IntroduçãoDesde a Idade Média apareceram as primeiras descrições da intoxicação por derivados da ergotamina. Eram descritas, naquela época, estranhas epidemias cujo sintoma característico era a gangrena das extremidades 1,2 .O ergot é produto de um fungo, Claviceps purpurea, que cresce no centeio e em outros cereais. Os componentes ativos da droga são alcaloides, todos derivados do ácido lisérgico 3,4 . Seus efeitos colaterais são basicamente de três categorias: neurológicos -cefaleia, vertigem, psicose, convulsão e agitação; gastrointestinais -diarreia, náusea, vômito e dor abdominal em cólica; e vasculares 2,4-7 .Apesar das múltiplas apresentações do ergotismo, a isquemia periférica secundária ao vasoespasmo é a forma mais comum e ocorre em 0,01-0,02% daqueles doentes em tratamento com alcaloides ergóticos 2,5 .Os derivados da ergotamina compõem diversas drogas amplamente utilizadas no tratamento de ataques agudos de migrânea e de hemorragias pós-parto 1,3,8 . A intoxicação por estas substâncias pode ser decorrente de overdose aguda, do uso de doses terapêuticas em pacientes que apresentam reação de hipersensibilidade e, de forma crôni-ca, em pacientes que fazem uso prolongado de doses terapêuticas 1-3,9,10 . Entretanto, nem sempre o grau de isquemia se correlaciona com a concentração sanguínea da ergotamina 8 .Cerca de 90% da metabolização dos compostos ergóticos é hepática, e existem múltiplos fármacos de metabolismo hepático que podem aumentar o seu potencial vasoconstritor, como macrolídeos, propranolol, sumatriptano, ampicilina, contraceptivos orais e nicotina 2,7,8 .O rápido desaparecimento do fármaco da circulação sanguínea e os efeitos prolongados sobre a árvore arterial sugerem que tanto a lenta separação do fármaco ao seu receptor como a presença de metabólitos ativos na circulação sanguínea prolongam o efeito vasoconstritor 8 .Os principais efeitos farmacológicos da ergotamina são: a estimulação dos receptores a-adrenérgicos, dopami- RELATO DE CASO ResumoOs derivados da ergotamina compõem diversas drogas amplamente utilizadas no tratamento de ataques agudos de migrânea. A intoxicação por estas substâncias resulta geralmente de sua administração crônica, promovendo sintomas secundários ao espasmo arterial e à consequente isquemia distal. Neste artigo, é relatado o caso de uma paciente de 47 anos com diagnóstico de oclusão arterial aguda em membros inferiores secundária ao uso de derivados da ergotamina. Após a suspensão da droga e a prescrição de anticoagulantes, vasodilatadores e antiagregante plaquetário, a paciente evoluiu com melhora da dor, da parestesia e com o retorno da coloração normal e dos pulsos distais em membros inferiores.Palavras-chave: Ergotismo, ergotamina, isquemia, insuficiência arterial, oclusão arterial.
