OBJECTIVE:To assess diffi culties in the accessibility to tuberculosis diagnoses in the health services in Brazil.
doi: 10.5216/ree.v12i3.5278A Estratégia Saúde da Família tem ganhado notoriedade no plano nacional, contudo alguns entraves denotam ser um campo pérvio a pertinentes estudos e discussões, neste tocante o estudo teve como objetivo: caracterizar a prática do enfermeiro na Estratégia Saúde da Família de Vitória, Espírito Santo. Estudo de Caso foi o caminho metodológico escolhido, baseado em evidências quantitativas, sendo os dados tratados a partir de distribuição de frequência simples, com aplicação de questionário nos meses de agosto a novembro de 2006. Participaram como sujeitos voluntários desta pesquisa 34 enfermeiros, que compunham as equipes da Saúde da Família, após a anuência do estudo e que este atende a Resolução 196/96 do Conselho Nacional de Saúde. Os resultados demonstraram que a prática do enfermeiro é direcionada de acordo com ações pré-estabelecidas pelo Ministério da Saúde e Secretaria Municipal de Saúde. Os dados evidenciam que os enfermeiros desempenham todas as ações minimamente preconizadas, todavia esta atuação parece não estar obtendo o suficiente impacto no que tange a prevenção de doenças e promoção da saúde, pois estão majoritariamente empenhadas nas ações curativas e administrativas. Limitações de cunho, organizacionais, estruturais e conceituais, foram citadas como principais fatores impeditivos para o desenvolvimento ideal das práticas de enfermagem. Purpose: To describe the knowledge, attitudes, and practice regarding the detection of prostate cancer among men aged between 50 and 80 years old attending a PSF of the municipal district of Juiz de Fora, MG. Methods: This was a cross-sectional home survey with a randomized sample of 160 men residents in the subscript of the program. Results: The majority of participants was married (69.4%), had children (88.8%), used the PSF (86.3%), and had knowledge about detection of prostate cancer. Great number of them had attitudes (40.6%) and engaged in adequate practice for the detection of prostate Participants with adequate attitudes reported almost twice adequate practice for the detection of prostate cancer (RP = 1.8; IC = 1.1 -3.0). Conclusion: The study's findings support that the use of constant, persistent, and dynamic educational activities is a fundamental requisite for the prevention and early detection of prostate cancer. RESUMOObjetivo: Analisar conhecimentos, atitudes e práticas em relação ao câncer de próstata de homens com idade entre 50 e 80 anos, adstritos à uma unidade do PSF no Município de Juiz de Fora -MG. Métodos: Estudo seccional realizado por meio de inquérito domiciliar abrangendo amostra aleatória de 160 homens residentes em área adscrita desse programa. Resultados: Da amostra, 69,4% eram casados, 88,8% tinham filhos, 86,3% informaram utilizar o PSF, 63,8% apresentaram conhecimento, 40,6% atitudes e 28,1% práticas adequadas. Os homens com conhecimento adequado tiveram prevalência 7,6 vezes (IC 95%=2,4-23,6) mais elevada de referir práticas adequadas. Aqueles com atitudes adequadas tiveram prevalência quase 2 vezes maior (RP=1,8; IC=1,1-3,0) de referir práticas adequadas. Conclusão: Este estudo corrobora a prevenção e a detecção precoce, como estratégias básicas para o controle do câncer de próstata, e têm como requisito essencial um conjunto de atividades educativas constantes, persistentes e dinâmicas para os homens. With the aim of describing barriers to screening for prostate cancer, a domicile survey was carried out covering 160 men of a Family Health Strategy (FHS) area. Slightly over half had undergone the examination. Regarding beliefs related to the disease, 95% of the men believed there is a cure if detected early, 29.4% mentioned the possibility of a normal life while ill, 56.3% believed it may be asymptomatic, 36.1% agreed/disagreed that the treatment is worse than the disease and 34.4% agreed that the examination affects masculinity and that if you are well it is not necessary to perform it. Regarding barriers, 15% reported that the physician had never requested it, 10.9% did not consider it important and 16.9% were afraid to take the examination. While not the sole determinant, the dissemination of adequate knowledge regarding the examination can constitute a key strategy for the formation of a positive attitude in relation to early detection. Barreras en relación a los exámenes de rastreo del cáncer de próstataCon el objetivo de describir barreras sobre rastreo del cáncer de próstata, se realizó una encuesta domiciliar abarcando 160 hombres de una área de la Estrategia de Salud de la Familia (ESF). Poco más de la mitad ya había realizado el examen. En relación a las creencias sobre la enfermedad, 95% de ellos creen en la cura si es detectado precozmente, 29,4% refirieron la posibilidad de vida normal a pesar de estar enfermo, 56,3% creen que puede ser asintomático, 36,1% está de acuerdo/no está de acuerdo que el tratamiento es peor que la enfermedad y 34,4% está de acuerdo que el examen afecta la masculinidad y si estuviese bien no sería necesario hacerlo. En cuanto a las barreras, 15% informaron que el médico nunca lo solicitó, 10,9% no lo encuentran importante y 16,9% tiene miedo de hacer el examen. A pesar de que no es el único determinante, diseminar los conocimientos adecuados sobre el examen puede constituirse en una estrategia fundamental para la formación de una actitud positiva en relación a la detección precoz.Descriptores: Religión; Neoplasias de la Próstata; Salud del Hombre.
Diante dos problemas apontados na literatura relacionados ao acesso na atenção primária à saúde, o estudo teve como objetivo analisar o acesso ao tratamento do doente com tuberculose em Itaboraí.Um estudo exploratório, quantitativo, com 100 indivíduos entrevistados em agosto e setembro/2007.Utilizou-se o instrumento Primary Care Assessment Tool validado e adaptado para tuberculose. Resultados: 71% sempre conseguem atendimento médico; 54% disseram que o profissional que acompanha seu tratamento nunca visita em casa; 77% fazem tratamento na unidade mais perto da sua casa; 66% nunca deixam de trabalhar e utilizam transporte motorizado, 62% pagam transporte; 93% relataram que nunca faltou medicamento; 63% relataram que nunca demora mais de 60 minutos para consulta. Aspectos como a visita domiciliar e a distribuição do auxílio transporte devem ser revistos. O atendimento médico, o tratamento próximo à residência, a oferta das medicações e a espera pelo atendimento foram satisfatórios. El 71% siempre reciben la atención médica, 54% dijeron que el profesional que les acompaña en el tratamiento nunca los ha visitado en casa, 77% realizan el tratamiento en la unidad más cerca de casa, 66% nunca dejan de trabajar y hacen el uso de transporte motorizado, 62% pagan el transporte, 93% informaran que nunca les faltó medicación, 63% dijeron que nunca se necesita más de 60 minutos para consulta. Aspectos como visitas a domicilios y la distribución de la ayuda de transporte deben ser revisados. La atención médica, el tratamiento cerca de casa, la oferta de medicamentos y el tiempo de espera para el atendimiento fueron satisfactorios. Results: 71 % always get medical service in 24 hours; 54 % related that the professional who supervises the treatment never visits their homes; 77 % receive treatment near their homes; 66% never miss the job and use motorized transportations; 62 % spend money for transportation; 93 % reported that never had lack of medications; 63 % reported that never wait more than 60 minutes for attendance. The fact is that some aspects like home visit, distribution of credit to transport needs to be reviewed. The medical service, the treatment near home, the offering of medications and the waiting for attendance was satisfactory.
