StreszczenieWprowadzenie. Związek chorób przyzębia z chorobami ogólnoustrojowymi jest od lat przedmiotem wielu badań. Wiadomo, że najbardziej udokumentowana pozostaje obustronna korelacja zapalenia przyzębia z cukrzycą. Cel pracy. Analiza porównawcza poszczególnych parametrów periodontologicznych u pacjentów ze świeżo rozpoznaną i leczoną cukrzycą w celu pokazania różnic w stopniu nasilenia i zaawansowania zmian w przyzębiu. Materiał i metody. Grupę ze świeżo rozpoznaną cukrzycą typu II stanowiło 108 osób, a grupa pacjentów leczonych z powodu cukrzycy liczyła 84 osoby. Badania przeprowadzono u pacjentów hospitalizowanych w CSK MSWiA oraz wśród pacjentów poradni diabetologicznej tego szpitala. Oceniano podstawowe parametry periodontologiczne -wskaźnik płytki (PI), wskaźnik krwawienia dziąsłowego (BOP), głębokość kieszonek (PD), utratę położenia przyczepu łącznotkankowego (CAL). Na ich podstawie zakwalifikowano również pacjentów do poszczególnych grup zgodnie z klasyfikacją chorób przyzębia wg Offenbachera. Oceniano także poziom glikemii i stężenie glikowanej hemoglobiny (HbA1C). Wyniki. Wartości wskaźnika krwawienia dziąsłowego (BOP) po korekcie o wiek były znamiennie wyższe (p = 0,0007) u osób ze świeżo rozpoznaną cukrzycą w odniesieniu do leczonych diabetologicznie. W grupie pacjentów z cukrzycą świeżo rozpoznaną nie obserwowano zależności między stanem tkanek przyzębia a poziomem glikemii lub HbA1c. W grupie pacjentów leczonych istniała dodatnia korelacja między głębokością aktywnych chorobowo kieszonek a stopniem kontroli cukrzycy wyrażonym parametrem HbA1c. Wnioski. Pacjenci ze świeżo rozpoznaną cukrzycą wykazywali bardziej nasilony stan zapalny przyzębia wyrażony wskaźnikiem krwawienia przy mniejszym zaawansowaniu zmian w przyzębiu oraz braku korelacji parametrów periodontologicznych z poziomem glikemii i HbA1c. Wyniki dokumentują znany pogląd o właściwej kontroli stanu przyzębia u pacjentów z prawidłowo prowadzoną cukrzycą oraz o efekcie kumulacyjnym długo trwającej cukrzycy na stan przyzębia (Dent. Med. AbstractBackground. The relationship between periodontitis and general diseases has been studied for ages. Nowadays the most documented is bilateral correlation of periodontitis with type 2 diabetes. It is well known that the longer diabetes lasts, the more severe periodontal tissues status is presented. Objectives.
