Multiple sclerosis (MS) may present with a cognitive impairment as disabling as the physical disabilities. Therefore, routine cognitive evaluation is pivotal. Valid and reliable neuropsychological tests are essential in follow-up and to define future therapeutic interventions. Objectives To investigate the correlation between the disabilities of MS patients and their cognitive impairment assessed by the Brief International Cognitive Assessment for Multiple Sclerosis (BICAMS). Methods Forty patients with definitive diagnoses of MS were selected. The correlation coefficient (r) between the Expanded Disability Status Scale (EDSS) and the neuropsychological tests of BICAMS were calculated. Results The correlation was clinically substantial and significant with r = 0.55 (p < 0.01) in the Symbol Digit Modalities Test (SDMT), 0.54 (p < 0.01) in the Brief Visuospacial Memory Test (BVMT) and 0.40 (p < 0.05) in the California Verbal Learning Test (CVLT). Conclusion BICAMS has easy and satisfactory application and evaluation for routine visits and presents a significant correlation with the EDSS. Its use may be indicated for screening and monitoring of cognitive impairment in patients with MS.
Patients with multiple sclerosis (MS) who present coronavirus disease 2019 (COVID-19) are of particular interest to neurologists. These patients have a neuroimmune disease and receive immunomodulatory or immunosuppressive therapies in the long-term. We present here data from 73 patients with MS and a confirmed diagnosis of COVID-19 from five Latin American countries. Fifteen patients (20.5%) were hospitalized and two patients died. The use of anti-CD20 therapies was the only risk factor associated to hospitalization and death. Despite the small sample size, this study highlights the awareness regarding therapeutic options for MS during the pandemic.
The Scientific Department of Neuroimmunology of the Brazilian Academy of Neurology (DCNI/ABN) and Brazilian Committee for Treatment and Research in Multiple Sclerosis and Neuroimmunological Diseases (BCTRIMS) provide recommendations in this document for vaccination of the population with demyelinating diseases of the central nervous system (CNS) against infections in general and against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which causes COVID-19. We emphasize the seriousness of the current situation in view of the spread of COVID-19 in our country. Therefore, reference guides on vaccination for clinicians, patients, and public health authorities are particularly important to prevent some infectious diseases. The DCNI/ABN and BCTRIMS recommend that patients with CNS demyelinating diseases (e.g., MS and NMOSD) be continually monitored for updates to their vaccination schedule, especially at the beginning or before a change in treatment with a disease modifying drug (DMD). It is also important to note that vaccines are safe, and physicians should encourage their use in all patients. Clearly, special care should be taken when live attenuated viruses are involved. Finally, it is important for physicians to verify which DMD the patient is receiving and when the last dose was taken, as each drug may affect the induction of immune response differently.
Cognitive Impairment (CI) is a common and distressing problem in Multiple Sclerosis (MS). Its identification is complicated and sometimes omitted in the routine evaluation by neurologists. The BICAMS (Brief International Cognitive Assessment for Multiple Sclerosis) is a promising tool to overcome this difficulty. However, there is some concern regarding the subjectivity in scoring of the BVMT-R (Brief Visuospatial Memory Test - Revised), one of its components.Objective:To evaluate the reliability of the BVMT-R in a sample of Brazilian MS patients, with the measure being administered and scored by neurologists.Methods:BICAMS was applied to seventy subjects comprising forty patients diagnosed with MS and thirty healthy controls. In the MS patients group, the coefficients of agreement between three different raters, using the same protocols, and the internal consistency of the BVMT-R were assessed. Also, the coefficients of correlation of the BVMT-R with the other tests of the BICAMS, CVLT II (California Verbal Learning Test II) and SDMT (Symbol Digit Modalities Test), and their respective effect sizes were calculated.Results:the BVMT-R presented a moderate inter-rater coefficient of agreement (k=0.62), an excellent Intraclass Correlation Coefficient (ICC=0.85), and high internal consistency (α=0.92). The correlation between the BVMT-R and CVLT II was moderate (ρ=0.36; p<0.025), but strong with the SDMT (ρ=0.60; p<0.01), with a large effect size.Conclusion:The BVMT-R is a reliable instrument for assessing CI in patients with MS, having a significant association with information processing speed, an aspect which should be considered when evaluating its score.
RESUMO -O objetivo deste estudo consiste em avaliar a prevalência de anticorpos antimicrossomais (AAM), a função tireóidea e a ocorrência de sintomas relacionados ao hipotireoidismo em pacientes com esclerose multipla (EM). Em um grupo de 21 pacientes com EM, foi realizado exameclínico, foram dosados o TSH, T4 e T4 livre e pesquisados AAM. A média de idade foi 41,05 anos e a média de tempo de doença foi 85,9 meses. Os sintomas relacionados ao hipotireoidismo foram fadiga, fraqueza, letargia e parestesias. Os AAM foram encontrados em 4 pacientes (19%). O tempo de doença foi dividido em três períodos: <60 meses (3 pacientes AAM+/7AAM-), 60-120 meses (8 pacientes AAM-) e >120 meses (1 paciente AAM+/2 AAM-). Dois pacientes apresentaram níveis de T4 livre diminuídos, porém com T4 e TSH normais. Em 1 paciente, constatou-se hipotireoidismo subclínico, e em outro, hipotireoidismo clássico. Conclui-se que na avaliação dos pacientes com EM, em vista da falta de precisão na avaliação clínica do hipotireoidismo ocasionada pela sobreposição de sintomas referentes à EM, devam ser incorporadas as dosagens das provas de função tireóidea (PFT) e dos AAM.PALAVRAS-CHAVE: esclerose múltipla, anticorpos antimicrossomais, hipotireoidismo. Prevalence of antimicrosomal antibodies in patients with multiple sclerosisABSTRACT -The aim of this study was to assess the prevalence of Antimicrosomal Antibodies AMA, thyroid function and the occurrence of hypothyroidism symptoms in patients with Multiple Sclerosis (MS). Clinical examination was carried out in 21 MS patients; thyroid-stimulating hormone (TSH), thyroxine (T4), free T4 and AMA were measured. Mean age was 41.05 years. Hypothyroidism symptoms included fatigue, weakness, lethargy and paresthesia. AMA were found in four patients (19%). Three categories of disease duration were considered: <60 months (3 patients AMA+; 7 AMA-), 60-120 months (8 patients AMA-), and >120 months (1 patient AMA+; 2 AMA-). Two patients presented decreased free T4 levels, but there was no associated decrease in T4 and TSH levels. In two patients, a mild increase in TSH levels was observed: one presented normal T4 levels (subclinical hypothyroidism) and the other one had low free T4 levels (classical hypothyroidism). INTRODUÇÃO: A Esclerose Múltipla (EM) é uma doença neurodegenerativa, inflamatória crônica que afeta o Sistema Nervoso Central. Embora importantes avanços terapêuticos tenham surgido nas últimas décadas, a população com EM apresenta taxas de hospitalização mais altas em relação a população geral, com impacto socioeconômico relevante. OBJETIVOS: Este estudo objetiva avaliar o uso de indicadores de gestão em pacientes com EM possibilitando melhorias no atendimento e manejo clínico desta população. MATERIAIS E MÉTODOS: Trata-se de um estudo observacional retrospectivo com analise de variáveis epidemiológicas e clínicas (idade, sexo, etnia, tempo de doença, ocorrência de surtos em 1 ano, EDSS, tratamentos, comorbidades). Quanto aos indicadores foram avaliados: Número de internações e pacientes internados (mensurando a produção), Tempo médio de permanência (mensurando a eficiência) e Reinternação e taxa de mortalidade (mensurando qualidade médico assistencial). RESULTADOS: Foram analisadas 332 internações em 153 pacientes (mensurando produção), resultando em média de 2.16 com valor máximo de 18 e mínimo de 1 internação por paciente, sendo a maioria em mulheres (75%). A média de idade foi de 36 ± 7.8 anos em 26.5% dos pacientes entre 18-39 anos. A maioria dos pacientes era do sexo feminino (75%) e autodeclarada branca (94.8%). A mortalidade hospitalar foi de 2.4% (mensurando a qualidade médico assistencial) e o tempo médio de permanência foi de 6.5 dias (mensurando a eficiência). O principal motivo de internação foi relacionado a exacerbação da doença, sendo a maioria dos pacientes internados para pulsoterapia. A maioria dos pacientes possuía outra comorbidade. CONCLUSÃO: Confirmou-se o elevado número de reinternações nesta população, sendo a EDSS da maior parte dos casos abaixo de 3.5. Internações de pacientes com EM podem ser reavaliadas afim de diminuir os custos hospitalares e, principalmente, aumentar a qualidade de vida destes casos, considerando a realização de procedimento a nível ambulatorial.

Embora importantes avanços terapêuticos para essa doença tenham surgido na última década, a população com EM apresenta taxas de hospitalização mais altas em relação a população geral e o impacto socioeconômico dessa doença permanece relevante. A construção do projeto atual é focada em estudar os indicadores de gestão em pacientes com Esclerose Múltipla, com objetivo de planejar melhorias no atendimento à essa população e as escolhas de manejo clínico. Trata-se de um estudo observacional retrospectivo, com inclusão de casos mediante prontuários clínicos. Serão analisados os seguintes dados: idade, sexo, etnia, tempo de doença, ocorrência de surtos em 1 ano, EDSS, tratamentos, comorbidades, objetivando identificar o perfil dos pacientes hospitalizados e elucidar quais são as variáveis capazes de auxiliar no diagnóstico mais precoce, visto a maior longevidade devido avanço terapêutico. Quanto aos indicadores, serão estudados: Número de Internações no período e de pacientes internados, mensurando a produção; Tempo Médio de Permanência, mensurando eficiência; Reinternação e taxa de mortalidade, mensurando qualidade médico assistencial. Internações de pacientes com EM podem ser reavaliadas afim de diminuir os custos hospitalares e, principalmente, aumentar a qualidade de vida destes casos, considerando a realização de procedimentos a nível ambulatorial.
