizó un estudio descriptivo, observacional, transversal y prospectivo en la clínica de Patología y Diagnóstico, utilizando un muestreo no probabilístico por conveniencia: se seleccionaron los pacientes que concurrieron a la clínica B de Patología y Diagnóstico para realizarse exodoncias y presentaron complicaciones postoperatorias después de la misma, desde agosto 2021 a junio 2022. Las variables estudiadas fueron: edad, sexo, grupo dentario (incisivo, canino, premolar, molar), localización según el maxilar, hábito de fumar, si recibió medicación pre o postquirúrgica, tratamiento que recibió la complicación posoperatoria. Durante la segmentación en burbujas por la pandemia COVID-19, a la clínica B asistieron 1184 pacientes, de los cuales 298 se realizaron exodoncias, quedaron excluidos 7 inmunosuprimidos. Del total de 291 se registraron n=15 (5%) complicaciones postoperatorias, las cuales correspondieron a alveolitis y hemorragias. El predominio de complicaciones postoperatorias se observó en mujeres, y con respecto a la ubicación de las piezas dentarias, la mayor frecuencia fue en piezas dentarias multirradiculares del maxilar inferior. Todas las complicaciones postoperatorias fueron tratadas con terapias locales. Dentro de las limitaciones del presente estudio, se puede señalar que, por el escenario de pandemia, el registro de pacientes se realizó con intervalos quincenales, lo que pudo contribuir a una subestimación de la prevalencia de complicaciones en los pacientes de la clínica.
