Delayed sternal closure causes a significant transitory decrease in stroke volume, cardiac output and arterial blood pressure. Also lung compliance and blood oxygenation are temporarily significantly compromised.

Odložený uzávěr hrudníku u novorozenců po operaci vrozené srdeční vady. Cor Vasa 2009;51(5): 327-333.Po komplexní úpravě vrozené srdeční vady kardiochirurgickou operací nelze u všech novorozenců primárně uzavřít sternotomickou ránu pro otok orgánů v mediastinu, dilataci srdce či pooperační krvácení. Jako prevenci nízkého srdečního výdeje indikujeme u těchto pacientů již na operačním sále metodu odloženého uzávěru hrudníku. Tuto metodu používáme také v bezprostředním pooperačním období u novorozenců s klinickými známkami nízkého srdečního výdeje nebo po úspěšné kardiopulmonální resuscitaci. Cíl: Retrospektivní zhodnocení metody odloženého uzávěru hrudníku u novorozenců operovaných pro vrozenou srdeční vadu s ohledem na indikaci a četnost použití, techniku provedení a výskyt komplikací. Soubor a metodika: V období od ledna 1992 do června 2008 jsme v Dětském kardiocentru Fakultní nemocnice v Motole operovali 577 novorozenců s použitím mimotělního oběhu. Pět novorozenců zemřelo na operačním sále, 572 přeživších bylo zařazeno do této studie. Metodu odloženého uzávěru hrudníku jsme použili u 239 (41,8 %) pacientů ve věku 0 až 28 dní (medián 7 dní) při váze od 1 450 g do 4 650 g (medián 3 250 g). Výsledky: Bezprostředně po operaci nebyl hrudník uzavřen u 223/239 pacientů pro otok srdce a měkkých tkání (kardiomediastinální disproporce) v 58 případech, pro krvácení z jiných než chirurgických příčin u 53 pacientů, jako prevence nízkého srdečního výdeje při nestabilní cirkulaci u 49 pacientů, pro dilataci srdce u 28 pacientů a pro plicní edém nebo problematickou ventilaci u pěti pacientů. Metodu odloženého uzávěru hrudníku jsme také použili u 16/239 pacientů, kteří měli po operaci hrudník primárně uzavřený, ale u nichž bylo třeba hrudník urgentně otevřít pro srdeční tamponádu v osmi případech, pro nízký srdeční výdej u šesti pacientů a při resuscitaci u dvou nemocných. Odložený uzávěr hrudníku jsme použili u všech pacientů s hypoplastickým levým srdcem jako součást operačního protokolu. Z ostatních vrozených srdečních vad jsme metodu odloženého uzávěru hrudníku indikovali nejčastěji u interrupce aortálního oblouku v 71,8 % případů a u dvojvýtokové pravé komory u 71,4 % operovaných novorozenců. U funkčně společné komory byla metoda odloženého uzávěru hrudníku použita u 68,7 % novorozenců a u společného arteriálního trunku s interrupcí aortálního oblouku v 66,6 % případů. U společného arteriálního trunku a kritické aortální stenózy byla metoda odloženého uzávěru hrudníku indikována u poloviny operovaných. U ostatních novorozenců s vrozenou srdeční vadou dominoval primární uzávěr hrudníku. Novorozenci s totálním anomálním návratem plicních žil a se společným arteriálním trunkem a odloženým uzávěrem hrudníku byli při operaci signifi kantně mladší než ti, u kterých byl hrudník primárně uzavřen.
