Extranodal Hodgkin lymphoma involving the breast is infrequent. Most cases reported in the literature were diagnosed by histology after lumpectomy. We present a Hodgkin lymphoma mimicking inflammatory breast carcinoma in a 57-year-old woman. The diagnosis was performed by fine-needle aspiration (FNA) of the breast lesion and the axillary lymph nodes with rapid on-site evaluation followed by immunocytochemistry, and it was confirmed by histology. The patient after first-line chemotherapy developed relapse/refractory disease. Salvage chemotherapy regimens were applied with poor results and severe toxicity. Total remission was achieved with monotherapy of brentuximab vedotin, a novel anti-CD30-targeted antibody drug conjugate. This is a unique case of breast HL with misleading clinical presentation initially diagnosed by cytology. FNA as a minimally invasive diagnostic tool was crucial in avoiding unnecessary breast surgery and further delay of chemotherapy. It is also the first report highlighting the importance of this novel immunotherapy in the management of refractory Hodgkin lymphoma with breast involvement.
Acute panmyelosis with myelofibrosis (APMF) is a rare, fatal hematological neoplasm that is characterized by the acute onset of cytopenias and fibrosis in the bone marrow in the absence of splenomegaly or fibrosis-related morphological changes in the RBCs. We present the case of a 59-year-old female who presented with a two-month history of anemia, leucopenia and a normal platelet count. The marrow was heavily fibrotic, and no aspirate material could be obtained; the biopsy showed extensive infiltration with small to medium size megakaryocytes, dysplastic changes in the erythroid compartment, and left shift in the myeloid cells. The patient was treated for four months with anabolic steroids (Danazol), growth factors and received regular blood transfusions. At 4 months after diagnosis, the patient was started on Lenalidomide, 10 mg/day for a 21-d-course along with growth factor support. At 6 months after treatment, the patient was transfusion-independent, had normalized blood counts, and, at 32 months on continuous lenalidomide treatment, her needs for growth factor support have been minimized. Repeat bone marrow biopsies showed a patchy distribution of fibrosis with areas of normal cellularity and morphology. To our knowledge, this is the first case for a medication that could reverse the fatal outcome of APMF.
Εισαγωγή: Οι μεταλλάξεις των γονιδίων NPM1 και FLT3, έχουν ιδιαίτερη προγνωστική αξία σε ασθενείς με οξεία μυελογενή λευχαιμία (ΟΜΛ). Μελέτες δείχνουν ότι ο συνδυασμός των ως άνω γονιδίων, μπορεί να αναδείξει υποπληθυσμούς με διαφορετική πρόγνωση, με ιδιαίτερη σημασία για τη θεραπεία και τη συνολική τους επιβίωση. Η επίπτωση, αλλά και η κλινική σημασία των μεταλλάξεων των NPM1 και FLT3 δεν έχουν καταγραφεί σε συνδυασμό στον Ελληνικό χώρο. Σκοπός της παρούσας μελέτης είναι η μελέτη των γονιδίων σε Έλληνες ασθενείς με ΟΜΛ και Μυελοδυσπλαστικά σύνδρομα (RAEB και CMML) και η αναζήτηση πιθανών συσχετίσεων της παρουσίας των μεταλλάξεων με τα ανοσοφαινοτυπικά και λοιπά εργαστηριακά ευρήματα.Υλικό και Μέθοδοι: Απομονώθηκε γενετικό υλικό από 140 ασθενείς, 109 με ΟΜΛ και 31 με μυελοδυσπλαστικά/μυελοϋπερπλαστικά νεοπλάσματα (RAEB και ΧΜΜΛ). Η FLT3-ITD μετάλλαξη μελετήθηκε με PCR, η σημειακή μετάλλαξη με PCR-RFLP, ενώ της Νουκλεοφωσμίνης με τη μέθοδο του Fragment analysis. Καταγράφηκαν τα κλινικά και εργαστηριακά χαρακτηριστικά των ασθενών, ο καρυότυπος και η ανοσοφαινοτυπική υπογραφή των βλαστών, με κυτταρομετρία ροής. Αποτελέσματα: Στους ασθενείς με ΟΜΛ η συχνότητα εμφάνισης της FLT3-ITD, της FLT3-TKD και της μετάλλαξης της NPM1 ήταν 11,9%, 6,4% και 17,4% αντίστοιχα. Οι FLT3-ITD+ είχαν επιθετικότερη νόσο, με υψηλότερα λευκά (p<0.001), έτσι που αριθμός λευκών >60000/mm3 είχε ειδικότητα 90% για ανεύρεση της μετάλλαξης και υψηλότερη LDH (p=0.006), έτσι που φυσιολογική τιμή απέκλειε με ευαισθησία 95% την παρουσία της. Οι ασθενείς είχαν 7πλάσια πιθανότητα να έχουν και τη μετάλλαξη της NPM1 και οι βλάστες είχαν χαμηλότερη (<50%) έκφραση CD34 (p=0,008). Οι FLT3-TKD+ είχαν συνήθως λεμφαδενοπάθεια (p=0,026) και οι βλάστες τους εξέφραζαν CD14 (p<0,001). Οι NPM1+ είχαν συνήθως de novo ΟΜΛ (p=0,039) και υψηλότερη LDH (p<0,001). Μάλιστα, φυσιολογική τιμή LDH πρακτικά επέκλειε με ευαισθησία περίπου 100% την παρουσία της. Ανοσοφαινοτυπικά, οι βλάστες είχαν χαμηλότερη έκφραση CD34 (p<0,001), CD13 (p=0.009) και CD117 (p=0,003), απουσία έκφρασης HLA-DR (p=0,038), ενώ σχετίσθηκαν θετικά με την έκφραση CD116 (p for linear trend<0,001), CD15 (p=0.014) και CD36 (p=0,007). Οι ασθενείς είχαν 8πλάσια πιθανότητα να φέρουν και την FLT3-ITD, ενώ με τη σημειακή δεν συσχετίσθηκαν.Η συχνότητα των τριών μεταλλάξεων στους Μυελοδυσπλαστικούς ασθενείς ήταν 6,4% για την καθεμία. Συμπεράσματα: Οι Θεσσαλοί ασθενείς με ΟΜΛ φέρουν την FLT3-ITD μετάλλαξη με τη μισή συχνότητα από ό,τι αναφέρεται στη βιβλιογραφία, ενώ παρουσιάζουν ίδια βιολογικά χαρακτηριστικά. Το ίδιο συμβαίνει και για τις μεταλλάξεις της Νουκλεοφωσμίνης. Οι 2 μεταλλάξεις συχνότατα συνυπάρχουν, συνδυάζοντας τα βιολογικά χαρακτηριστικά των νόσων, καθιστώντας απαραίτητη τη μελέτη και των 2 γονιδίων και στους Έλληνες ασθενείς. Από την ανάλυσή μας προκύπτει επίσης, πως απλές και χωρίς ιδιαίτερο κόστος μετρήσεις (αριθμός λευκών και LDH) έχουν ίσως μεγαλύτερη σημασία για την πρόγνωση, αλλά και θα μπορούσαν να βοηθήσουν τον κλινικό Αιματολόγο στην Ελλάδα να επιλέξει σε ποιους ασθενείς να στείλει μοριακό έλεγχο, μειώνοντας το κόστος της αρχικής διάγνωσης και κατάταξης του λευχαιμικού σε ομάδα κινδύνου. Στους 31 ασθενείς με RAEB και CMML που συμπεριλήφθηκαν στη μελέτη, η συχνότητα είναι διπλάσια, εως δεκαπλάσια από ό,τι αναφέρεται στη διεθνή αρθρογραφία, το οποίο μπορεί να κάνει στους Έλληνες πιο εύκολες τις μελέτες των μεταλλάξεων σε προλευχαιμικές καταστάσεις.
