Objetivos: Estima-se que mais metade das mulheres com cancro da mama ou cancro ginecológico apresentem problemas sexuais. Contudo, apenas uma minoria recebe aconselhamento médico nesse âmbito. A presente revisão narrativa tem como objetivo descrever os principais problemas sexuais das mulheres com esta tipologia de neoplasias, bem como identificar estratégias de abordagem e tratamento dos mesmos. Métodos: Foi efetuada uma pesquisa da literatura na base de dados PubMed, utilizando os termos MeSH: Sexuality, Neoplasms, Female e Women. Foram utilizados livros e artigos datados entre 1998 e 2020. Resultados: O cancro da mama afeta a vida sexual desde o diagnóstico, associando-se a intenso sofrimento psicológico. A este acresce o impacto do tratamento. A cirurgia altera a autoimagem, sensibilidade mamária e mobilidade. A radioterapia condiciona alterações tecidulares. A quimioterapia apresenta efeitos psicológicos e físicos com impacto direto e indireto na sexualidade. A terapêutica hormonal condiciona atrofia urogenital e sintomas vasomotores. Por outro lado, também a maioria das mulheres com cancro ginecológico apresenta vários níveis de disfunção sexual secundários à terapêutica. Tratamentos mais recentes como a braquiterapia, técnicas de nerve-sparing modificadas e cirurgia reconstrutiva têm contribuído para uma diminuição da morbilidade associada ao tratamento. Verifica-se uma consciencialização crescente da importância da saúde sexual dos doentes oncológicos. Neste contexto, cabe ao médico abordar o assunto, informando as doentes acerca do possível impacto da doença e do respetivo tratamento na sexualidade. Existem questionários validados e modelos estruturados facilitadores de comunicação neste sentido. Cabe também aos clínicos o controlo dos sintomas de disfunção sexual associados à terapêutica oncológica, privilegiando sempre que possivel fármacos com impacto neutro ou positivo na sexualidade. Conclusão: Qualquer médico deve estar sensibilizado para a disfunção sexual associada à patologia oncológica e ao seu tratamento, sendo capaz de a rastrear e adaptar o plano terapêutico em conformidade, colocando em prática o cancer care continuum.
Introduction: There are several barriers discouraging clinicians from undertaking research, including insufficient funding, lack of time, organizational issues and lack of support. The strengthening of research capacity is perceived from three levels: characteristics of the researcher, the environment, and organizational issues. To date, Portugal is lacking studies on this subject. The aim of this study was to identify the best practices to promote research in Portuguese Primary Health Care.Methods: We conducted a qualitative study using semi-structured interviews with family doctors with broadly recognized research work and other stakeholders. We selected a sample by convenience and snowball sampling. From a total of 14 doctors invited by email, 12 responded positively, and we subsequently included two other stakeholders. We conducted the interviews in digital or face-to-face formats. Two team members handled the coding of interviews independently. We kept all recordings and transcripts confidential, only accessible to researchers.Results: We identified 16 strategies: 1) increasing institutional support; 2) creating support structures; 3) redefining the residency program; 4) investing in research training; 5) redefining curriculum evaluation; 6) establishing dedicated time for research; 7) increasing funding; 8) improving access to research data; 9) being a research driver; 10) establishing a research culture; 11) working in collaboration; 12) creating formally organized research groups; 13) creating autonomous research centers; 14) improving the definition of the research subjects and study designs; 15) reviewing procedures for ethics’ committees; and 16) reviewing the current selection of articles for publication.Conclusion: Overall, a greater proportion of interviewees identified the following as the most relevant strategies for research promotion: institutional support, including technical and scientific support from public institutions, private entities and academic centers; the reorganization of working hours with protected time for research; increased funding directed towards research and breaking isolation in research, promoting teamwork with clinicians within the same area or from different professional backgrounds.
Introdução: Os abcessos das glândulas de Skene são raros e subdiagnosticados. O seu diagnóstico baseia-se na anamnese e no exame objetivo. Descrição do caso: O presente caso clínico relata o seguimento de uma mulher de 26 anos que recorreu ao centro de saúde por surgimento de uma massa perineal de novo, sensação de esforço para iniciar a micção e repleção vesical. À observação apresentava uma massa procidente periuretral com 2cm de diâmetro, localizada na parede vaginal anterior, com envolvimento do meato urinário, aflorando aos grandes lábios. Foi colocada a hipótese de prolapso uro‑vaginal e pedido estudo ecográfico. Aproximadamente um mês mais tarde, a utente regressa à consulta por dor perineal intensa desencadeada pela introdução da sonda ecográfica endovaginal. Ao exame físico apresentava a mesma massa periuretral – agora com sinais inflamatórios locais francos e saída de exsudado purulento abundante pela uretra. O exame ecográfico efetuado não descrevia quaisquer alterações relevantes. Assumiu-se, como hipótese mais provável, a formação de um abcesso das glândulas de Skene. A utente foi então medicada com azitromicina e cefuroxima, com resolução do quadro. Comentário: Os abcessos das glândulas de Skene apresentam-se como massas periuretrais, associadas a exsudado purulento, dor uretral e/ou alterações urinárias. A evolução temporal do quadro clínico pode ser crucial para o estabelecimento do diagnóstico. Uma vez estabelecido o diagnóstico preconiza-se a antibioticoterapia empírica, tendo em conta os agentes mais frequentemente implicados (Escherichia coli, Neisseria gonorrhoeae e agentes da flora vaginal). Deste modo, perante uma massa periuretral dolorosa em utentes do sexo feminino, a lista de diagnósticos diferenciais deverá incluir a hipótese de abcesso das glândulas de Skene, de forma a não perder a oportunidade de tratamento precoce.
