Resumen
Introducción:El cáncer de mama metastásico se considera una enfermedad incurable. Menos del 7% de las pacientes se encuentran en estadio IV o M1 al diagnóstico. Alrededor del 30-40% desarrollarán metástasis a lo largo de la enfermedad. Aunque el tratamiento sistémico es el principal en estas pacientes, en algunos casos la cirugía puede tener intención curativa.
Caso clínico:Mujer de 24 años diagnosticada de carcinoma infiltrante de mama de tipo no especial metastásico, con inmunohistoquímica compatible con subtipo Luminal B. Tras 9 meses de tratamiento sistémico se observa respuesta completa de focos metastásicos, así como una respuesta del primario grado 3 de Miller-Payne. Se propone tumorectomía del tumor primario, con resultado de ypT1c. La paciente continúa asintomática y libre de enfermedad tras 2 años de seguimiento.
Discusión:Actualmente la cirugía del tumor primario en el cáncer de mama metastásico puede modificar el pronóstico, beneficiándose un grupo de pacientes seleccionadas. Debe ofrecerse como complemento a la terapia sistémica siempre y cuando garanticemos márgenes quirúrgicos libres.
