Vertebral artery hypoplasia (VAH) belongs to the relatively frequent Doppler ultrasonography (US) findings. However, its clinical significance remains controversial. This was a retrospective study analyzing clinical data of patients undergoing US because of cerebrovascular disease in a single academic neurology center. In the dataset of 2500 US examinations, 80 individuals with VAH (VA diameter < 2.0 mm) were identified (3.2% of all patients). Patients with significant vertebral artery asymmetry (SVAA, difference in VA diameters > 1.0 mm) (n = 80) and patients with normal VA diameter (n = 80) were also recruited. Clinical parameters including clinical signs and symptoms, concomitant diseases, imaging findings and the hospitalization outcome were compared between groups. The frequency of vertigo was highest in VAH group. Ischemic lesions of the cerebellum were found in 10% of VAH patients, 16% of SVAA patients and 5% of control subjects. Neurological deficits improved in over 60% of patients in each group, whereas ca. 30% of patients remained in a stable neurological status. The percentage of patients who deteriorated did not exceed 5% in any of the groups. The results of our study support a relatively high frequency of VAH. Our observations suggest coexistence of VAH with a higher frequency of neurological presentations associated with posterior arterial circulation of the central nervous system.
Jednym z częściej zlecanych badań dodatkowych w neurologii jest ultrasonograficzna ocena tętnic domózgowych z funkcją Doppler. Głównym celem tego badania jest ocena morfologii i przepływów w zewnątrzczaszkowych odcinkach tętnic szyjnych wewnętrznych oraz w tętnicach kręgowych i szyjnych wspólnych. W artykule w zwięzły sposób przedstawiono przegląd piśmiennictwa dotyczącego znaczenia w codziennej praktyce klinicznej anomalii naczyniowej, jaką jest hipoplazja tętnicy kręgowej. Pojęcie to jest przedmiotem wielu kontrowersji, rozpoczynających się już od kwestii definicji i spójnych kryteriów rozpoznania hipoplazji tętnicy kręgowej. W literaturze graniczna średnica tętnicy kręgowej określana jest najczęściej jako 2 mm. Jednak niektóre badania wskazują, że w pewnych sytuacjach już średnica naczynia poniżej 3 mm może ograniczać perfuzję w zaopatrywanym obszarze mózgowia. W zależności od przyjętych kryteriów rozpoznania i badanej populacji częstość hipoplazji tętnicy kręgowej oceniana jest na 1,9-26,5%. Wyniki opublikowanych badań sugerują możliwy związek hipoplazji tętnicy kręgowej ze zwiększonym ryzykiem wystąpienia incydentu niedokrwiennego z tylnego kręgu unaczynienia. Na podobną zależność wskazywano również w przypadku wrodzonej asymetrii tętnic kręgowych przekraczającej 1:1,7. Z całą pewnością znaczenie kliniczne hipoplazji tętnicy kręgowej jest dodatkowo uwarunkowane wieloma parametrami, takimi jak wiek i choroby współistniejące. Istotnym aspektem jest również wydolność krążenia obocznego, a zwłaszcza tętnic łączących tylnych, wchodzących w skład koła tętniczego Willisa. Ciekawym zagadnieniem jest ponadto potencjalny związek hipoplazji tętnicy kręgowej z migreną z aurą. Podsumowując, pomimo wielu aktualnych kontrowersji wydaje się, że hipoplazja tętnicy kręgowej nie powinna być automatycznie uznawana za anomalię budowy naczyń bez znaczenia klinicznego dla pacjenta.Słowa kluczowe: tętnica kręgowa, hipoplazja, udar niedokrwienny, zawroty głowy, TIA Doppler assessment of the intracranial arteries is one of the most commonly ordered additional tests in neurology. The main purpose of the test is to assess morphology and blood flow in the extracranial segments of the internal carotid arteries as well as vertebral and common carotid arteries. The paper presents a brief literature review on the role of a vascular anomaly known as vertebral artery hypoplasia, in everyday clinical practice. The abnormality is a subject of much controversy, including the definition and consistent criteria for the diagnosis of hypoplastic vertebral artery. In literature, the diameter threshold for vertebral artery is most often defined as 2 mm. However, as pointed out in some studies, there are certain situations when vascular diameter below 3 mm may reduce perfusion in the supplied brain region.
