Este artigo se origina de uma pesquisa cujo objetivo foi avaliar as estratégias locorregionais de articulação entre os níveis de cuidado à saúde, considerando tanto a adequação às normas existentes (estratégia deliberada), quanto os desenvolvimentos criativos (estratégias emergentes) realizados nos contextos locorregionais estudados. A referência para julgamento (ato inerente à pesquisa avaliativa) foi a coerência com os princípios constitucionais definidores do SUS. Na realização do estudo de caso aqui relatado, foram obtidas informações através de documentos secundários, observação direta extensiva, questionários e entrevistas em profundidade com informantes-chave. Os resultados mostraram, com relação à articulação da atenção básica com os demais níveis de atenção à saúde dos Bororo no Polo-Base Rondonópolis, o predomínio de um modelo de organização dos serviços voltado para ações especializadas e de alto custo, em detrimento da valorização dos conhecimentos populares e das práticas de saúde tradicionais. Além disso, a estratégia deliberada para os Distritos Sanitários Especiais Indígenas foi transformada, tendo emergido uma estrutura como a preconizada pela noção de distrito sanitário sem atenção às especificidades da saúde indígena. Confirmou-se a suposição geral que orientou este estudo, segundo a qual existe uma enorme distância entre o que é declarado e formalizado nos planos elaborados de acordo com as normatizações e o que efetivamente ocorre no cotidiano dos serviços e no fluxo dos usuários.
RESUMO Estudo de caso que objetivou descrever o fluxo entre os níveis da atenção à saúde dos Paresi, do Distrito Sanitário Especial Indígena de Cuiabá, no Mato Grosso. O fluxo previsto pelo Plano Distrital de Saúde Indígena não considera a organização local de saúde e a articulação entre as instâncias do Sistema Único de Saúde, situação que foi prevista no fluxo estabelecido pelos profissionais de saúde. A estes soma-se o fluxo estabelecido pelos indígenas. Concluiu-se que o fluxo que consta do Plano Distrital de Saúde Indígena é inaplicável; o estabelecido pelos profissionais de saúde prevê aproximação ao real, mas a não-observação da particularidade gera empecilhos para o acesso a outros níveis de atenção.
RESUMO Neste artigo, pretendemos demonstrar o (des)compromisso do Estado com a conquista dos direitos indígenas à saúde a partir de cortes transversais ao longo dos séculos XX e XXI; e da análise grupal das organizações da saúde indígena em três contextos históricos: i) Serviço de Proteção ao Índio - SPI, em 1910, após a Proclamação da República; ii) Fundação Nacional do Índio - FUNAI, em 1968, após o Golpe Militar; e iii) Secretaria de Saúde Indígena - SESAI, em 2010, após a Constituição Federal Brasileira. Numa reflexão sobre o pensamento social de “ninguendade”, de Darcy Ribeiro, constatamos a não participação indígena na construção da nacionalidade brasileira sob a influência das ideias positivistas de assimilação presentes na criação do SPI e de integração da FUNAI, ainda embutidas na consciência dos brasileiros. Os direitos constitucionais emancipatórios na recomendação da interculturalidade na Política Nacional de Atenção à Saúde dos Povos Indígenas apresentou avanços e recuos nas últimas décadas na SESAI, sob gestão federal do Ministério da Saúde desde 2002.
Este artigo apresenta resultados e reflexões a partir de dois projetos de pesquisa que tiveram por objetivo avaliar a articulação da atenção básica com os demais níveis do sistema de saúde, considerando tanto a adequação às normas existentes (estratégia deliberada), quanto os desenvolvimentos criativos (estratégias emergentes) em diversos contextos locorregionais. Foram realizados cinco estudos de caso, dois no Rio Grande do Sul (municípios da fronteira Brasil-Uruguai e municípios vinculados ao Consórcio Regional de Saúde da Região Centro) e três no Mato Grosso (três Polos-Base do Distrito Sanitário Especial Indígena Cuiabá). Todos os casos estudados apresentaram um fluxo formal distinto do planejado. A definição das estratégias de acesso resultou de arranjos criativos organizados pelos profissionais de saúde e usuários, que vão além daquelas produzidas no âmbito do aparato administrativo.
