Fetal growth restriction remains a leading cause of preterm birth and neurological disorders in children and is associated with high neonatal and perinatal morbidity and mortality. Fetal growth restriction is strongly associated with preeclampsia, placental insufficiency, and does not tend to decrease in frequency. The terms small for gestational age and fetal growth restriction are similar in terms of fetometry, however, in modern literature, these concepts differ based on blood flow disorders in the mother-placenta-fetus system and perinatal complications. A deep understanding of the multifactorial pathogenesis of early and late fetal growth restriction will allow for developing targeted therapy of placental insufficiency. Determining the role of new placental growth biomarkers plays a significant role in understanding the pathogenesis of placental dysfunction and developing measures to predict placenta-associated pregnancy complications. This review article highlights new approaches to effective fetal growth restriction screening. Recent advances in ultrasound diagnosis of fetal growth restriction provide the basis for multivariate testing that can provide cost-effective screening for placental-associated pregnancy complications, including fetal growth restriction.
This review examines the international role of perinatal protection of a fetus in preservation of the health of the nation. Historical perspectives, present-day developments, and future priorities of perinatal medical science are given. The etiology of decompensated placental insufficiency and intrauterine growth restriction is considered. The authors also considered the epidemiology of premature delivery and perinatal outcomes, the problem of development, diagnosis and obstetric management in critical fetal condition is covered in detail. The current review of literature was conducted in order to advance the theoretical knowledge of specialists on modern complex issues of perinatal medicine. The considered practical aspects will contribute to the improvement of interdisciplinary interaction between clinicians of various specialties in the field of perinatal protection of a fetus and, through this, to the preservation of the health of the nation and the multiplication of future generations.
Аннотация. Нами проведен регрессионно-факторный анализ для оценки значимости каждого из изученных факторов риска формирования СЗРП. Проанализировав все факторы риска, мы определили спектр наиболее прогностически значимых из них. Наиболее важным фактором был уровень ИФР-1, а минимальный -количество бета-ХГЧ, аборты до наступления беременности и самопроизвольные выкидыши в первом триместре при предыдущих беременностях. В группу отрицательных факторов, снижающих риск развития СЗРП, вошли: нормальные уровни β-ХГЧ, витамина D. В группу положительных факторов, повышающих риск СЗРП, вошли: сниженный уровень PAPP-А, нормальные роды в анамнезе, ИФР-1 и наличие прерывания беременности в первом триместре в предыдущих беременностях. Впервые термин "сидром задержки роста плода" появился в 1973 году на Первом между-народном симпозиуме, посвященном этой проблеме. Последние достижения медицины прояснили этиоло-гию и патогенез данного осложнения перинатального периода. Постоянно совершенствуются и методы ме-дикаментозной коррекции плацентарной недостаточ-ности и СЗРП [1,2,3,4]. Однако, при формировании суб-и декомпенсированного состояния плода синдром остается малокурабельным или вообще некурабель-ным состоянием, и зачастую только родоразрешение позволяет спасти жизнь и здоровье внутриутробного пациента [5,6,7,8]. Задержка роста плода осложняет от 5 до 24% беременностей [3,4,5,7], причем отмечается постоянная тенденция к возрастанию данного показа-теля, что может быть обусловлено как реальным воз-растанием числа беременных высокого риска развития перинатальной патологии, так и совершенствованием системы пренатального выявления нарушений разви-тия и роста плода. Следовательно, исход зависит от тщательного дородового наблюдения, выявления групп риска, своевременной профилактики развития данного синдрома, грамотного лечения и своевремен-ного родоразрешения. Поиск наиболее точных мето-дов прогнозирования является первостепенной зада-чей, позволяющей снизить перинатальную заболевае-мость и смертность.Среди множества причин развития синдрома за-держки роста плода ведущее значение принадлежит нарушению плацентации, которое включает наруше-ние ангиогенеза и трансформации сосудов маточно-плацентарного комплекса. Процессы формирования полноценной гемодинамической системы при бере-менности зависят и сами влияют на изменения в крови матери уровня биологически активных соединений. К ним относятся биохимические маркеры первого пери-натального скрининга (PAPP-А, АФП, β-ХГЧ), фак-торы роста (ИФР 1, ФРП, СЭФР и др.) и их рецепторы, несколько других плацентарных маркеров (АDAM-12, РР 13, фетальная ДНК, внеклеточная мРНК, активин А и др.), витамины (наиболее важная роль принадлежит витамину Д). В целом, биомаркеров открыто более 37, но ни один не продемонстрировал высокую точность как одиночный предиктор СЗРП [6,8,9,14].
