IMPORTANCE An intraoperative higher level of positive end-expiratory positive pressure (PEEP) with alveolar recruitment maneuvers improves respiratory function in obese patients undergoing surgery, but the effect on clinical outcomes is uncertain. OBJECTIVE To determine whether a higher level of PEEP with alveolar recruitment maneuvers decreases postoperative pulmonary complications in obese patients undergoing surgery compared with a lower level of PEEP. DESIGN, SETTING, AND PARTICIPANTS Randomized clinical trial of 2013 adults with body mass indices of 35 or greater and substantial risk for postoperative pulmonary complications who were undergoing noncardiac, nonneurological surgery under general anesthesia. The trial was conducted at 77 sites in 23 countries from July 2014-February 2018; final follow-up: May 2018. INTERVENTIONS Patients were randomized to the high level of PEEP group (n = 989), consisting of a PEEP level of 12 cm H 2 O with alveolar recruitment maneuvers (a stepwise increase of tidal volume and eventually PEEP) or to the low level of PEEP group (n = 987), consisting of a PEEP level of 4 cm H 2 O. All patients received volume-controlled ventilation with a tidal volume of 7 mL/kg of predicted body weight. MAIN OUTCOMES AND MEASURES The primary outcome was a composite of pulmonary complications within the first 5 postoperative days, including respiratory failure, acute respiratory distress syndrome, bronchospasm, new pulmonary infiltrates, pulmonary infection, aspiration pneumonitis, pleural effusion, atelectasis, cardiopulmonary edema, and pneumothorax. Among the 9 prespecified secondary outcomes, 3 were intraoperative complications, including hypoxemia (oxygen desaturation with SpO 2 Յ92% for >1 minute). RESULTS Among 2013 adults who were randomized, 1976 (98.2%) completed the trial (mean age, 48.8 years; 1381 [69.9%] women; 1778 [90.1%] underwent abdominal operations). In the intention-to-treat analysis, the primary outcome occurred in 211 of 989 patients (21.3%) in the high level of PEEP group compared with 233 of 987 patients (23.6%) in the low level of PEEP group (difference, −2.3% [95% CI, −5.9% to 1.4%]; risk ratio, 0.93 [95% CI, 0.83 to 1.04]; P = .23). Among the 9 prespecified secondary outcomes, 6 were not significantly different between the high and low level of PEEP groups, and 3 were significantly different, including fewer patients with hypoxemia (5.0% in the high level of PEEP group vs 13.6% in the low level of PEEP group; difference, −8.6% [95% CI, −11.1% to 6.1%]; P < .001). CONCLUSIONS AND RELEVANCE Among obese patients undergoing surgery under general anesthesia, an intraoperative mechanical ventilation strategy with a higher level of PEEP and alveolar recruitment maneuvers, compared with a strategy with a lower level of PEEP, did not reduce postoperative pulmonary complications.

O objetivo do estudo foi investigar qual o impacto na autoestima de mulheres vítimas de violência. Materiais e Métodos: Trata-se de um estudo de natureza qualitativa. A amostra foi selecionada por conveniência não probabilística e composta por 11 mulheres atendidas da delegacia da mulher do município de Campina Grande, Brasil. Utilizando a técnica de análise de conteúdo elencou-se as categorias: Impacto na saúde da mulher; Impacto na autoimagem e perda da identidade feminina e Submissão e perda da liberdade. Resultados: As idades variaram entre 20 e 68 anos, o a violência trouxe sofrimento e impactos na vida da mulher como perda de sua identidade, desestruturação na autoimagem, sentimento de impotência e redução da autoestima. Discussão e Conclusões: A carga de sofrimento de saúde que devem estar capacitados para realizar um atendimento integral, apoiando-a no seu empoderamento prestar assistência a mulher vítima de violência doméstica, buscando minimizar seus impactos. The aim of this study was to investigate the impact upon the self-esteem of women victims of violence. Materials and Methods: This was a qualitative study. The sample was selected through non-probabilistic convenience and comprised 11 women attending the women's police station in the city of Campina Grande, Brazil. By using the content-analysis technique, the following categories were listed: Impact upon the woman's health; Impact upon self-image and loss of female identity, and Submission and loss of freedom. Results: Ages varied from 20 to 68 years; number of feelings of impotence, and reduced self-esteem. Discussion and Conclusions: them integrally, taking them to depressive states. Este trabalho teve como objetivo investigar percepções de enfermeiros e médicos intensivistas no tocante à prática da eutanásia em pacientes terminais. Esta pesquisa qualitativa foi realizada entre novembro e dezembro de 2009 em duas unidades de terapia intensiva no município de Campina Grande, Paraíba. Foram entrevistados três médicos e dez enfermeiras. Os resultados apontaram as categóricas concepções acerca da eutanásia e o posicionamento em relação à prática da eutanásia. Os enfermeiros e médicos intensivistas reconhecem os sentimentos vivenciados pelos pacientes irreversíveis. As ostomias, ostomas, estomas ou estomias são derivados do grego e significam "boca" ou "abertura". Correspondem à exteriorização de vísceras ocas através do corpo, e são realizados, cirugicamente, devido a doenças congênitas, tumores, câncer intestinal, traumatismo e doenças inflamatórias. Esta condição, associada ao uso das bolsas coletoras, pode causar constrangimento, vergonha e dificuldade no convívio social ao indivíduo que a possui. Neste sentido, esta pesquisa teve como objetivo investigar as mudanças ocorridas na vida após a colostomia. Trata-se de um estudo exploratório e descritivo, com abordagem qualitativa, realizado na Associação dos Ostomizados do Agreste e Sertão da Paraíba no mês de agosto de 2009, onde foram entrevistados todos os associados, tendo em vista a questão norteadora "mudanças ocorridas na vida". Os depoimentos foram submetidos à análise temática do conteúdo, onde emergiram três categorias: "Discriminação", "Vida sexual" e "Alimentação". A análise demonstrou que os ostomizados enfrentam, no seu cotidiano, muita dificuldade de adaptação no âmbito social.
Background: To review the scientific literature on violence against women. Method: Integrative review carried out in the Virtual Health Library, in June and July 2015. These inclusion criteria were considered: full text and freely available; female public; studies carried out in Brazil; in Portuguese; with 2011-2014 time frame. Results: The sample was composed of 15 publications, all found in the LILACS (100%) database, of which 9 (60%) had publications in A 2 magazines, 4 (26.6%) in B 1 magazines and 2 (13.4%) in Qualis B 2 magazines. Most of the publications was Quantitative 8 approach (53.3%), showing different thematic perspectives and 7 (46.6%) were qualitative, applying violence in the perception of women and professionals who offer them assistance. Conclusions: It is essential to find coping strategies for violence, as well as the effective detection in primary health care.
Objetivo: compreender os fatores que influenciam a oferta da alimentação às crianças de zero a cinco anos na perspectiva das mães. Método: foi realizado um estudo descritivo e exploratório, com abordagem qualitativa que envolveu 16 mães. Os dados resultantes das entrevistas foram organizados por meio do software Atlas ti e submetidos a análise de conteúdo. Resultados: Entre as mães do estudo, 62,5% receberam informações, no pré-natal, sobre a importância do aleitamento materno; 56,2% disseram não ter participado de palestras sobre o aleitamento materno e 75% responderam que a criança não mamou na 1º hora de vida. Os significados das mensagens e dos conceitos apreendidos foram organizados em três categoriais: Amamentação - suas particularidades e seus determinantes; Início da Alimentação Complementar e suas modalidades; e A pandemia do Covid-19, como influenciador da rotina alimentar infantil. Conclusão: verificou-se uma multiplicidade de fatores que determinam a alimentação de crianças de 0 a 5 anos. As mães expressam o desejo de amamentar, mas esse não é um papel social fixo, exigindo muitas adequações da realidade que vão desde problemas materno-infantil, questões de contexto e legais, postura dos profissionais de saúde e organização dos serviços e até mesmo questões mais amplas como a pandemia da Covid 19. Por se tratar de situações diversas e mudanças constantes, as mães devem ser apoiadas no processo de amamentação/alimentação das crianças por uma equipe de profissionais preparados, o que implica na necessidade que as unidades básicas de saúde têm em se certificar na Estratégia Amamenta Alimenta Brasil.
