Debates about universalism in health have been gaining ground in Latin America and have entered the policy agenda with differing results. Notwithstanding the country's federalism, the most profound changes that took place in Argentina in the last decade occurred in the arena of national politics. Based on the theoretical contributions of historical neo-institutionalism and implementation studies, this paper aims to analyze, from a macro institutional approach, the scope of the national policy regarding health universalization. This descriptive study is based on secondary sources and the review of research results on the implementation of the programs Remediar, Sumar and Plan Nacer in relation to four variables: coverage, access, sets of benefits and rights included in the policy. Given the characteristics of the Argentine institutional matrix, program implementation in subnational scenarios can be expected to confront complex and heterogeneous terrain in which the programs acquire new meanings with respect to the goal of universality that each poses.
Resumen Desde un abordaje que recupera los aportes de la Teoría de las Organizaciones y de la literatura en redes, el trabajo describe y analiza las redes de servicios que operan en el sistema de salud del Gran Buenos Aires (GBA). Pone el foco en la estructura organizacional que muestran las iniciativas, a la vez que explora los mecanismos y dispositivos que despliegan para articular recursos, prestaciones y servicios dispersos en el territorio. Para la identificación y la selección de las redes se entrevistó a los Directores Ejecutivos de las cuatro Regiones Sanitarias (RS) del conurbano bonaerense (4 entrevistas). Posteriormente, se aplicó una ficha de relevamiento en cada una de las redes seleccionadas; la indagación se focalizó en cuatro grandes dimensiones derivadas de la perspectiva teórica: dimensión estructural, amplitud de la red y dimensión organizacional. Finalmente, se realizaron 18 entrevistas en profundidad a sus coordinadores. La reconstrucción y el análisis se completaron triangulando la evidencia empírica con fuentes secundarias de organismos oficiales. Las redes estudiadas alcanzan distintas escalas territoriales y se apoyan en programas de salud nacionales y/o provinciales preexistentes. Además de establecimientos de mediana y alta complejidad del subsector público, involucran - en muchos casos - prestaciones del primer nivel de atención; sin embargo, la lógica organizacional se centra en el hospital. Finalmente, se observa una carencia de instancias, dinámicas y recursos formalmente destinados al funcionamiento. Esto ha implicado un alto compromiso de los equipos involucrados para poder gestionarlas y, se ha traducido en importantes limitaciones para su desarrollo y autonomía.
La cuarentena declarada en marzo de 2020, obligó a las familias Argentinas a aislarse. Algunos informes y artículos comenzaron a demostrar cómo el aislamiento social, ha tenido un impacto diferencial, empeorando las condiciones de vida de los sectores vulnerados. En el Gran Buenos Aires, entre los problemas más importantes experimentados por estos sectores se ha ubicado al acceso a los servicios de salud.
El trabajo analiza el papel que jugaron los establecimientos del primer nivel en facilitar el acceso a la atención primaria de la salud en los barrios, y la reorganización que estos efectores de salud han experimentado a partir de la pandemia. Con este objetivo, se han recuperado algunos resultados de un relevamiento remoto realizado en los barrios populares del conurbano bonaerense (Gran Buenos Aires) a referentes del campo de la salud en el territorio.
En la fase de impacto la prioridad que se le dio a ciertas prácticas esenciales, ha fortalecido las relaciones de proximidad en un territorio asolado por la pandemia. En paralelo, desde los equipos de salud, se han generado estrategias y/o iniciativas que apuntaron a reestablecer la atención de la salud No COVID en el primer nivel, y garantizar la disponibilidad de determinados servicios y de algunos medicamentos y vacunas.
