Fortes Álvarez a y María de los Desamparados Pérez López b. RESUMEN Una paciente de 29 años, fue hospitalizada por alteraciones de conducta, desorientación y convulsiones tónico-clónicas. La evolución fue tórpida, presentando síntomas, que fueron desde la confusión, soliloquios, risas inmotivadas, movimientos involuntarios, síntomas psicóticos hasta mutismo y estupor. Múltiples analíticas (uroporfirinas, coproporfirinas, zinc, ceruloplasmina), cultivos, serología (VIH, Brucella, Citomegalovirus, etc.), tomografía y resonancia magnética cerebral fueron negativas. El líquido cefalorraquídeo (LCR) mostró pleocitosis linfocítica. Los anticuerpos para enfermedades paraneoplásicas (anti-Hu, anti-Yo, anti-Ri, anti-CV2, antifisina, anti-Tr y anti-canales de calcio) fueron negativos, excepto los anticuerpos anti-NMDA (positivos en LCR). La ecografía ginecológica mostró un ovario aumentado de tamaño. El diagnóstico final fue de encefalitis contra los receptores NMDA (encefalitis límbica paraneoplásica). Su mecanismo autoinmune, proveería un modelo autoinmune para explicar la sintomatología psicótica asociada.
