Alkaptonuria, with its sequel, ochronosis, is a rare disease, with an incidence of 1:125,000 to 1:1 million worldwide. Reported cases of ochronotic arthropathy and other orthopedic manifestations are mostly limited to a single family tree, and few cases have been reported. This study highlights 9 previously unreported patients with sporadic presentation and varied orthopedic manifestations of alkaptonuria. Patient age ranged from 34 to 50 years. One patient who had severe arthropathy of the right hip joint along with subcutaneous nodules over both knees and Achilles tendons underwent total hip replacement. Another patient had intramedullary calcification of the femur. An additional patient had associated caries of the spine at L3, L4, and L5, with resolution of symptoms after antitubercular chemotherapy. Another patient had associated features of hyperthyroidism, which was an incidental finding. A further patient had nonunion fracture of the neck of the femur and underwent total hip replacement. The remaining 4 patients had typical features of low backache and arthritis of the large joints. The parents were nonconsanguineous, and only 2 patients had affected siblings. The remaining 7 patients had sporadic nonfamilial presentation. Diagnosis was established by typical clinical and radiologic findings and biochemical analysis. At 2 years of follow-up, both patients who underwent total hip replacement were normal, with no radiologic signs of loosening or lysis. Clinicians need a high index of suspicion and awareness to make the diagnosis of ochronosis. The current study is unique because of presentation with subcutaneous nodules in 1 patient and associated caries of the spine in another patient.
Recently, using a non-linear programming method authors have demonstrated that based on certain criteria if some non-significant selectivity constraints are relaxed, all remaining constraints happen to be feasible but the solution of coordination problem is not possible with the conventional objective function as optimization procedure leaves the solution space [20]. In coordination studies solution with the conventional objective function provides minimum relay operating times. The present paper describes an additional criterion so that the solution with the conventional objective function can be achieved. This paper also achieves the acceptable speed of the primary protection while attempting to coordinate the maximum relay pairs. Complete results are presented in this paper in this regard. This paper uses the non-linear Random Search Technique to solve the coordination problem, which has been successfully applied in many problem areas.
High data transmission capacity is increasingly needed in short-and medium-haul optical communication links. Cost-effective wavelength division multiplexed (WDM) transceiver architectures, achieving high information spectral densities (ISDs) (>1 b/s/Hz) and using low-complexity direct detection receivers are attractive solutions for such links. In this paper, we assess the use of dual-drive Mach-Zehnder modulators (DD-MZMs), and compare them with in-phase quadrature (IQ)modulators for generating spectrally-efficient single sideband Nyquist pulse-shaped 16-QAM subcarrier (N-SCM) modulation format signals. The impact of the extinction ratio (ER) of a modulator on the optical sideband suppression ratio (OSSR) was investigated for the SSB signals in WDM systems, together with the resulting impact on inter-channel crosstalk penalties. First, in back-to-back operation, an IQ-modulator with an ER of 30 dB and a DD-MZM with an ER of 18 dB were experimentally compared in a 6×25 Gb/s WDM system by varying the channel spacing. Following this comparison, 16 GHz-spaced 6×25 Gb/s WDM signal transmission was experimentally demonstrated using the DD-MZM. The experiment was performed using a recirculating loop with uncompensated standard single-mode fiber (SSMF) and EDFA-only amplification. The maximum achievable transmission distances for single channel and WDM signals were found to be 565 and 242 km, respectively, at a net optical ISD of 1.5 b/s/Hz. This is the first experimental comparison of modulator types for SSB N-SCM signal generation and the highest achieved ISD using a DD-MZM in direct-detection WDM transmission.
