Introducción: Distintos fármacos influyen en la duración del bloqueo neuromuscular durante la anestesia general. Objetivo: Determinar el efecto de una dosis única de betametasona en la duración del bloqueo neuromuscular inducido por rocuronio. Material y métodos: Estudio observacional, aleatorizado, longitudinal y prospectivo. 36 pacientes adultos programados para colecistectomía laparoscópica recibieron 200 μg de betametasona intravenosa una hora previa a la cirugía (grupo A), previo a la inducción anestésica (grupo B) o en cuidados postanestésicos con recuperación íntegra del bloqueo neuromuscular (grupo C). El curso del tiempo del bloqueo neuromuscular con rocuronio (300 μg/kg) se evaluó usando aceleromiografía. La medición principal fue el tiempo transcurrido desde el inicio de la administración de rocuronio hasta la recuperación del índice tren de cuatro de 0.9. Resultados: El inicio de acción fue más corto en el grupo B (1.3 ± 0.1 min) comparado con el grupo A (1.4 ± 0.4 min) y C (1.6 ± 0.9 min). El índice de recuperación 25-75% fue menor en el grupo B (14 ± 7.8) en comparación con el grupo A (15.0 ± 11.1 min) y C (15.2 ± 11.1 min). Conclusión: El tiempo del bloqueo neuromuscular inducido con rocuronio disminuye aproximadamente 5% ante la administración de una dosis única de betametasona a la inducción anestésica.
La náusea y vómito postoperatorio (NVPO) en cirugía laparoscópica de colecistitis es frecuente. El uso de ondansetrón y dexametasona (GOD), y palonosetrón (GP) es muy prevalente. Objetivo: Comparar la eficacia en el control de la NVPO. Material y métodos: Realizamos un estudio comparativo de dos cohortes retrospectivas de 62 pacientes cada una con cirugía electiva de colecistectomía; uno con profilaxis con GOD y otro con GP. Se evaluó el riesgo de NVPO por la escala de Apfel. Las NVPO fueron estudiadas en las 24 horas postrecuperación de la anestesia. Resultados: No hubo diferencias en cuanto al sexo, valoración preanestésica y riesgo de NVPO entre los grupos. El 12.9% (8/62) de los pacientes del grupo GOD presentó náuseas, contra ninguno del grupo GP (p = 0.003). Ningún paciente presentó vómitos postoperatorios (p = 1.00). En el grupo GOD seis requirieron rescate farmacológico (9.6%), contra ninguno del GP (p = 0.014). No hubo efectos secundarios en ningún grupo. Conclusión: El uso de palonosetrón evidenció mejor control de la náusea, pero ambas estrategias profilácticas controlaron el vómito postoperatorio.
Introducción: Recientemente los objetivos de la cirugía mayor tratan de enfocarse en ofrecer las mejores posibilidades para una pronta recuperación y reanudación a las labores cotidianas con adecuada calidad analgésica, cuestiones que representan un reto para el anestesiólogo en la elección de los medicamentos a usar. Material y métodos: Se realizó un estudio prospectivo, observacional y descriptivo del tipo casos y controles. Se estudiaron 26 pacientes en quienes se llevó a cabo algún tipo de cirugía mayor no cardiaca bajo anestesia general balanceada, teniendo como objetivo cuantificar el consumo de fentanilo transoperatorio en una población homogénea. Se determinó que existe menor consumo de opioides transanestésicos en el grupo al cual se le perfundió dexmedetomidina, con un valor de p < 0.05 así como mejor calidad analgésica midiendo ésta con la escala numérica del dolor. Resultados: Se concluyó que el uso de dexmedetomidina en perfusión durante el periodo transanestésico disminuye el consumo de opioides y probablemente contribuya con su pronta recuperación.
Introducción: el bloqueo subaracnoideo es la modalidad de anestesia que más se utiliza en cesárea electiva, pero conlleva el riesgo de inestabilidad hemodinámica materna (hipotensión y bradicardia). El uso de vasopresores como efedrina en bolo o norepinefrina en infusión permite controlar la presión arterial, pero no existe suficiente evidencia para guiar la elección del agente específico. Objetivos: comparar la frecuencia de eventos de inestabilidad hemodinámica en mujeres sometidas a cesárea electiva con bloqueo subaracnoideo tratadas con efedrina o con norepinefrina. Material y métodos: realizamos un ensayo clínico fase IV, pragmático, de etiqueta abierta y de no inferioridad. Los desenlaces primarios fueron el número de eventos de bradicardia e hipotensión, y también se registraron eventos de náusea, vómito, eventos adversos graves y puntaje de Apgar. Resultados: el uso de norepinefrina redujo el riesgo de hipotensión en 71%, de náusea en 93% y de emesis en 67%. No se observaron diferencias en los eventos de bradicardia ni en el puntaje de la escala de Apgar. Conclusión: la norepinefrina no es inferior a la efedrina para el manejo terapéutico de la hipotensión materna asociada a bloqueo subaracnoideo en cesárea electiva.
