O carcinoma urotelial (UC) é a histologia mais comum do câncer de bexiga. UC são células pluripotentes que podem diferenciar em muitas variantes histológicas. Aproximadamente, 80% dos UC são puros com histologia convencional urotelial, enquanto os 20% restantes são representados por arquiteturas histológicas diferentes. Essas variantes têm se tornado um tópico com aumento de interesse, haja vista que algumas delas possuem piores prognósticos, podendo mudar o curso da doença, requerendo manejos terapêuticos diferenciados. Nesta publicação, relatamos um caso da variante plasmocitoide do carcinoma urotelial que se apresentou como doença altamente agressiva.
