Background: Deficits in static and particularly dynamic postural control and force production have frequently been associated with an increased risk of falling in older adults. Objective: The objectives of this study were to investigate the effects of salsa dancing on measures of static/dynamic postural control and leg extensor power in seniors. Methods: Twenty-eight healthy older adults were randomly assigned to an intervention group (INT, n = 14, age 71.6 ± 5.3 years) to conduct an 8-week progressive salsa dancing programme or a control group (CON, n = 14, age 68.9 ± 4.7 years). Static postural control was measured during one-legged stance on a balance platform and dynamic postural control was obtained while walking on an instrumented walkway. Leg extensor power was assessed during a countermovement jump on a force plate. Results: Programme compliance was excellent with participants of the INT group completing 92.5% of the dancing sessions. A tendency towards an improvement in the selected measures of static postural control was observed in the INT group as compared to the CON group. Significant group × test interactions were found for stride velocity, length and time. Post hoc analyses revealed significant increases in stride velocity and length, and concomitant decreases in stride time. However, salsa dancing did not have significant effects on various measures of gait variability and leg extensor power. Conclusion: Salsa proved to be a safe and feasible exercise programme for older adults accompanied with a high adherence rate. Age-related deficits in measures of static and particularly dynamic postural control can be mitigated by salsa dancing in older adults. High physical activity and fitness/mobility levels of our participants could be responsible for the nonsignificant findings in gait variability and leg extensor power.
A high rate of psychiatric comorbidity is found in patients with somatic diseases in general hospitals. Reasons for psychiatric disorders in somatically ill patients are variable and can lead to or be a result of the physical illness or just occur coincidentally. Consultation-liaison services (C/L) assist the physician and the treatment team on internal surgical wards in general hospitals in paying attention to and caring for somatic psychiatric comorbidities with diagnostic, therapeutic and, if appropriate, secondary preventative methods. In this article an overview of the development of C/L psychiatry is given. Furthermore, the importance of particular psychiatric disorders seen by psychiatric C/L services and their treatment is described. Finally, specific aspects of service delivery with regard to collaborative care between general hospital physicians and private practitioners are discussed.

Zur Behandlung von unerfülltem Kinderwunsch stehen heute vielfältige Strategien, Techniken und Medikamente bereit. Ebenso liegt eine Fülle von Studienergebnissen im Bereich der Reproduktionsmedizin vor, die es dem Arzt ermöglichen, seine Empfehlung für Diagnostik und Therapie individuell auf ein betroffenes Paar zuzuschneiden. Begonnen wird immer mit einer sorgfältigen Anamneseerhebung, die beide Partner sowohl individuell betrachtet als auch in der Gesamtschau. Zusammen mit weiteren diagnostischen Befunden wird dann das weitere diagnostische und therapeutische Vorgehen mit dem Paar erörtert. Wichtig bei diesem Prozess ist, dass das Paar durch Kenntnis der Chancen und Risiken der verschiedenen Behandlungsmöglich-keiten in die Lage versetzt wird, den Entscheidungsprozess zu verstehen und selbstbewusst eigene Entscheidungen zu treffen. Dies ist im Bereich der Kinderwunschbehandlung umso wichtiger, da es sich niemals um eine lebensbedrohliche Erkrankung handelt, die eine sofortige und einseitige Behandlung erfordert. Die . Tab. 1 gibt die für die systematische Therapieentscheidung wichtigen Faktoren wieder, die vor therapeutischen Schritten geklärt werden sollten. Bei der Besprechung der Befunde und bei der Ableitung von Empfehlungen ist es wichtig, individuell und realistisch auf die zu erwartenden Gefahren und Risiken einer Behandlung hinzuweisen. Diese Information ist für die betroffenen Paare wichtig bei der gemeinsamen Entscheidungsfindung und der Erwägung von Alternativen. Generell sollten bei der Aufklärung über Risiken und Folgen für die Schwangerschaft nach assistierter Reproduktion die in . Tab. 2 aufgeführten Punkte Erwähnung finden. Zu den Einzelheiten des Fehlbildungsrisikos für die Feten, des Mehrlingsrisikos und des Risikos eines OHSS sei auf die speziellen Beiträge in diesem Heft verwiesen. Operative Risiken der Follikelpunktion Die transvaginale, ultraschallgesteuerte Follikelpunktion wird weltweit seit über 20 Jahren durchgeführt [34]. Der Eingriff wird üblicherweise in Vollnarkose oder Sedierung durchgeführt. Mögliche Risiken bei dem Verfahren sind die Verletzung anderer Organe im Bauchraum, vaginale Blutungen, Unterbauchinfektionen und Komplikationen bei der Narkose. Daten der European Society of Human Reproduction and Embryology (ESHRE) von 2001 haben 235.324 IVF/ICSI-Zyklen in 23 europäischen Ländern auf Zwischenfälle nach der Punktion überprüft. Blutungskomplikationen wurden in 394 Zyklen (0,17%), postoperative Infektionen nach 24 Zyklen (0,01%) beobachtet. Retrospektive Daten des deutschen IVF-Registers von 2000-2004 zu Komplikationen nach Follikelpunktion geben vaginale Blutung in 0,7%, intraabdominelle Blutungen in 0,05%, Darmverletzungen in 0,001% und postoperative Peritonitis in 0,005% der Behandlungen an. Die Rate der Patientinnen, die nach Follikelpunktion stationär behandelt werden mussten, lag bei 0,019% [12].
