We propose an extended model of cardiovascular regulation to assess heart rate variability in patients poisoned with organophosphate during their treatment with mechanical ventilation. The model was modified to fit a population of twentyone (21) patients poisoned with organophosphorus compounds and undergoing mechanical ventilation. The extended model incorporated the respiratory muscle activity measured by surface electromyography for quantifying the vagal-sympathetic engagement during spontaneous breathing test. The order and structure of the parasympathetic and the sympathetic transfer function with respect to the original model were modified to a second-order system. In this extended model, the parameters related to the vagal-sympathetic response (corner frequency and constant gain) were correlated with respiratory muscle activity. When the diaphragm's contractions were stronger, the sympathetic corner frequency increased while the parasympathetic corner frequency and gain decreased. Thus the proposed model could be useful to improve the ventilatory support and pharmacological treatment for patients poisoned with organophosphorus compounds considering the vagalsympathetic response inferred from the respiratory muscle activity.
Abstract-This paper describes a prototype of a metabolic monitor designed for field work in biological anthropology, human ecology, and bioengineering. Such applications require robustness, reliability, easy replacement and low costs of usage and technical support. The purpose of this system is to monitor the resting energy expenditure (REE) and during physical activity, using the method of indirect calorimetry from expired CO 2 measurement and estimation of O 2 consumed. In the design and construction of the system have been used low-cost industrial sensors. The system performs continuous measurement of the pressure generated in the mouth and the CO2 produced during physical activity and stores this information on a removable flash memory. INTRODUCCIÓNLos estudios de ecología humana, nutrición y antropología biológica aportan valiosa información para el adecuado seguimiento, preservación, y estudio de la salud de las poblaciones [1]. Un aspecto que aporta información relevante por su carácter cuantitativo es la evaluación del gasto energético durante la actividad física [2], pero el estudio de poblaciones tradicionales (campesinos y pescadores) requiere estrategias de recolección de datos en situaciones de trabajo de campo fuera de las clínicas u hospitales [3]. La estimación del gasto energético se realiza por medio de calorimetría indirecta, los equipos utilizan la medición del consumo de oxigeno (VO 2 ) y la producción de CO 2 (VCO 2 ), para determinar el gasto energético, siendo el método más viable y preciso en condiciones de trabajo de campo [4].A nivel clínico, el gasto energético en reposo (REE), es un parámetro de gran importancia a la hora de estimar el balance energético, ya que se ha encontrado que un balance energético negativo está altamente relacionado con complicaciones y con el aumento en la tasa de morbilidad, sobre todo en pacientes ventilados cuyas condiciones de salud son críticas [5].Los equipos portátiles disponibles en el mercado para el monitoreo del REE están orientados al estudio de medicina deportiva, en ambientes controlados y caracterizados por su elevado costo y mantenimiento.Los sensores de O 2 y CO 2 utilizados por los equipos biomédicos tienen un costo muy elevado [6], [7] y dado que dichos sensores requieren recambios frecuentes, el costo de mantenimiento de estos equipos resulta muy elevado.
