Introducción: El abordaje multidisciplinario (MDT) en centros de alto volumen es el estándar de oro para el tratamiento de pacientes con cáncer de recto (CR) . No hay datos poblacionales en Latinoamérica que reflejen el tratamiento de estos pacientes en la región.Objetivo: Realizar una encuesta incluyendo países de Latinoamérica, cubriendo distintos puntos críticos en el manejo de pacientes con cáncer de recto.Materiales y métodos: Durante Mayo/2022, cirujanos de países dentro de la región fueron invitados a completar una encuesta cubriendo 3 puntos sobre manejo de pacientes con CR: Volumen anual de pacientes atendidos y complejidad del centro, técnica quirúrgica, y disponibilidad de MDT y tipos de tratamiento oncológico utilizados. Los cirujanos participantes fueron categorizados según el número de pacientes con cáncer de recto atendidos en su institución (bajo -CBV-, mediano -CMV- y alto volumen -CAV-).Resultados: 385 respuestas a la encuesta fueron incluidas en el análisis incluyendo centros de 18 países de Latinoamérica: 100/210/75 de centros de bajo, medio y alto volumen, respectivamente. CMV y CAV tuvieron mayor incidencia de cirujanos especializados a cargo de los pacientes con CR. 67.79% de los centros tienen acceso a técnicas mini-invasivas, siendo esto más frecuente en CMV y CAV. La utilización de neoadyuvancia en tumores localmente avanzados fue de 93.51%, con adherencia a protocolos de Watch & Wait de 60.78%. CMV y CAV presentaron mayores índices de abordaje multidisciplinario de los pacientes. Por último, la adherencia a estrategias de terapia neoadyuvante total y radioterapia de curso corto es baja, con mayor prevalencia en CMV y CAV.Conclusiones: Esta encuesta es la primera en aportar información sobre el tratamiento de CR en distintos centros y países de Latinoamérica. Futuros estudios deberán analizar el impacto de las diferencias entre centros sobre el resultado del tratamiento.
Introducción: Es fundamental que cada equipo quirúrgico audite la calidad de sus cirugías, mas en un centro docente, el cual debe garantizar el correcto aprendizaje de la cirugía oncológica. Objetivo: evaluar la calidad de la cirugía oncológica por cáncer colorrectal en un centro universitario. Material y métodos: estudio retrospectivo sobre una base de datos prospectiva de pacientes operados por cáncer colorrectal en el hospital Pasteur, entre 2017-2021. Se analizó la seguridad de la cirugía, la calidad oncológica de la resección y el nivel formativo del cirujano principal. Resultados: de los 89 pacientes, no hubo diferencias de género. La mediana de edad fue 69 años. Predominaron los pT3 (50%) y pT4 (36,5%), pN0 (60%) y pN1 (29%). Los procedimientos mas realizados fueron la colectomía derecha (43%) y la resección anterior de recto (40%). El 69% fueron laparoscópicos, con una conversión del 5%. La dehiscencia anastomótica fue 9,8%, mas frecuente en las anastomosis ileotransverso (13%) y las manuales (16%). Hubo 10% de complicaciones quirúrgicas moderadas-severas (grados 3 y 4 de Clavien-Dindo). La tasa de reintervención fue 11% y la mortalidad 2%. La mediana de internación fue 7 días. El número de ganglios resecados tuvo una mediana de 16. No hubo compromiso del margen distal y el circunferencial estuvo libre en 85%. La fascia rectal fue completa/casi completa en 93%. Casi 70% de las cirugías fue realizada por cirujanos jóvenes. Conclusión: en nuestro centro, la cirugía por cáncer colorrectal cumple con estándares internacionales de seguridad y calidad oncológica. La mayoría de los procedimientos fueron realizados por los jóvenes, en forma tutorizada.
Introducción: La hemorragia digestiva baja es relativamente frecuente y su incidencia está en aumento. Hasta un 15% de los pacientes requieren terapias invasivas como la endoscopía, la cirugía o la angioembolización. Siendo esta última un procedimiento seguro y efectivo como para controlar la hemorragia digestiva. Nuestro objetivo es presentar un caso de un paciente con hemorragia digestiva baja resuelto mediante angioembolización. Caso clínico: Paciente de 17 años que se presenta con enterorragia y síndrome funcional anémico que persiste con el sangrado a pesar del tratamiento médico. Se solicita una arteriografía que identifica a nivel de una rama que se dirige hacia el ángulo izquierdo del colon, dependiente de la arteria mesentérica superior, extravasación de medio de contraste, por lo que se decide realizar su embolización. Con buena evolución post procedimiento se otorga el alta. Discusión: La hemorragia digestiva baja sigue siendo un desafío diagnóstico y terapéutico. De presentarse un paciente con persistencia del sangrado la angiotomografía debería ser la primera opción diagnóstica y la angioembolización la terapéutica. Conclusiones: Se presentó el caso de un paciente joven, con una hemorragia digestiva baja de entidad, que correspondía a un vaso que irriga el ángulo izquierdo del colon. El mismo fue tratado mediante angioembolización.
