INTRODUÇÃOAs reações adversas medicamentosas (RAMs) são definidas pela Organização Mundial de Saúde (OMS) como respostas nocivas e não intencionais a um fármaco e que ocorrem com doses normalmente toleradas pelo Homem.1 Um estudo conduzido em cinco países demonstrou uma incidência de RAMs de cerca de 16,5% em crianças hospitalizadas. 2As toxidermias são os efeitos adversos medicamentosos mais frequentes, tanto em adultos como em crianças, estando estimado que cerca de 2,5% das crianças tratadas com um fármaco e 12,5% das que tomam um antibiótico irão manifestar uma toxidermia.1,3 Felizmente, as manifestações mais graves são raras e estão associadas a taxas de mortalidade mais baixas na idade pediátrica em comparação com o adulto, mas são ainda assim uma importante fonte de morbimortalidade. 4 Durante muitos anos os ensaios clínicos em crianças foram negligenciados e apenas há cerca de uma década surgiu regulamentação europeia que obriga à formulação de um plano de investigação pediátrica para todos os novos fármacos que visam a administração em idade pediátrica. 5De facto, estima-se que cerca de 50% a 90% dos fármacos utilizados em crianças não estão estudados nesta faixa etária.6 Ainda assim, os ensaios clínicos em crianças são neste momento limitados e, portanto, a farmacovigilância tem um papel preponderante na prevenção de efeitos adversos, através dos sistemas de notificação espontâneos.5,7 Contudo, este método é limitado pela falta de notificações quer por parte dos profissionais de saúde quer dos cuidadores. 5,8 É ainda importante notar que as toxidermias em idade pediátrica têm particularidades especiais que as distinguem das toxidermias do adulto. As toxidermias em idade pediátrica são muito frequentes, contudo existem características específicas desta faixa etária pouco estudadas. Estas diferem das toxidermias do adulto em termos de apresentação clínica, fármacos implicados, prognóstico e tratamento. O seu reconhecimento precoce e suspensão do fármaco causador são de extrema importância para diminuição do risco de morbimortalidade.O objetivo deste trabalho é rever as principais características que diferenciam as toxidermias da criança das do adulto, de forma a facilitar o seu reconhecimento e perceber como a sua investigação pode ser melhorada.
