r e v i s t a c o l o m b i a n a salud libre 98Revista Colombiana Salud Libre. 2016; 11 (supl) ResumenLa hipofosfatasia es una enfermedad congénita caracterizada por una deficiencia de fosfatasa alcalina no específica de tejido, que genera una mineralización anormal del tejido óseo y dental. Las manifestaciones clínicas son variables, desde formas neonatales con alta mortalidad, hasta formas más leves del adulto con fracturas por fragilidad y osteomalacia. El diagnóstico bioquímico se basa en la determinación de valores séricos bajos de fosfatasa alcalina e incremento sérico o urinario de fosfoetanolamina, piridoxal 5'-fosfato y pirofosfato inorgánico. El tratamiento ha tenido resultados limitados. Dentro de estos se ha estudiado la administración de hormona paratiroidea en la hipofosfatasia del adulto y el tratamiento de sustitución enzimática con una forma soluble de fosfatasa alcalina recombinante humana en las formas clínicas más graves.Palabras clave: Fosfatasa alcalina, Hipofostasia, Raquitismo. AbstractHypophosphatasia is a congenital disorder characterized by a deficiency of non-specific alkaline phosphatase tissue, which generates an abnormal mineralization of bone and tooth tissue. The clinical manifestations are variable, from neonatal forms with high mortality, to milder forms of adult fragility fractures and osteomalacia. The biochemical diagnosis is based on the determination of low serum alkaline phosphatase and serum or increased urinary phosphoethanolamine, pyridoxal 5'-phosphate and inorganic pyrophosphate. The treatment has had limited results. Within these has studied the administration of parathyroid hormone in adult hypophosphatasia and enzyme replacement therapy with a soluble form of recombinant human alkaline phosphatase in the most severe clinical forms.
La hemofilia B o enfermedad de Christmas se diferenció por primera vez de la hemofilia A en 1947. Su forma clásica consiste en un trastorno hereditario de la coagulación causado por mutaciones en el gen F9, que codifica para el factor IX de la coagulación. Su herencia está ligada al cromosoma X; las mujeres son portadoras, pero se manifiesta clínicamente en hombres, aunque se han descrito casos de mujeres portadoras sintomáticas. El factor IX activado es una proteína dependiente de vitamina K, sintetizada en el hígado, que forma parte del complejo tenasa, cuya función es formar la mayor cantidad de trombina en el nuevo modelo de la coagulación basado en células. De acuerdo a la actividad del factor IX, su deficiencia se puede clasificar en leve (5% a 40%), moderada (1% a 5%), o severa (<1%). Su diagnóstico se realiza con la presencia de un TPT alargado que corrige con plasma normal y con la determinación del nivel funcional del factor IX, y se confirma con el estudio molecular que demuestra la mutación en el gen F9. Su diagnóstico diferencial incluye otras patologías como la hemofilia A. El tratamiento con factorIX recombinante es el más utilizado en la actualidad, pero se vienen desarrollando nuevas terapias con virus adeno-asociados recombinantes que prometen mejorar la calidad de vida para algunos pacientes afectados. La profilaxis juega un papel fundamental, en particular en los casos de enfermedad moderada y severa.
Introducción: El síndrome de Goldenhar es un trastorno caracterizado por la presencia de anomalías craneofaciales y vertebrales. Fue descrito inicialmente en 1950, sin embargo sus aspectos moleculares no han sido totalmente aclarados. Caso clínico: Paciente femenina de 9 meses de edad en quien se evidenciaron desde su nacimiento características fenotípicas de esta desconocida condición; se incluye una revisión de los aspectos más importante de este síndrome. Conclusiones: El abordaje inicial del paciente con diagnóstico de síndrome de Goldenhar es complejo, se deben considerar diversos diagnósticos diferenciales para llegar a un abordaje correcto con tratamiento temprano que además debe ser multidisciplinario e integral. Introducción: El síndrome de Larsen (SL) es un trastorno caracterizado por la presencia de luxaciones congénitas múltiples de diversas articulaciones que incluyen rodillas, caderas y codos, además de la deformidad podálica marcada, pie equino varo y valgo, asociado con rasgos fenotípicos característicos como depresión del puente nasal, hipertelorismo y frente prominente. Fue descrito inicialmente 1950 por Larsen et al. Se presenta el caso de un recién nacido con esta rara y distintiva displasia esquelética conjunta, se describen adicionalmente hallazgos clínicos y revisión de la literatura médica disponible.Caso clínico: Recién nacido, hijo de madre de 17 años con antecedente de uso de misoprostol en dosis de 2200 mµg vía vaginal y 800 mµg vía oral durante las semanas 4 y 16 de gestación; interconsulta al servicio de genética clínica por presentar múltiples luxaciones articulares bastante llamativas sobre todo la presencia de flexión de caderas e hiperextensión de rodillas que le otorgaban una posición bastante peculiar. Después de un estudio clínico y paraclínico se llegó al diagnóstico de síndrome de Larsen. Conclusiones: El abordaje inicial del paciente con síndrome de Larsen es complejo, el reconocimiento puede ser limitado y merece un enfoque multidisciplinario que mejora la evaluación clínica y el tratamiento rehabilitador de los pacientes, lo que a su vez mejora el pronóstico.
Proporcionar a los profesionales de la salud, conocimientos básicos en la detección, abordaje y seguimiento de las anomalías congénitas del riñón y de la vía urinaria. Materiales y métodos: Revisión no sistemática de la literatura, se accedió a las bases de datos PubMed, LILACS, Science Direct, Tripdatabase, Cochrane Library, Biblioteca Virtual en Salud y revistas de nefrología pediá- trica. Se incluyeron publicaciones de los últimos 25 años. Se utilizaron los términos MeSH “Congenital Anomalies of the Kidney and Urinary Tract, Hydronephrosis, risk factors, prenatal diagnosis, congenital abnormalities”. Resultados: Se han identificado dos importantes factores de riesgo para desarrollar estas patologías, el primero la Diabetes pregestacional, es decir“, aquella diabetes conocida previamente a la gestación, bien diabetes mellitus tipo 1 (DM1), diabetes mellitus tipo 2 (DM2) o intolerancia a los carbohidratos”, en segundo lugar el bajo o tardío consumo de micronutrientes de la gestación. Conclusiones: La diabetes mellitus pregestacional y el bajo consumo de micronutrientes antes o durante la gestación, se ven relacionado con el desarrollo de las anomalías renales y de la vía urinaria en el feto, aunque no las explican en la mayoría de casos.
