Sponsored by the Health Administrations of nine cantons, this study was conducted by the University Institute of Social and Preventive Medicine in Lausanne in order to assess how DRGs could be used within the Swiss context. A data base mainly provided by the Swiss VESKA statistics was used. The first step provided the transformation of Swiss diagnostic and intervention codes into US codes, allowing direct use of the Yale Grouper for DRG. The second step showed that the overall performance of DRG in terms of variability reduction of the length of stay was similar to the one observed in US; there are, however, problems when the homogeneity of medicotechnical procedures for DRG is considered. The third steps showed how DRG could be used as an account unit in hospital, and how costs per DRG could be estimated. Other examples of applications of DRG were examined, for example comparison of Casemix or length of stay between hospitals.
Teoría de la metáfora y cognición corporeizada ¿Cómo se introduce la teoría de la metáfora conceptual en los estudios musicales?La Teoría de la Metáfora de Lakoff y Johnson señala que parte de nuestra forma de comprender el mundo es metafórica, en el sentido de vincular patrones experienciales de un dominio cognitivo conocido con otro menos conocido. En este proceso participan ciertas estructuras imagen-esquemáticas, interrelacionadas y dinámicas, que surgen de nuestra propia experiencia corporal con el entorno. Considerando esto, podemos situar esta teoría dentro de las teorías cognitivas corporeizadas. Algunos investigadores han tomado la Teoría de la Metáfora Conceptual para explicar la experiencia y la cognición en el ámbito musical, donde se observan diferentes interpretaciones e implicancias. Proponemos, entonces, analizar diferentes componentes de la teoría, integrando los estudios en música ya existentes, a partir de cuatro niveles explicativos: (i) el origen corporeizado de los esquemas-imagen como un estrato profundo de base sensorio-motora; (ii) la proyección metafórica como un proceso que depende de esa experiencia corporal; (iii) la metáfora lingüística como descripciones verbales de una experiencia y comprensión metafórica; y (iv) las representaciones gráficas de los esquemasimagen en la explicación teórica y la grafía como descripción de la experiencia musical. En el artículo se describe un estudio empírico realizado con espectadores de una obra de teatro-danza, cuyo objetivo fue explorar la captación de formas de la vitalidad (Stern, 1985 y 2010). El estudio se caracteriza por un diseño complejo que implicó (a) la puesta en escena de una obra teatral, (b) la conformación de un estímulo novedoso a partir de la obra, y (c) la creación de tareas a ser realizadas por los espectadores al finalizar la función. La obra a la que asistieron los espectadores/participantes fue Dibaxu del director Hugo Aristimuño. El estímulo creado fue la proyección de dos fragmentos de la obra filmados en vivo en una de las funciones. Las tareas fueron cuatro: dos tareas no-verbales, originales y novedosas, y dos verbales. Se describen los rasgos particulares de la obra, el proceso de creación del estímulo y las tareas creadas. Se presentan los resultados de la tarea verbal que consistió en la selección de palabras descriptoras de formas de la vitalidad para caracterizar los fragmentos seleccionados de la obra presenciada. Se comentan luego los desafíos metodológicos del estudio de la recepción de espectáculos en vivo, se realizan comparaciones con otros trabajos del campo, y se resalta la virtud del diseño que permitió crear un estímulo breve que conserva, sin embargo, las cualidades de la obra completa. Finalmente, se discute la contribución de la investigación realizada tanto al estudio de las formas de la vitalidad como a los estudios de públicos de teatro y danza de orientación cognitiva.
La experiencia musical en la infancia temprana es un campo de estudio que está siendo desarrollado con aportes de la psicología cognitiva y la psicología de la música. En el ámbito educacional, se presenta aún un área de vacancia. Se considera fundamental dar lugar a la vivencia de la música como arte temporal y experiencia estética, como conocimiento en sí mismo y no subsumido a otros campos de conocimiento, entendiéndola desde una mirada multimodal, intersubjetiva, situada y corporeizada.
La presente tesis se desarrolla en torno a una investigación teórico-empírica de la experiencia narrativa del tiempo musical, abordada desde el estudio de la experiencia temporal con el aporte de la teoría de la conceptualización metafórica. La elección de este tema surge, por un lado, de la reflexión pedagógica. A partir de mi labor como docente de Educación Auditiva (FBA-UNLP), donde se analizan diferentes componentes musicales principalmente desde la audición, me encuentro inmersa en un contexto en el que se cuestiona un modo tradicional de abordar los contenidos musicales, anclado en una visión estructuralista de la música y sustentado desde la psicología cognitiva clásica, y, al mismo tiempo, se debate sobre la práctica de significados desde una postura más experiencialista (y también de mayor validez ecológica), aún cuando los contenidos de la asignatura requieren ser tratados, a menudo, desde sus características teórico-estructurales. Sin embargo, al investigar algunas diferencias entre la escucha musical en la vida cotidiana y la escucha musical en las clases, comienzan a surgir una serie de tópicos relacionados con los modos de escucha. Entre ellos aparecen las cuestiones de la narratividad de la música y de la existencia de componentes narrativos en la experiencia musical, vinculadas al contexto de audición de la música, la escucha de las obras ‘por partes’ vs. ‘en su totalidad,’ el condicionamiento en la experiencia temporal de la música a partir de ciertas consignas o señales extramusicales (títulos, argumentos, programas, etc.) y la incidencia del momento en que se dan o se conocen dichas pistas (antes, durante, después de la audición de una obra), etc. Por ello, resulta esencial profundizar el estudio del aspecto narrativo de la experiencia temporal como un punto clave en la cognición musical. No obstante, como se verá en el Capítulo 1, la investigación en ‘música y narración’ no es un tema nuevo, sino que ha estado presente en la agenda musicológica por décadas. Por otro lado, el tema de tesis fue tomando forma a partir de las discusiones actuales sobre los nuevos paradigmas en psicología cognitiva. Particularmente, se apuntala en la importancia del componente dinámico-temporal en la explicación de la experiencia humana, y el desarrollo de una concepción más amplia de mente.
