Toch is er nog te vaak sprake van vermijdbare complicaties, die extra inspanningen en geld kosten. 1) De meest in het oog springende, zoals decubitus, ondervoeding, incontinentie en valincidenten, zijn sinds 2004 opgenomen in de basisset prestatie-indicatoren zorg. 2) Jaarlijks wordt landelijk in dwarsdoorsnede het vóórkomen van deze problemen gemeten en gerapporteerd in de Landelijke Prevalentiemeting Zorgproblemen (LPZ). 3) Uit de resultaten van 2010 blijkt dat bovenstaande complicaties nog veel voorkomen. Zo was in 2010 gemiddeld één op vier cliënten in een instelling ondervoed. Als in alle fasen van de behandeling aandacht gegeven Meten met beleid Vermijdbare complicaties, zoals ondervoeding, scoren al jaren hoog in de Landelijke Prevalentiemetingen Zorgproblemen. Gemiddeld één op de vier cliënten in zorginstellingen is ondervoed. De gevolgen zijn zichtbaar in bijvoorbeeld trager herstel en een grotere kans op overlijden. Meetinstrumenten kunnen gebruikt worden om de mate van ondervoeding in kaart te brengen en te monitoren. Het biedt meerwaarde aan de gegevensverzameling: omdat beslissingen transparant worden voor de cliënt, collega's en andere zorgaanbieders. Het doel van het gebruik van meetinstrumenten is het verpleegkundig handelen te ondersteunen en de kwaliteit te verhogen.
