The case of a 28-year-old male patient with a locally aggressive lesion of the distal tibia is presented. Following the diagnosis of giant cell tumor of bone (GCT) on biopsy and curettage, a rapid malignant course was observed with recurrence 2.5 months later. Multiple metastases appeared 6 months after initial presentation. Following initial chemotherapy according to the COSS protocol and later with carboplatin and VP-16, therapy was changed to Adriamycin and later gemcitabine due to progressive disease. Good palliation was achieved, and the patient felt well with less shortness of breath on exertion and was ambulatory with walking aids. The malignant nature of the tumor was not detected in the initial pathologic examinations. Review of the pathologic material provided histologic clues permitting the diagnosis of a primary malignant GCT with a fibrohistiocytic/fibrosarcomatous component. Malignancy in a giant cell tumor is a much debated diagnostic dilemma when a frank sarcomatous component is lacking. Cytologic atypias and flame-like tufts of infiltration of soft tissue are important clues. Surgical treatment should be commensurate. Monotherapy with Adriamycin or gemcitabine can be considered in order to inhibit the disease progression.
resumo O propósito deste artigo é discutir a concepção de tempo na História do cerco de Lisboa (1989), de José Saramago. Para tanto, a partir da leitura do romance, recorre-se a considerações filosóficas - de Agostinho, Kant, Nietzsche, Bergson, Cioran e Koselleck - sobre tempo e história. Pretende-se, assim, evidenciar e analisar os estratos temporais dessa História saramaguiana com base nas relações entre ficção e filosofia.
Este artigo analisa as forças centrífugas que organizam a epopeia de Camões, asseguram a vitória sobre os obstáculos representados ou referidos pelo Velho do Restelo, Gigante Adamastor e Baco, e coroam o desfecho da narração do poema, ocorrido na “ilha namorada”, quando Vasco da Gama e seus companheiros se unem às ninfas e consumam a simbiose entre homens e “aquáticas donzelas”, filhas do Oceano. Propõe-se uma leitura retórico-poética d’Os Lusíadas (1572), avessa aos critérios românticos, psicológicos, burgueses e ilustrados que naturalizam tabus e paradigmas incompatíveis com as práticas letradas portuguesas do século XVI.
Há 450 anos, quando a epopeia Os Lusíadas foi publicada, as práticas letradas eram atos enunciativos formulados com base em artifícios e convenções datadas. As letras, amparadas em artefatos culturais, reúnem esquemas de argumentação tributários de outros discursos, conjunturas, pessoas e não prescindem de protocolos, normativas, regras e prescrições que condicionam a licença poética e as possibilidades históricas do pensar, dizer e fazer. Sendo assim, os enunciados não se autonomizam em relação aos agentes que os proferem, pois inventam o próprio agente como figuração discursiva.Ora, as representações, independentemente dos gêneros ou subgêneros aos quais pertencem, são leituras de mundo e, por isso mesmo, frutos de silêncios, escolhas, esquecimentos. Sendo assim, são iniciativas restritas, precárias, provisórias, respaldadas em técnicas que configuram sistemas de legibilidade particulares. A despeito dessas condicionantes, as representações também podem consumar sentenças edificantes, solidárias, inclusivas, instrutiva, aspecto que demarca sua ambiguidade e multiplica os seus efeitos sobre o leitor/ouvinte, sempre sujeito aos limites temporais da interpretação e aos autoritaris-
Este trabalho propõe-se a discutir dois textos: a relação intitulada Nuevo descubrimiento del gran Río de las Amazonas (1641), de Cristóbal de Acuña, e a epopeia Viagem (1746), de Pedro de Santo Eliseu. As discussões propostas baseiam-se em preceptivas retóricas e poéticas e em concepções teológico-políticas pertinentes aos Impérios espanhol e português dos séculos XVI a XVIII. Assim, pretende-se evidenciar que, naquela relação do padre jesuíta espanhol e nesta epopeia do frade carmelita português, as representações do Rio das Amazonas são feitas de história, poesia e política, visando à concórdia entre os membros do corpo místico do Reino.
