Comparison of CT results with diagnostic reference levels revealed the need for revisions, partly because the newer scanners have improved technology that facilitates lower patient doses.
SummaryBackgroundTo evaluate the impact of the Clarity IQ technology on reducing radiation risk in patients undergoing cardiac interventional radiology (IR) procedures.Material/MethodsPhantom studies were performed with two angiographic systems, FD10 Allura Xper and FD10 Allura Clarity. In the study, we performed left atrial appendage closure. Dosimetric measurements were performed with thermoluminescent dosimeters (TLD) placed inside a CIRS anthropomorphic phantom. Radiation risk was estimated based on the TLD readings and expressed as the dose absorbed by particular organs. The Mann–Whitney U test was carried out to test for significance of differences in the absorbed radiation doses between the techniques.ResultsDuring left atrial appendage closure, the estimated dose absorbed by particular organs was lower in the case of the FD10 Allura Clarity system in comparison to the Allura Xper. In this procedure, dose reduction for particular organs ranged between 49–86%.ConclusionsApplication of the FD10 Allura Clarity system resulted in a significant dose reduction, thereby leading to a significant decrease in radiation risk for patients undergoing IR procedures.
This paper presents the results of phantom studies to investigate possible dose reduction in relation to image quality in head examinations. The studies were performed using five single-slice computed tomography (CT) scanners. Beginning from the manufacturer's protocols (i.e. default protocols in the scanner software) for routine head (adult) examinations, the values of kV(p), anode current and time were modified. Low-contrast resolution and spatial resolution were controlled using a Catphan 424 phantom. Radiation dose was checked using a polymethylmethacrylate phantom and a pencil ionisation chamber. It was found that CT dose index may be reduced up to three times with practically no loss of image quality.
StreszczenieWstęp: Wykorzystanie promieniowania rentgenowskiego (rtg.) w gastroenterologii stanowi jeden z licznych podobszarów radiologii interwencyjnej. W gastroenterologii fluoroskopia rtg. jest używana w procedurach terapeutycznych, z których częsta jest endoskopowa cholangiopankreatografia wsteczna (ECPW). Procedura ECPW pokazuje radiologiczny obraz dróg żółciowych i przewodu trzustkowego. Materiał i metody: W ramach niniejszej pracy przeprowadzono ocenę narażenia na promieniowanie rtg. gastroenterologa wykonującego zabiegi ECPW pod kontrolą fluoroskopii. Badania przeprowadzono w Pracowni ECPW Uniwersyteckiego Szpitala Klinicznego im. Wojskowej Akademii Medycznej -Centralnego Szpitala Weteranów w Łodzi, gdzie gastroenterolog wykonuje je samodzielnie. Realizując cel pracy, wykonano 2 serie pomiarów -pierwsza składała się z zabiegów wykonanych w trybie fluoroskopii ciągłej, druga -z zabiegów, które wykonano w trybie fluoroskopii impulsowej (o częstotliwo-ści 3 pulsy/s). Podczas każdej procedury rejestrowano parametry ekspozycji, dane anatomiczne pacjenta, a także otrzymywane przez gastroenterologa równoważniki dawek dla soczewek oczu i skóry dłoni oraz dawkę efektywną dla całego ciała. Wyniki: Zebrano dane dotyczące ogółem 70 zabiegów ECPW, w tym 40 zabiegów wykonanych w trybie fluoroskopii ciągłej i 30 -w trybie fluoroskopii impulsowej. Na podstawie uzyskanych wyników stwierdzono, że gastroenterolog wykonujący procedury ECPW w trybie fluoroskopii impulsowej otrzymuje dawki promieniowania niższe o 45-60% niż podczas pracy w trybie fluoroskopii ciągłej. Wnioski: Procedury ECPW mogą być źródłem narażenia na promieniowanie rtg. dla wykonującego je gastroenterologa. Używanie trybu fluoroskopii ciągłej może pozwolić na osiągnięcie obniżonej wartości rocznego limitu dawki równoważnej dla soczewek oczu (tj. 20 mSv), rekomendowanej przez Międzynarodową Komisję Ochrony Radiologicznej (International Commission on Radiological Protection -ICRP). StreszczenieWstęp: Rozszerzone monitorowanie narażenia personelu wykonującego zabiegi radiologii interwencyjnej jest od kilku lat wymagane prawnie. Poziom tego narażenia zależy od wielu czynników, w tym rodzaju zabiegu oraz uwarunkowań technicznych i metodycznych. W ortopedii fluoroskopia rentgenowska jest wykorzystywana do kontroli rekonstrukcji chirurgicznych. W pracy przedstawiono wyniki pomiarów dozymetrycznych, na których podstawie analizowano wpływ poszczególnych czynników na zarejestrowane wartości dawek otrzymywanych przez członków zespołu zabiegowego podczas osteosyntezy kości kończyn. Materiał i metody: Badania przeprowadzone w ramach niniejszej pracy polegały na pomiarach dawek otrzymywanych przez osoby pełniące określone funkcje w zespole operacyjnym podczas serii kilkunastu zabiegów ortopedycznych polegających na zespalaniu (osteosyntezie) kości kończyn. Każdy z 3 członków zespołu otrzymał zestaw 4 indywidualnych dawkomierzy zawierających dozymetry termoluminescencyjne, co umożliwiło ocenę dawki efektywnej, dawek równoważnych dla oczu, skóry dłoni i dawki na poziomie szyi. Badania prowadzono na blokach operacyjnych 3 łódzkich szpitali. Wyniki: Dawki równoważne otrzymywane przez operatora podczas 1 zabiegu wynoszą 0,029-0,073 mSv dla oczu i 0,366-1,604 mSv dla skóry dłoni. Istotnie wyższe dawki otrzymywane są zwłaszcza przez lekarza operatora podczas zabiegów osteosyntezy metodą śródszpikową (współczynnik Pearsona R 2 = 0,94). Średni wiek pacjentów i wskaźnik masy ciała nie różniły się statystycznie w grupach operowanych w monitorowanych szpitalach. Wnioski: Z analizy wyników pomiarów dozymetrycznych wynika, że w ortopedycznych procedurach radiologii interwencyjnej (interventional radiology -IR) największy wpływ na poziom narażenia personelu ma rodzaj wykonywanego zabiegu, a dokładniej poziom trudności jego przeprowadzenia. Kolejnym ważnym czynnikiem jest optymalizacja warunków pracy aparatu, w tym wykorzystanie fluoroskopii impulsowej. Najbardziej narażonym członkiem zespołu zabiegowego jest lekarz operator. Med.
