RESUMENEsta revisión bibliográfica analiza los diferentes tipos de láser usados en otorrinolaringología, destacando las propiedades de cada uno. Posteriormente describe las características de una sesión con anestesia y uso de láser y las patologías con indicaciones de cirugía láser. Estas se clasifican en cinco rubros. Patología laringotraqueal, patología faringobucal, patología sinusal, otología y dermatología cérvico facial. En la primera se analizan: lesiones fonoquirúrgico, papilomatosis laríngea, parálisis laríngeas, laringomalacia, angioma subglótico, estenosis y displasia laríngea, cáncer laríngeo. En patología faríngea y bucal se analiza su uso en atresia coanal, amígdalas palatinas linguales, roncopatías, divertículo de Zenker. En fosas nasales se describen los usos en cirugía endoscópica (FEELS). En patología otológica se analiza su uso en CAE y oído medio, para concluir con las lesiones cervicofaciales.Palabras clave: Láser, patologías ORL. SUMMARYThis bibliographic review analyzes the different types of laser used in otorhinolaryngology, outlining the properties of each one. Later on it describes the characteristics of a session with anesthesia and use of laser and the and the pathologies with indications of laser surgery. These are classified in five items. Laryngotracheal pathology, pharyngobuccal pathology, sinusal pathology, cervicofacial otology and dermatology. In the first one, analysis is made of phonosurgical injuries, laryngeal papilomatosis, laryngeal paralysis, laryngomalacia, subglotic angioma, stenosis and laryngeal dysplasia, laryngeal cancer. In pharyngeal and buccal pathology, its use is analized in choanal atresia, lingual palatine tonsils, hoarseness, Zenker's diverticulum. In nasal fossae the uses on endoscopic surgery are described (FEELS). In otologic pathology its use is analyzed on CAE and middle ear, to end with cervicofacial injuries.
RESUMENLos tumores de punta de peñasco se pueden presentar con hipoacusia sensorioneural y un síndrome vertiginoso secundario a hidrops endolinfático. Dentro del diagnóstico diferencial se encuentran los tumores de saco endolinfático, los cuales han sido reportados cada vez con mayor frecuencia en la literatura.A continuación informamos el caso de un paciente de 50 años que se presenta a la consulta con hipoacusia sensorioneural fluctuante de oído derecho y síndrome vertiginoso. Las neuraimágenes muestran tumor de punta de peñasco que ha tenido un crecimiento progresivo en el tiempo erosionando secundariamente estructuras del oído interno. Se realizó biopsia que no fue concluyente, la cual no ha podido ser repetida por decisión personal de la paciente, por lo que se ha mantenido en manejo expectante.Dada la evolución y las imágenes se plantea diagnóstico de tumor de saco endolinfático, a la espera de una biopsia confirmatoria.Palabras claves: Tumor saco endolinfático, tumores de punta de peñasco, tumores de oído, hídrops endolinfático. SUMMARYThe petrous point tumors may appear with sensoneuronal hypoacousia and a vertiginous syndrome secondary to endolymphatic hidrops. Within the differential diagnosis there are the tumors of the endolymphatic sack, which have been reported more and more frequently in the literature.Below we report the case of a 50 year old patient who arrives to consult with fluctuating sensoneuronal hypoacousia of the right ear and vertiginous syndrome. The neuroimages show a petrous point tumor that has had a progressive growth in time, secondarily eroding structures of the inner ear. A biopsy was taken but it was not conclusive, and in has not been repeated due to the personal decision of the patient, whereby it has been maintained under expectant handling.
RESUMENLa timpanoplastía es una cirugía frecuente para el otorrino. El año 1998 el Dr. Eavey presenta la técnica de injerto en mariposa, la que es avalada posteriormente por varios autores y se afirma que es una técnica eficiente, con menor tiempo quirúrgico y mejor confort para el paciente.Se trata de un estudio prospectivo realizado en el Hospital Dr. Sótero del Río entre los meses de septiembre 2005 a febrero 2006. En el que se realizó esta técnica a 5 pacientes obteniendo como resultado anatómico cierre de la perforación en 4 pacientes y una extrusión del injerto en 1 paciente post supuración ótica. En cuanto al resultado auditivo la mejoría promedio de los PTP aéreos fue de 15,64 dB. Se concluye que esta técnica presenta las ventajas descritas en la literatura, y se deben realizar estudios sucesivos para determinar qué pacientes se benefician mejor de esta técnica comparán-dolas con las técnicas tradicionales.Palabras Claves: Timpanoplastía, injerto en mariposa.
La disección espontánea de arteria vertebral es una causa frecuente de accidente vascular encefálico en población joven. A continuación informamos el caso de un paciente masculino de 42 años, quien se presenta en la consulta otorrinolaringológica con un cuadro de instalación aguda de cefalea asociada a déficit de pares craneanos e inestabilidad de la marcha, el cual fue interpretado en un servicio de urgencia como un síndrome vertiginoso periférico, luego de una tomografía computada de cerebro sin contraste normal. Se procede a solicitar angiorresonancia, diagnosticándose disección espontánea de arteria vertebral derecha, motivo por el cual es hospitalizado y manejado con anticoagulación sistémica con buena evolución.
