Objetivos. Comparar diferentes métodos de extracción de ADN a partir de quistes y trofozoítos de Giardia spp. mediante la técnica de reacción en cadena de la polimerasa (PCR) convencional. Materiales y métodos. Se aislaron quistes de Giardia spp. a partir de 65 muestras coprológicas procedentes de hospitales de referencia nacional, obteniéndose una carga promedio de 5x10 4 parásitos. Asimismo, se cultivaron trofozoítos de Giardia intestinalis (ATCC® 30957™) obteniéndose una carga parasitaria de 5x10 6. Se compararon once métodos de extracción para quistes y seis para trofozoítos. La concentración y pureza del ADN extraído se determinó por espectrofotometría y el rendimiento de la extracción se evaluó mediante la amplificación de los genes beta giardina (bg) y glutamato deshidrogenasa (gdh) por PCR semi-anidada. Resultados. Se observó que el método I mostró la mayor concentración de ADN a partir de quistes (12,24 ng/µL), pureza (1,4) y mejor rendimiento (100% amplificación bg, 60% gdh) en comparación con los otros métodos evaluados. En el caso de los trofozoítos el método que no tuvo pretratamientos presentó la mayor concentración de ADN, pureza y rendimiento (26,56 ng/µL; 1,85; 100% amplificación bg y gdh). Conclusiones. Los pretratamientos mecánicos, de choque térmico y enzimáticos son necesarios para la ruptura de la pared quística de Giardia spp., siendo el marcador molecular bg de mayor rendimiento para detección de ADN de quistes. Los trofozoítos no requieren pretratamientos para lograr resultados satisfactorios. Se cuenta con una metodología reproducible para la extracción de ADN de Giardia spp. a partir de cualquier estadio evolutivo.
Objetivo: Determinar la frecuencia de enteroparasitosis en niños de 1 a 10 años de una comunidad rural del departamento de Lima-Perú. Material y métodos: Estudio observacional, descriptivo y transversal. La población de estudio fueron los niños de 1 a 10 años. Se usó el examen coproparasitológico microscópico directo y el método deGraham. Se analizaron los datos mediante el programa Microsoft Excel 2010. Resultados: Se estudiaron 36 (100%) niños, 22 (61,1%) resultaron ser mono parasitados y 10 (27,8%) multiparasitados. El parásito intestinal más observado fue el Blastocystis hominis en el 33,3% de los casos. Conclusiones: Se identificó a una población que en su gran mayoría presentaron carencias sanitarias y que vivían en condiciones rurales, y que tuvieron una alta frecuencia de enteroparasitosis tanto con el examen coproparasitológico microscópico directo como con el método de Graham.
ResumenSe reporta un caso de Fasciola hepatica en una mujer de 50 años de edad, natural de Huaral, procedente de un asentamiento humano del distrito de Ate en Lima, con un tiempo de enfermedad de siete días caracterizado al inicio por dolor abdominal tipo cólico de curso progresivo en el epigastrio, dolor que se irradia a hipocondrio derecho. Luego presentó luego náuseas y vómitos en cuatro oportunidades, de contenido alimenticio y bilioso, por lo que se decide su hospitalización previa atención por emergencia. Al examen físico se encontró conjuntivas oculares levemente ictéricas, abdomen balonado, blando, depresible, no visceromegalia, Murphy positivo, ruidos hidroaéreos presentes, tórax y pulmones sin alteraciones. Posteriormente a los exámenes auxiliares, se diagnosticó colecistitis calculosa aguda y fascioliasis coledociana. En el acto quirúrgico se extrajo tres especímenes identificados como Fasciola hepatica. La paciente después de recibir el triclabendazol, concurrió al Hospital para la evaluación y control, habiendo logrado mejoría y bienestar. AbstractWe report a case of Fasciola hepatica in a 50 year-old woman born in Huaral, Lima, and living in Ate Vitarte, district of Lima, Peru, who was admitted because of seven days of disease characterized by progressive abdominal pain. She visited Ate's Hospital because of right hipocondrial and epigastric cramping abdominal pain in the morning accompanied by nausea and vomiting in four opportunities. Conjunctives were slightly icteric, abdomen was distended and depressible, there was no visceromegaly, Murphy's sign was positive, and thorax and lungs showed no alterations. At cholecistectomy three specimens identified as Fasciola hepatica were extracted. Following treatment with triclabendazole, clinical course was uneventful.
