Objetivo: avaliar as condições clínicas e intervenções terapêuticas que predispõem a ocorrência da IRA nos pacientes internados em UTI. Método: trata-se de uma Revisão Sistemática da Literatura nas seguintes fontes de dados: PUBMED, MEDLINE, LILACS, BDENF e SCIELO. As evidências dos estudos foram classificadas conforme (XX). Foram utilizados os seguintes descritores: “Hemodiálise” AND “Terapia intensiva”. Resultados: destaca-se que a IRA na UTI está relacionada à: fatores de risco associados à terapêutica (uso de DVA, VMI, antibioticoterapia, nefropatia medicamentosa e induzida pelo uso de contraste à base de iodo); comorbidades (DM, HAS, pulmonares, digestivas, hepáticas, neoplasias, anemia, obesidade e imunossupressão), agravos agudos (trauma, hemorragia, acidente ofídico, choque, infecção, sepse, eclâmpsia; complicações de doença cardiovascular, de doença neurológica aguda, necrose tubular aguda, síndrome hepatorenal e acidentes com animais peçonhentos), idade, distúrbios hidroeletrolíticos (hipercalcêmia, hipocalcêmia, hipernatremia e a hiponatremia), distúrbios do equilíbrio ácido básico, balanço hídrico acumulativo e necrose tubular aguda. Conclusão: constata-se que a IRA na UTI tem relaciona-se com a condição clínica do paciente e intervenções terapêuticas realizadas na UTI.
Objetivo: avaliar a gravidade clínica do paciente admitido em UTI aplicando o Modified Early Warning Score. Método: estudo transversal, analítico, quantitativo; foi realizado na UTI de um hospital de referência regional. Os dados foram coletados nos prontuários dos pacientes usando um instrumento focando: motivo da admissão na UTI; doença de base, comorbidades; complicações clínicas e intervenções realizadas na UTI. A amostra foi composta por 265 prontuários considerando um nível de significância de 5% e um erro amostral relativo de 8%. Resultados: prevaleceram as faixas etárias de 41-50 (13%) e 71-80 anos (18%); quanto ao motivo de admissão identificou-se: agravos traumáticos (30%), pneumonia (12%), Acidente Vascular Encefálico (11%) e o Acidente Vascular Encefálico Hemorrágico (10%). A avaliação da gravidade através do MEWS identificou: valores entre 1 e 12 com média de 5,2; maior taxa de mortalidade associada ao MEWS classificado entre 4 a 6. Entre pacientes que usaram VMI, para 40,3% o MEWS ficou entre 4-6. A maior permanência na UTI foi registrada em pacientes com MEWS de 1-4. Conclusão: pontuações mais elevadas no MEWS estão associadas a uma maior gravidade clínica, bem como maior necessidade assistencial, permitindo assim o estabelecimento de prioridades na assistência.

Objetivo: identificar como o Modified Early Warning Score (MEWS) tem sido aplicado na prática clínica hospitalar pelo enfermeiro. Método: trata-se de uma revisão integrativa da literatura; para a elaboração da questão norteadora utilizou-se a estratégia PICO conforme recomendação do Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA): O nível de evidência dos estudos selecionados foi baseado no Oxford Centre for Evidence-based Medicine. Resultados: identificaram-se 24 artigos nas seguintes fontes: PUBMED, SCIELO, LILACS, SCOPUS e WEB OF SCIENCE; os níveis de evidência foram: 7 artigos classificados com evidência 1A; 12 artigos classificados com 2A; 1 artigo classificado como 2A; 2 artigos 2C e 2 artigos classificados como 3A. O MEWS é aplicado como escore preditor de deterioração clínica das funções vitais, na identificação do risco para ocorrência de Parada Cardiorrespiratória e no estabelecimento de critérios para admissão na UTI. Conclusão: contatou-se que a utilização do MEWS na prática clínica hospitalar pelo enfermeiro objetiva a avaliação do estado de saúde do paciente, identificando sinais de deterioração grave das funções vitais. Ampara a adoção dos princípios de Segurança do Paciente além de favorecer a comunicação entre a equipe agiliza intervenções do Time de Resposta Rápida.
