RESUMO -Relatamos caso incomum de um paciente de 37 anos com hematoma extradural traumático do vértex com sintomas de hipertensão intracraniana. O diagnóstico foi feito através da tomografia do crânio em cortes coronais e o paciente foi submetido a craniotomia com drenagem do hematoma. Seu relato na literatura é escasso e, quando apresentadas, as séries são limitadas a poucos casos. Constituem um grupo peculiar dos hematomas intracranianos traumáticos 3 . A incidência de HEDV relatada em alguns estudos varia de 0 a 8%, com mortalidade de 18 a 50% 1,4-8 . Em 92 a 100% dos casos sua origem está relacionada a fraturas da região do vértex ou diástase da sutura sagital 4,5 . Geralmente, seu diagnóstico é tardio e necessita de semiologia peculiar para ser suspeitado, uma vez que os sintomas podem ser vagos e sua localização constituir-se num potencial "ponto cego" para o diagnóstico tomográfico 9 .Neste artigo, relatamos o caso de um paciente que apresentou um hematoma extradural do vértex após bater a cabeça contra uma porta. CASOHomem de 37 anos de idade, comerciante, destro, deu entrada no Pronto Socorro do Hospital Heliópolis em julho de 2002 com história de ter batido a cabeça contra uma porta, apresentado perda de consciência por tempo indeterminado, seguida de sonolência e confusão mental. Era tabagista. Não era etilista crônico; não utilizava medicação nem teve crise convulsiva, delirium tremens ou apresentava outro antecedente.À admissão apresentava-se sonolento, confuso, com náuseas e vômitos, tendo obtido 13 pontos (abertura ocular, 3 pontos; melhor resposta verbal, 4 pontos; melhor resposta motora, 6 pontos) na escala de coma de Glasgow (ECG). As pupilas estavam isocóricas e o reflexo à luz, normal; não apresentava déficit motor. À inspeção geral não havia escoriações ou outras lesões. O exame físico geral não apresentou anormalidades.A tomografia computadorizada (TC) axial do crânio demonstrou área hiperatenuante na região fronto-parietal da convexidade direita que ultrapassava a linha média nos cortes mais pró-ximos do vértex, aí também incluindo a região frontoparietal (Fig 1) esquerda. Com estas imagens foi solicitada uma TC de crânio com cortes coronais. Este exame revelou extenso hematoma extradural fronto-parietal bilateral, sendo maior à direita (Fig 2). Observou-se ainda que havia desvio da linha média para a esquerda de 5 mm associado à imagem de herniação subfalcina mais compressão dos ventrículos laterais e do terceiro ventrículo.
