ResumoA leitura e a escrita são atividades complexas, compostas por múltiplos processos. No Brasil, não há consenso sobre os desempenhos em leitura e escrita esperados para cada série escolar, além da diversidade de formas de avaliação destas habilidades. Este estudo visa analisar, em uma abordagem neuropsicológica cognitiva, a precisão no uso das rotas de leitura em voz alta e escrita de palavras isoladas de 109 crianças de segunda série de escolas estaduais. Os resultados evidenciaram maior precisão no uso da rota fonológica de leitura e de escrita, em função do melhor desempenho nas pseudopalavras do que nas palavras irregulares, dos fortes efeitos de regularidade e extensão e dos erros de neologismos e regularizações. Porém, houve indícios de que a rota lexical também é usada. Conclui-se que as crianças de 2ª série de escolas públicas usam ambas as rotas de leitura e de escrita de palavras isoladas, mas preferencialmente a rota fonológica. In Brazil there is no consensus regarding the expected performance for reading and writing for each year of schooling, besides there being a diversity of evaluation forms for these abilities. This study aims to analyze, in a neuropsychological cognitive approach, the precision in the use of routes of oral reading and writing of isolated words in 109 children attending the 2 nd grade in state schools. The results showed a greater precision in the use of the phonological reading and writing route, due to the better performance in the use of pseudo-words than in the use of irregular words, high effect of regularity and extension and, the mistakes in neologisms and regularizations. However, there were signs that the lexical route is also used. It can be concluded, that children of the 2 nd grade of state schools, use both routes of reading and writing of isolated words, but preferably the phonological route. A leitura pode ser estudada sob vários aspectos: sócio-cultural, afetivo, pedagógico e cognitivo. O presente estudo baseia-se na abordagem cognitiva da leitura, ou seja, procura desvendar os processos cognitivos subjacentes a esta habilidade, tendo como aportes teóricos a Psicologia Cognitiva, abordagem de Processamento da Informação, e a Neuropsicologia Cognitiva.Muitos estudos têm sido feitos a respeito da aquisição e desenvolvimento da leitura. Alguns autores nacionais que adotaram os processos de leitura em crianças como tema de pesquisa foram Pinheiro (1994), Capovilla e Capovilla (2000), Cardoso-Martins (1991, Rego (1995), Kato (1999) e Cielo (1996 Resumo A leitura é composta por múltiplos processos interdependentes. Este estudo analisou o uso preferencial de uma das rotas de leitura (Modelo Cognitivo de Dupla-Rota) e as possíveis relações com compreensão e tempo de leitura em 76 crianças de segunda e terceira séries do Ensino Fundamental de escola particular. Avaliou-se a leitura de palavras isoladas e a compreensão e tempo de leitura textual. Identificou-se quatro grupos segundo as habilidades de leitura de palavras: bons leitores por ambas as rotas; maus leitores por ambas as rotas; leitores preferencialmente lexicais e leitores preferencialmente fonológicos. Os primeiros foram os mais rápidos na leitura textual. Os grupos não diferiram significativamente em compreensão textual, exceto na 3ª série. Concluiu-se que ambas as rotas de leitura são funcionais na amostra, porém a rota fonológica parece estar melhor desenvolvida do que a rota lexical, que está em expansão, sugerindo um processo de desenvolvimento das habilidades de leitura. Cognitive Processes Involved in Childrens Word Reading: Relations with Reading Comprehension and Reading Time AbstractMultiple interdependent processes compose the reading. The present study analyzed the preferential usage on one of the reading routes (Dual Route Cognitive Reading Model) and the possible relations with reading comprehension and reading time in 76 children from second and third grades, in a Elementary private school. The reading of isolated words, comprehension and textual reading time were evaluated. Four groups were identified according with the skills of word reading: good readers in both routes (phonological and lexical), poor readers in both routes, preferably lexical readers and preferably phonological readers. The group of good readers in both routes was the fastest in textual reading. There werent significant differences among the groups in reading comprehension, except in the third grade. It was concluded that both reading routes were functional in the sample, however the phonological route seems to be better developed than the lexical route, which is in expansion, suggesting a developmental process of the reading skills.
This study provided a systematic review on nonpharmacological interventions applied to patients diagnosed with Primary Progressive Aphasia (PPA) and its variants: Semantic (SPPA), Nonfluent (NFPPA) and Logopenic (LPPA) to establish evidence-based recommendations for the clinical practice of cognitive rehabilitation for these patients.METHODSA PubMed and LILACS literature search with no time restriction was conducted with the keywords PPA (and its variants) AND rehabilitation OR training OR intervention OR therapy OR treatment OR effectiveness. To develop its evidence-based recommendations, a research committee identified questions to be addressed and determined the level of evidence for each study according to published criteria (Cicerone et al., 2000). Overall evidence for treatments was summarized and recommendations were derived.RESULTSOur search retrieved articles published from 1995 to 2013: 21 for SPPA, 8 for NFPPA, 3 for LPPA and 8 for PPA with no specification. Thirty-five studies were rated as Class III, consisting of studies with results obtained from one or more single-cases and that used appropriate single-subject methods with adequate quantification and analysis of results. The level of evidence of three functional interventions could not be established. One study was rated as Class II and consisted of a nonrandomized case-control investigation.CONCLUSIONPositive results were reported in all reviewed studies. However, in order to be recommended, some investigation regarding the intervention efficacy was required. Results of the present review allows for recommendation of some nonpharmacological interventions for cognitive deficits following PPA as Practice Options. Suggestions for further studies on PPA interventions and future research are discussed.
TEMA: um instrumento de avaliação de déficits comunicativos após lesão de hemisfério direito: Bateria Montreal de Avaliação da Comunicação, versão brasileira adaptada do instrumento original canadense Protocole Montréal d'Évaluation de la Communication. Ferramentas de avaliação dos déficits discursivos, pragmáticos, léxico-semânticos e prosódicos são necessárias para o diagnóstico dos distúrbios da comunicação presentes em aproximadamente 50% dos indivíduos lesados de hemisfério direito. O quadro comunicativo após acometimento desse lado do cérebro vem sendo estudado sistematicamente há apenas duas décadas. OBJETIVO: apresentar a Bateria Montreal de Avaliação da Comunicação aos fonoaudiólogos brasileiros. CONCLUSÃO: o instrumento apresentado mostra-se uma ferramenta clínica útil no exame das habilidades lingüísticas e comunicativas relacionadas a quatro processamentos: discursivo, pragmático-inferencial, léxico-semântico e prosódico. Está normatizado, validado e sua fidedignidade foi confirmada. Embora tenha sido construído e adaptado para o exame dos distúrbios comunicativos em pacientes lesados de hemisfério direito, também pode auxiliar na investigação de seqüelas na comunicação em quadros de traumatismo crânio-encefálico, demência, lesões frontais bilaterais, lesões de hemisfério esquerdo, psicopatologias como a esquizofrenia, entre outros.
Resumo O presente estudo teve por objetivo investigar a tomada de decisão em jovens e idosos. O instrumento utilizado foi a Iowa Gambling Task, a qual foi adaptada para o Português Brasileiro no presente estudo. Participaram do estudo 82 indivíduos, 42 jovens de 20 a 35 anos e 40 idosos de 61 a 78 anos. Os resultados demonstraram que não houve diferença entre os grupos quanto às escolhas de cartas de cada baralho. Ambos os grupos selecionaram mais cartas dos baralhos "A" e "C" e menos cartas dos baralhos "B" e "D". Não houve diferença entre os grupos, no que se refere ao aprendizado desenvolvido ao longo da tarefa. Jovens e idosos foram gradualmente adquirindo uma predileção pelos baralhos "C" e "D", a qual culminou no quarto bloco, porém não se manteve no último. Peculiaridades na aplicação do instrumento podem responder pela não manutenção do aprendizado. Palavras-chave: Tomada de decisão; envelhecimento; busca e aversão ao risco.
Objective: To present performance norms and discrepancy score of three one-minute verbal fl uency tasks (VFTs); to investigate age and education effects; to analyze the differences between time intervals; and to investigate whether these differences varied according to age and education. Method: Three hundred adults divided into three age groups (19-39; 40-59; 60-75) and two groups of educational level (2 to 7 years; 8 years or more) performed unconstrained, semantic, and phonemic VFTs. We compared the performance of the groups using two-way ANOVA with post-hoc Bonferroni test. The depression scale score was covariate. The time interval of verbal fl uency was the variable used for subjects' comparison (repeated measures ANOVA). Results and conclusions: Our results suggest that there are age and education effects on phonemic and unconstrained VFTs. We also found an interaction between those variables in the semantic VFT (time intervals and total time) and in the differences between semantic and phonemic tasks. The repeated measures analysis revealed age effects on semantic VFTs and education effects on the phonemic and semantic VFTs. Such fi ndings are relevant for clinical neuropsychology, contributing to avoid false-positive or false-negative interpretation. ResumoObjetivo: Esse estudo teve como objetivo apresentar normas de desempenho e discrepância em três tarefas de fl uência verbal de um minuto e investigar os efeitos de idade e escolaridade. Além disso, investigamos as diferenças entre os intervalos de tempo e se essas diferenças variaram em função da idade e escolaridade. Método: Participaram 300 adultos divididos em três grupos de idade (19-39; 40-59; 60-75) e anos de escolaridade (2 a 7; 8 ou mais), avaliados com tarefas de fl uência verbal livre, fonêmica ou semântica. O desempenho entre grupos foi comparado por uma two-way ANOVA com post-hoc Bonferroni, com o escore da escala de depressão como co-variável. A variável de comparação intragrupos foi o tempo da fl uência verbal (ANOVA medidas repetidas). Resultados e conclusões: Os resultados indicaram efeitos principais de idade e escolaridade nas tarefas ortográfi ca e livre, e uma interação entre essas variáveis na tarefa semântica (intervalos e total), e na discrepância entre as tarefas semântica e ortográfi ca. A análise de medidas repetidas demonstrou efeitos principais de idade na tarefa semântica e de educação nas tarefas semântica e ortográfi ca. Tais achados são relevantes para futuras interpretações de dados clínicos, evitando assim falsos positivos ou negativos. This paper aims to present the process of construction and content validity of the Brazilian Brief Neuropsychological Assessment Battery Neupsilin, developed to briefly examine the neuropsychological profile of both clinical and healthy populations. It evaluates time and spatial orientation, attention, perception, memory, arithmetic abilities, language, praxia and executive functions (problem solving and verbal fluency). The process of construction was based on psychometric procedures: 1) construction of a preliminary version of the instrument, founded on clinical and research experience of the three specialists, as well as on research on the literature and on already available instruments; 2) blind judges' analyses: a) an analysis of the importance of the inclusion of each subtest considering the assessed construct and the instrument's goal, b) an analysis of the adequacy of each item regarding the construct; 3) reformulation of the instrument; 4) semantic analysis of the items and brainstorming; 5) final analysis done by three specialist judges; 6) pilot study and final version, and 7) content validity. Modifications were done in each stage of the development of the Neupsilin. It demonstrated adequate content validity, with advantages and limitations.
