La cirugía tiene el objetivo supremo de curar, o al menos mejorar al enfermo, con la convicción de que, en un momento dado, es la mejor si no la única opción terapéutica que permite, dentro de lo humanamente posible, ofrecer la eliminación de una patología bajo el sustento de una consciente capacidad profesional y un comportamiento ético. La cirugía laparoscópica o “mínimamente invasiva” es una técnica especializada para realizar cirugía. La cirugía laparoscópica constituye uno de los grandes avances de la cirugía en el siglo XX al poder efectuar las mismas intervenciones que en cirugía abierta, pero evitando grandes incisiones. Generalmente, es la cirugía hecha a nivel abdominal mediante un abordaje a través de pequeñas incisiones entre 2 y 15 mm. De manera técnica consiste en crear un espacio a nivel abdominal tras la introducción de un gas (CO2). Por pequeñas incisiones en el abdomen se introduce una cámara conectada a un televisor o monitor o pantalla, a través del cual se sigue la intervención, y también el instrumental necesario para ejecutar las diferentes maniobras de disección, corte, sutura, etc., que son necesarias en la intervención.
Los abscesos hepáticos son patologías de importancia clínica, dado a la gran morbimortalidad de estos sobre todo en los países en condiciones de pobreza y desnutrición. Cuando se desarrollan abscesos hepáticos piógenos, es más común después de una peritonitis debido a la pérdida de contenido intestinal intraabdominal con diseminación posterior al hígado a través de la circulación portal o en el contexto de una infección biliar por diseminación directa. También pueden resultar de la siembra arterial hematógena en el contexto de una infección sistémica. Por lo cual se considera de suma importancia realizar una revisión del tema para actualizar el abordaje diagnóstico y tratamiento de este. Objetivo: el objetivo del presente trabajo es realizar una actualización en el diagnóstico y tratamiento del absceso hepático causado por infecciones bacterianas mediante la búsqueda bibliográfica de información reciente y relevante, utilizando los principales motores de búsqueda disponibles. Conclusiones: La sospecha clínica de absceso hepático es lo fundamental a la hora de realizar el diagnóstico de este. El diagnóstico oportuno del paciente con absceso hepático y el inicio de tratamiento dirigido, reduce la morbi-mortalidad de los pacientes. El uso de terapia antibiótica dirigida disminuye el riesgo de resistencia antimicrobiana y reduce los días de internación de los pacientes.
