Resumen. La Activación Conductual (AC) se define como un proceso terapéutico focalizado a incrementar conductas que coloquen al paciente en un ambiente que proporcione contingencias reforzantes. Este tipo de terapia está validada empíricamente como tratamiento no farmacológico para la depresión y se caracteriza por ser flexible y estar centrada en las necesidades únicas del paciente. El objetivo de este trabajo fue revisar la literatura disponible relacionada a los efectos de la terapia de activación conductual sobre el Trastorno Depresivo Mayor en pacientes con cáncer. Se realizó una revisión bibliográfica sistemática en siete bases de datos, utilizando el sistema PICO para la identificación de los artículos. Se incluyeron estudios en idioma inglés y español publicados en el período 2000-2016 localizados con los descriptores "depresión", "terapia de activación conductual" y "pacientes con cáncer". Se encontraron un total de 373 artículos, de los cuales sólo diez cumplieron con los criterios establecidos para esta revisión. Los estudios se realizaron en pacientes con cáncer no hospitalizados, atendidos en la consulta externa y la intervención se realizó de manera individualizada. Los estudios reportan la utilidad de la AC para el tratamiento del Trastorno Depresivo Mayor en población oncológica. Se sugiere realizar una evaluación clínica para implementar la AC, considerando: la biología del tumor, el estadio clínico, la modalidad de tratamiento y los efectos adversos derivados del mismo, la toxicidad acumulada y la historia natural de la enfermedad. La Púrpura Trombocitopénica Inmune (PTI) es una enfermedad hemorrágica caracterizada por la destrucción de plaquetas por los anticuerpos. El tratamiento de base es la prednisona, la cual, se asocia con cambios en el estado de ánimo, por eso es un reto para el psicólogo especialista en Medicina Conductual, diferenciarlo de problemas completamente psicológicos. Se trata de una paciente femenina de 20 años con PTI que acude a atención psicológica por “cambios de humor” y deficiente autocontrol emocional, principalmente del enojo, desde que estuvo sometida al tratamiento. Para la evaluación se usó entrevista conductual, observación clínica, IDARE, CES-D y ENA de enojo, así como el Análisis Funcional de la Conducta basado en el Mapa Clínico de Patogénesis. La intervención estuvo compuesta de once sesiones bajo la Terapia Cognitivo Conductual, esta reveló tener buenos resultados y se demostró que la causa del problema era psicológico, por un mal manejo de emociones y déficit de ciertas habilidades. El enojo puede ser una manifestación de ansiedad y depresión.
La conducta ante la enfermedad desempeña un papel fundamental en la búsqueda de atención médica para el diagnóstico y tratamiento, así como en las consecuencias físicas, psicológicas y sociales para el paciente, especialmente cuando se trata de enfermedades que requieren vigilancia continua, como es el caso de los padecimientos hematológicos. La evaluación de la conducta ante la enfermedad permite el diseño de intervenciones multidisciplinarias orientadas a la adaptación al padecimiento, el aumento de la adherencia terapéutica, la disminución de secuelas y la administración de los recursos destinados al cuidado de la salud; de allí la importancia del diseño y desarrollo de instrumentos de medición confiables y válidos, entre los que se encuentra el Cuestionario de Respuestas Conductuales ante la Enfermedad. El objetivo del presente estudio fue adaptar y validar psicométricamente este instrumento para la población mexicana mediante un análisis factorial exploratorio. Se trabajó con jueces expertos y no expertos para la validación del contenido, y con 239 pacientes hematológicos de un hospital de la Ciudad de México para la validación psicométrica y el análisis factorial exploratorio. Se encontró una estructura de quince ítems y cuatro factores con una confiabilidad aceptable, por lo que se concluye que el citado instrumento es una medida que se puede aplicar a pacientes hematológicos; sin embargo, se recomienda revisar exhaustivamente los reactivos y llevar a cabo más estudios de validación psicométrica en la población meta.
