Introdução: a gastrostomia tem sido ainda indicada como procedimento de exceção no tratamento paliativo do câncer avançado de cabeça e pescoço e tubo digestivo alto no Serviço de Tratamento Paliativo do Hospital de Câncer de Pernambuco. Método: retrospectivo com análise de prontuário e entrevista com familiares e pacientes após a cirurgia. Resultado: analisamos as gastrostomias realizadas durante o período deste último ano em pacientes do Serviço de Tratamento Paliativo, particularmente os portadores de doença avançada do tubo digestivo alto e de tumores de Cabeça e Pescoço, sendo a maioria destes com estado de desnutrição significativa. A mortalidade relacionada a cirurgia alcançou 20% dos casos. A morbidade foi de 100% dos casos, principalmente com problemas de deiscência e infecção da ferida cirúrgica. Mais de 40% dos entrevistados mostraram-se insatisfeitos com a realização da gastrostomia como procedimento para melhorar a condição clínica do paciente. Conclusões: A percepção de contribuir mal para o tratamento paliativo que tem o objetivo de melhorar a qualidade de vida de pacientes que não tem mais como aumentar o tempo de vida, nos leva a repensar critérios de indicação de gastrostomias para todos os pacientes com disfagia completa e sem acesso a sonda naso enteral.
Introdução: O crescimento lento de tumores desmoides, classificados anátomopatologicamente na categoria de tumores benígnos, contribui para o retardo no tratamento cirúrgico e para a grande extensão destes tumores. Método: apresentação de dois casos clínicos. Resultado: Duas mulheres com condições clínicas e faixa etárias distintas, são portadoras de tumores semelhantes em localização e extensão. Apresentamos detalhes técnicos das cirurgias, dificuldades de obtenção de margens adequadas e morbidade pós-operatórias. A importância do trabalho da psicologia, nutrição, ccih e fisioterapia para o sucesso da recuperação. Conclusão: o tumor desmoide geralmente apresenta-se em serviços de oncologia com história de recidivas e como grandes massas, aumentando a complexidade do tratamento cirúrgico.
