Se ha establecido un nuevo nivel objetivo de lipoproteína de baja densidad (C-LDL) en pacientes con muy alto riesgo cardiovascular. Sin embargo, no se ha evaluado si el tratamiento con evolocumab en combinación con atorvastatina permite alcanzar estos niveles. Objetivo: Evaluar la eficacia de evolocumab para lograr los niveles objetivo de C-LDL en pacientes con cardiopatía isquémica y muy alto riesgo cardiovascular. Material y métodos: Veinte pacientes con cardiopatía isquémica y muy alto riesgo cardiovascular fueron tratados con evolocumab más atorvastatina durante 24 semanas. Se determinaron los niveles de C-LDL, C-HDL, colesterol total y triglicéridos en suero antes y después del tratamiento. Resultados: Después de las 24 semanas de tratamiento, se obtuvo un promedio del porcentaje de reducción de C-LDL de 55% y 11 de los 20 pacientes alcanzaron los niveles objetivos de C-LDL. No se encontraron diferencias en los niveles de (lipoproteínas de alta densidad) HDL ni de triglicéridos. Conclusiones: El tratamiento con evolocumab fue seguro y eficaz, ya que redujo la concentración de C-LDL en todos los pacientes; sin embargo, sólo se alcanzó el nivel objetivo de C-LDL en la mitad de los pacientes.
TRC) ha sido un tratamiento efectivo en pacientes seleccionados con IC. Sin embargo, aun en los pacientes con recomendación clase I, la tasa de no respondedores es de hasta 30%, por lo que es necesario identificar predictores de no respuesta. Objetivos: encontrar predictores de no respuesta a la terapia de resincronización cardiaca. Material y métodos: se revisaron las características electrocardiográficas y ecocardiográficas en pacientes que recibieron TRC en el Centro Médico ISSEMyM Toluca entre el 1 o de junio de 2003 y el 1 o de junio de 2019. Se buscó la asociación entre estos factores y la respuesta o no respuesta a un año del implante. Se analizó una muestra de 24 pacientes con recomendación clase I para TRC. Se utilizó análisis de regresión logística multivariable para identificar predictores de no respuesta. Resultados: el 62.5% de los pacientes que tenían un Diámetro Telediastólico del Ventrículo Izquierdo (DTVI) ≥ 77 mm fueron no respondedores (p = 0.003); 75% de los pacientes con patrón fragmentado del QRS en el electrocardiograma fueron no respondedores, aunque este último resultado tuvo una p estadísticamente no significativa (0.083). Conclusiones: el DTVI ≥ a 77 mm es un fuerte predictor de no respuesta a la TRC. En cuanto al patrón fragmentado del QRS, a pesar de que 75% no respondió, podría ser necesaria una muestra de pacientes más grande para encontrar significancia estadística.
La endocarditis infecciosa en válvulas nativas o en válvulas reemplazadas quirúrgicamente ha sido ampliamente descrita en la literatura; sin embargo, respecto a la endocarditis infecciosa en válvulas bioprotésicas transcatéter, aún se encuentra en un proceso continuo de investigación. Presentamos el caso de una paciente de 66 años con diagnóstico de endocarditis infecciosa temprana de válvula protésica aórtica percutánea SAPIEN 3, quien tuvo una evolución clínica y desenlace favorable con tratamiento médico conservador basado en antibióticos, a pesar de que la incidencia de mortalidad intrahospitalaria a causa de EI-TAVR, representa más de 40%. Reportar nuevos casos clínicos con sus diferentes características, sus tratamientos y resultados obtenidos, se considera de suma importancia para apoyar a un mejor diagnóstico y tratamiento médico en futuros pacientes.
La COVID-19 ha sido responsable de una alta morbimortalidad, la cual se caracteriza principalmente por síntomas respiratorios. Su ingreso a la célula está favorecido por la enzima convertidora de angiotensina tipo 2, presente en diversas células del organismo con efectos sistémicos. Los principales síntomas en el sistema cardiovascular son arritmias, insuficiencia cardiaca, choque cardiogénico, síndrome coronario agudo, bradicardia y taquicardia sinusal. Esta enfermedad tiene un pronóstico dependiente de factores genéticos y demográficos, con una evolución a la recuperación, la muerte o secuelas; a esto último diversos autores lo han descrito como «síndrome post-COVID-19». El compromiso cardiovascular se ve reflejado por la alta incidencia de taquicardia, detectada en el seguimiento de pacientes recuperados de la COVID-19 que refieren palpitaciones, y al explorar los signos vitales se comprueba el aumento en la frecuencia cardiaca. Sin embargo, su patogenia es desconocida, motivo por el que se realizó una revisión sistemática con el propósito de proponer hipótesis sobre los mecanismos fisiopatológicos para explicarla, algoritmos para la detección temprana y un tratamiento estratificado, para así contribuir en futuras investigaciones sobre esta asociación de taquicardia como daño cardiovascular posterior a la COVID-19, debido a la escasa evidencia científica sobre el tema.
