Woche 2018 Diese Woche 24/2018 Drei Fälle von Gonorrhö mit ausgeprägter Antibiotika-Resistenz und Therapieversagen in Australien und Großbritannien Die Weltgesundheitsorganisation (WHO) und das Europäische Zentrum für die Prävention und die Kontrolle von Krankheiten (ECDC) berichten aktuell von drei Fällen einer Infektion mit extensiv-resistenten Stämmen von N. gonorrhoeae (XDR-NG). 1 Der erste Fall wurde im Februar 2018 in Großbritannien bei einem Mann mit Urethritis diagnostiziert. Zwei weitere Fälle wurden im März 2018 aus Australien berichtet. Die Isolate, waren multiresistent und wiesen kombiniert zum einen Resistenz gegen Ceftriaxon (MHK * = 0,5 mg/L) und zum anderen eine high-level-Resistenz gegen Azithromycin (MHK > 256 mg/L) auf. Der Fall aus Großbritannien sowie ein Fall aus Australien waren reiseassoziiert, und wurden beide in verschiedenen Ländern in Südost-Asien erworben. Alle drei Fälle waren phänotypisch nahezu identisch. Ceftriaxon und Azithromycin sind in Deutschland, in Europa sowie nach WHO, entsprechend der Therapieleitlinien für Infektionen mit N. gonorrhoeae, als duale Kombinationstherapie Medikamente der ersten Wahl. 2 Der Patient aus Großbritannien wurde empirisch mit Ceftriaxon 1 g und einer 7-tägigen Doxycyclin-Therapie behandelt. Da das multiresistente Isolat ausschließlich sensibel für Spectinomycin war (einem in Deutschland nicht für den humanmedizinischen Gebrauch verfügbaren Antibiotikum), wurde nach Vorliegen der Empfindlichkeitstestung zusätzlich damit therapiert. Eine Kontrolluntersuchung mittels einem Nukleinsäure-Amplifikationstest (NAAT) aus einer Urinprobe ergab nach der Therapie einen negativen Befund. Ein Rachenabstrich zeigte sich jedoch bei der Kontrolluntersuchung NAAT-positiv. Eine Reinfektion wurde ausgeschlossen. Der Patient wurde erst mit einer mehrtägigen Ertapenem-Behandlung erfolgreich therapiert.
