Un dorso recto es uno de los principales objetivos para los pacientes que se realizan una rinoseptoplastía. Sin embargo, una resección dorsal excesiva puede traer consecuencias estéticas y funcionales.
Introducción: El papiloma invertido (PI) es definido por la OMS como un tumor epitelial benigno, de crecimiento hacia el estroma subyacente, sin invasión de la membrana nasal. Tiene una alta recurrencia y se ha asociado a carcinoma de células escamosas. La resección quirúrgica completa es el tratamiento de elección. El estudio imagenológico nos permite delimitar la extensión de la lesión antes de la cirugía, distinguiéndola del tejido inflamatorio circundante.Pacientes y métodos: Se analizaron exámenes de resonancia magnética (RM), correspondientes a los Departamentos de Otorrinolaringología y Radiología de la Universidad de Brescia (Italia), durante un periodo de 4 años, de 23 pacientes con PI, para identificar posibles factores de utilidad en el diagnóstico preoperatorio. Veinte y tres pacientes con diagnóstico de tumores malignos sinusales (TMS) fueron utilizados como grupo control. La revisión de exámenes (RM), fue realizada por 2 radiólogos experimentados. La edad promedio del grupo 1 (PI) fue de 54,4 años, de predominio masculino. La mayoría correspondía a lesiones primarias y un tercio de los casos a lesiones recurrentes. En el grupo 2 (TMS), el promedio de edad fue de 62,6 años, también en su mayoría hombres. El 52% fueron adenocarcinomas, 9 casos correspondían a carcinoma de células escamosas y 2 a carcinoma neuroendocrino. Todos los exámenes fueron realizados en secuencia T2 y T1 con y sin contraste (gadolinio) en planos axial, coronal y sagital. La presencia de un patrón columnar o estriado (bandas delgadas hipo e hiperintensas contiguas), fue evaluada en ambos grupos. Los cambios óseos fueron definidos como remodelación o erosión. Un valor p ≤0,05 fue considerado como estadísticamente significativo en todas las comparaciones.Resultados: Después de la administración de contraste, la mayoría de las lesiones del grupo 1 (21 casos) mostraron un marcado aumento de intensidad, aunque heterogéneo. Por otro lado, el 91,3% de las lesiones del grupo 2, fueron hipointensos. El patrón columnar fue más prevalente en el grupo de PI, tanto en T2 (69,5%) como en las secuencias T1 contrastadas (100%) (p <0,0001). No obstante, en T2 dicho patrón no fue detectado en lesiones ≤2 cm de diámetro mayor. El 91,3% de las lesiones del grupo 2, tuvieron signos de erosión ósea, comparado con el grupo 1 (p =0,00001).Discusión: El PI es el tumor benigno más frecuente del tracto sinusal, siendo unilateral en más del 90% de los pacientes. Es conocido que la RM contribuye a distinguir entre tumor y secreciones adyacentes. Según los autores, el primer punto clave para diferenciar un PI de un TMS, está dado por la captación de contraste, la cual fue mayor en el grupo de 1. La detección del patrón columnar fue un valioso indicador sugerente de PI. El epitelio hiperplásico (hipointenso en T2 y T1 por su alta celularidad) crece hacia el estroma
