BackgroundCancer antigen (CA) 125 (CA-125) is used in ovarian cancer detection and monitoring, whose serum level has a positive correlation with tumor stage. The aim of this study was to obtain a prediction metastasis equation in a group of patients with ovarian cancer based on Ca-125.MethodsA 2-group comparative observational study was conducted at a single oncologic institution (SOLCA) in Cuenca-Ecuador. All patients who were diagnosed with ovarian cancer between January 1996 and December 2016 were included in the current study. Group 1 (G1) patients with the I and II International Federation of Gynecology and Obstetrics (FIGO) stage and Metastasis Group (MG), with III and IV stage, were subdivided. A logistic regression equation was performed to predict metastasis based on Logarithm of serum Ca-125 levels.ResultsWe included 85 cases in G1 and 64 patients in MG, with 47.8 ± 15 years (G1) and 57.5 ± 13.6 years (MG) of age (P < 0.001). Mortality in G1 was 2 cases (3.1%) and 53 cases (62.4%) in MG (P < 0.001). The CA-125 serum level was 163.5 ± 236 in G1 and 1220.9 ± 1940 u / ml in MG (P < 0.001). The equation to predict metastasis = (Age*0.053) + [(Logarithm Ca-125 value) * 1.078] − 8.163 with an OR 2.940 (CI 95% 2.046–4.223) P < 0.001. The sensitivity of the equation was 82.4% and the specificity was 79.7%.ConclusionsIt is possible to predict the presence of metastasis in a group of patients with ovarian cancer based on Ca-125.Electronic supplementary materialThe online version of this article (10.1186/s12885-018-4499-y) contains supplementary material, which is available to authorized users.
Introducción: La clasificación molecular del cáncer de mama se utiliza en el diagnóstico de lesiones desafiantes y para tener un pronóstico. El objetivo del presente estudio fue determinar la prevalencia de los tipos moleculares de cáncer de mama y sus características clínico-patológicas en pacientes que acudieron a un centro oncológico. Métodos: Este estudio analítico-transversal fue realizado en el Instituto del Cáncer SOLCA-Cuenca, entre Agosto 2010-julio 2017. Se compilaron registros de mujeres con cáncer de mama con inmunohistoquímica molecular. Se midieron datos demográficos, clínicos, tipo Histológico, grado Histológico Combinado de Nottingham, estadio clínico de la NCCN-2017. La muestra fue no probabilística tipo censo. Se usa estadística descriptiva y análisis de asociación entre los tipos moleculares de cáncer de mama y características clínico-patológicas. Resultados: 582 casos ingresaron al estudio. El Luminal A 205/582 (35.22%), Luminal B 204/582 (35.02%), Her2 80/582 (13.75%), Triple negativo 93/582 (15.98) su tipo histológico predominante fue el Ductal (92.96%). El estadio clínico de mayor prevalencia es el IIA (29.21%) y respecto al grado de Nottingham, el 76.80% fue grado III. Asociación estadísticas se reportaron entre edad >40 años y Tipo Luminal A, Edad >40 años y Triple Negativo; Tipo Ductal y Luminal A, Grado II De Nottingham y luminal A y triple negativo, Estadio clínico tardío y luminal A y HER 2 P<0.05. Introducción: La citología de nódulos tiroideos es una técnica que, evita procedimientos quirúrgicos innecesarios por lo que se lo ha determinado como primera línea dentro del algoritmo de diagnóstico, el objetivo del estudio fue determinar la sensibilidad y la especificidad de la citología y biopsia por congelación frente al estudio histopatológico en el diagnóstico de nódulos tiroideos en pacientes atendidos en Solca desde el año 2009 - 2017. Métodos: Es un estudio de tipo observacional, retrospectivo y de correlación diagnóstica; los datos fueron obtenidos de las historias clínicas de pacientes intervenidos quirúrgicamente por nódulos tiroideos con biopsia por congelación, a quienes se les realizó previamente un estudio citológico en el Departamento de patología de SOLCA de la ciudad de Cuenca, Ecuador. El cálculo del tamaño de la muestra fue de 324 casos. Resultados: 324 casos fueron incluidos. El 8.3% correspondió a hombres y el 91.7% a mujeres. La media de la edad fue 51.8 años; la gran mayoría provenían de la provincia Azuay con el 64.8%. En los estudios citológicos el 34.6% [112 casos] corresponden a lesiones inflamatorias benignas; el 11.1% [36 casos] a patologías malignas y 14.2% [46 casos] fueron insatisfactorios. En la biopsia por congelación el mayor porcentaje estuvo concentrado en enfermedades benignas con un 62.6% y 35.5% a lesiones malignas. Hubo 6 casos con el 1.9% en donde fue diferido el criterio diagnóstico. En el histopatológico definitivo el 60.2% [195 casos] fueron patologías benignas y el 39.8% [129 casos] fueron lesiones malignas. La sensibilidad de la PAAF frente a histopatológico es alta con un 91.79%, pero la especificidad es baja con un 51.94%. La sensibilidad y la especifidad de la biopsia por congelación frente a histopatológico es alta con un 98.97% y 90.70% respectivamente lo que le confiere una metodología óptima. Conclusiones: La PAAF de tiroides demuestra ser una metodología útil en el diagnóstico de nódulos, siempre y cuando sea realizada y observada por personal capacitado. La biopsia por congelación constituye una técnica con alta sensibilidad y especifidad que nos permite discriminar lesiones benignas de las malignas.
Introducción: La neoplasia pseudopapilar sólida del páncreas es una patología poco frecuente, que se presenta en mujeres jóvenes y es de comportamiento incierto. La prevalencia es del 1-2% y el tratamiento es quirúrgico. En el presente caso se describe a una paciente de sexo femenino de 10 años de edad que muestra esta patología y su correspondiente manejo. Caso clínico: Paciente de sexo femenino, de 10 años de edad, con antecedentes de una nefrectomía hace 6 años por hidronefrosis, refiere dolor abdominal localizado en egigastrio tipo cólico y pérdida de peso. En los estudio de imagen a nivel peritoneal se observa lesión de aspecto ocupativo de tipo sólido neoplásico con área central de probable necrosis y que desplaza en sentido posterior al páncreas así como al estómago y se asocia con adenomegalias peritoneales y retroperitoneales. Evolución: La paciente fue sometido a una espleno-pancreatectomía distal. El estudio anatomo-patológico concluyó en neuplasia pseudo papilar sólida del páncreas. Continúa en valoración por consulta externa. Conclusión: Neoplasia Pseudopapilar sólida del Páncreas debe ser sospechada en pacientes con una masa sólida o quística pancreática con un estudio de imágenes compatibles.
BACKGROUND: Clear cell adenocarcinomas of the minor salivary glands are rare epithelial tumors that account for less than 1% of all malignant salivary gland tumors. CASE REPORT: We present the case of a 54-year-old male patient who presented with a painless right retroauricular tumor of progressive growth. Ultrasound reported a heterogeneous retroauricular soft tissue tumor with no local or regional lymphadenopathies. Culture and tumor markers were negative, BAFF was reported as suspicious for malignancy. CLINICAL COURSE: Right retroaurilar tumor was excised, without complications. The pathology report informed a right retroauricular clear cell carcinoma with negative limits. The patient has been under follow up care for one year, with no evidence of disease recurrence. CONCLUSION: Although clear cell carcinoma of the minor salivary glands is a rare tumor, it should be considered as a differential diagnosis in patients with head and neck injuries. Its standardized treatment is surgical; adjuvant treatments have not shown an improvement in the control of recurrence, nor have they improved survival rates.
